Uses
Indications
Albiomin 20 is designated in the following cases:
Emergency treatment cases stun due to hypoglycemia that other methods are ineffective.
Preventing blood concentration, anti -dehydration and electrolytes when severe burns.
In case of reducing blood protein, especially involving too many albumin.
Combined with blood transfusion to treat hypoglycemia in hemolytic disease in newborns.
Combining to dilute blood in heart -lung circulation surgery and in adult respiratory insufficiency syndrome (if calculated the pulmonary capillary is not increasing much).
Albumin preparations who do not play a role in the treatment of kidney failure with kidney failure, chronic cirrhosis, lack of nutrition, despite reducing blood albumin.
Take the drug as directed by the physician.
Pharmacokology
Albumin solution prepared from all blood contains soluble proteins and electrolytes but no blood clotting factors, antibodies or plasma cholinesterase, so it can be transmitted without paying attention to the blood type of the recipient.
The treatment effect of albumin is related to the effect of glue penetration of plasma, which can be held up to 60 - 80% of that glue osmotic pressure, in case the normal blood -blooded albumin concentration (40 - 50g/liter). Transmitting 1g of albumin into the blood can increase the volume of circulating plasma to about 18ml. This additional fluid reduces hematocrit and blood viscosity. Albumin preparations do not contain coagulation factors and do not affect normal coagulation mechanism or increase blood clotting phenomenon.
Mechanism of action
Albumin is the most important protein of serum involved in two main functions: maintaining glue osmotic pressure in plasma, linking and transporting some endogenous or external substances such as bilirubin, steroid hormones and medications in the blood.
pharmacokinetics
Albumin is allocated in non -cell fluid, over 60% in the outer cavity.
Effect time: Only 15 minutes after the 25% albumin injection has increased blood volume for patients to be fully water. The effect of albumin depends on the initial blood volume of the patient. If the amount of blood decreases, the time to increase blood volume will last for hours, if the amount of blood is normal, the effect will be shorter.
Deduction time is about 15-20 days.
Before taking 20% albiomin infusion biotest for emergency treatment cases due to a decrease in blood volume (50ml)
How to use
Dosage, the concentration of the specified albumin preparation and the transmission speed should be adjusted based on the treatment needs of each patient. It is necessary to fully identify the parameters of blood circulation, check the hemodynamics of blood regularly including:
electrolytes.
Sugar used
Albiomin 20% can be used directly with intravenous lines, or can also be mixed in isothermal solution (for example, 0.9% physiological saline solution).
The transmission speed must be adjusted to suit the clinical condition and indicated treatment for each patient.
Albiomin 20% suitable for patients with hemolytic and infant.
The speed of infusion must be adjusted accordingly based on the circulating characteristics of the individual and the indication of treatment.
Dosage
A albiomin preparation concentration of 20% indicated, indicated dose as well as translation speed must be adjusted suitable for each individual patient.
Albumin dose depends on the clinical and response of the patient and is determined by monitoring pulmonary blood pressure, central venous blood pressure while used to avoid excessive blood volume.
Albumin transmission speed is 1 - 2ml/minute (5%solution) or 1ml/minute (25%solution), can increase the speed of transmission in the treatment of shock.
Do not transmit more than 250g/48 hours. If this overdose is needed, it must be replaced by plasma.
When stunned by a decrease in acute blood volume
The initial dose for adults is 25g of albumin (500ml of 5% or 100ml solution of 25%) and for children about 1g/kg. If necessary, repeat a dose after 15 - 30 minutes. When there is hemorrhage, there may be a total blood transfusion.
When reducing blood protein
The maximum dose in 1 day is 2g/kg body weight.
When burned
Dosage and time depending on the burn area, the dose should be sufficient to recover the volume of plasma and reduce blood concentration: 500 ml of 5% or 100ml solution of 25% with electrolyte solutions. To treat non -urgent burns in children, use 6.25 - 12.5g.
When increasing bilirubin blood in infants
Use 1g/kg body weight before transmitting blood (albumin at a dose of 1.5 - 2.5g/100ml of blood can also be added to the blood fluid replacement fluid).
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? As soon as the initial signs of the cardiac burden (such as headache, nausea, intravenous veins, or hypertension, increased pulmonary venous pressure, pulmonary edema), immediately stop the infusion.
What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
Mild reactions such as face, itching, fever and nausea.
Side effects that cause tremor, vomiting, erythema, hypotension, fast vessels, shortness of breath occur in some cases. These reactions often disappear quickly when reducing the speed of infusion or when the transmission stops.
Anaphylaxis may occur but very rare. In case of anaphylactic shock, suspension of infusion and appropriate treatment.
Notify the doctor about unwanted effects when using the drug.
Warnings
Contraindicated
Albiomin 20 50ml Contraindicated for patients with a history of sensitivity to human albumin, patients with severe anemia, patients with heart failure, patients with increased blood volume, pulmonary edema.
Prisable when using
albumin should be used in some pathologies that can cause increased blood volume (along with its harmful effects) or cause blood thinning, such as loss of heart failure, high blood pressure, varicose esophagus, pulmonary bleeding, severe hemorrhage, severe anemia, urology due to kidney and after kidney.
The penetrating glue pressure of albumin 20% is about 4 times higher than that of plasma, so when treating with the hypertonic albumin, it is necessary to calculate to ensure adequate water for patients, prevent excessive volume increases and increase water loss.
Albumin solution 20 - 25% has a relatively lower concentration of electrolytes than the electrolyte concentration of albumin solutions 4 - 5%.
It is necessary to check the patient's electrolyte status as well as take other necessary steps to restore or balance the electrolyte concentration for patients when conducting treatment with albumin.
Do not dilute the albumin solution with distilled water injecting because it can cause hemolysis on patients.
In case of transmission with a relatively large volume of 20%Aumin solution, it is necessary to check the condition of blood clots and hematocrit values of the patient. It is possible to apply the necessary care measures to ensure the full addition of substances to other components of the blood (such as coagulation factors, electrolytes, platelets and red blood cells).
Special warning
can cause increased blood volume if the dose and the infusion rate are not adjusted to suit the circulatory condition of each patient. As soon as one of the first signs of cardiovascular burden such as headache, nausea, carotid veins, or hypertension, increased venous and pulmonary edema, need to stop infusion.
Albiomin 20% is produced from human plasma. It may not completely exclude the possibility of infectious infections when treated with preparations produced from blood or plasma in general. These risks may also be encountered in the case of unknown virus infection or arising strain, or other pathogens.
Standard measures to prevent infections caused when treating with products derived from blood or plasma include selecting blood donors, screening each individual donor blood and blood type to distinguish each strain of bacteria, and inactivated or eliminating viruses. However, despite measures, when using preparations from human blood or serum, pathogens are not completely excluded with pathogens that so far science has not yet been detected.
So far, there has been no report on the case of viral infection when treated with albumin solutions produced according to European pharmacopoeia standards through control of production stages.
Special recommendations: Every treatment with 20% albiomin products for patients should record the lot number of the preparation to ensure the connection between the patient and the used drug batch.
The ability to drive and operate machinery
There is no sign of the influence of Albiomin 20% on the ability to drive and operate machinery.
Pregnancy and lactation
Clinical studies that control the safety of Albiomin 20% when treating for pregnant women have not yet been conducted. However, clinical experimental treatment with albumin shows no harmful effects on pregnant women, fetuses and children after birth.
Animal experimental studies do not have enough data to evaluate the safety of the drug on the reproductive, developing process of embryos or fetuses, on pregnant women as well as the development of the baby before and after birth.
However, it should be noted that albumin is a common ingredient in the blood formula.
Drug interaction
Prkallikrein in some types of preparations that can stimulate kininogen changes in human blood into kinin causing hypotension.
Albumin solution contaminated with aluminum can be toxic to those who have been transmitted a large amount of albumin, leading to bone care and brain disease.
Interaction with food
Food, alcohol and tobacco can interact with certain drugs. Consult your doctor about taking medicine with food, alcohol and tobacco.
Health condition affects albiomin 20
Your health status may affect the use of this drug. Tell your doctor if you have any health problems.
