Uses

indications

Acefalgan 500 is indicated in the following cases:

  • A reduction of fever.
  • Mild to medium analgesic. The drug acts on the central cashew center in the hypothalamus, causing cooling, increasing heat due to vasodilation and increasing peripheral blood flow reduces body temperature in the fever, but rarely reduces normal heat body.

Acetaminophen has no effect on inflammation treatment.

With the treatment dose, acetaminophen metabolizes mainly through a sufat and glucuronid complex reaction. A small amount usually converts into a toxic metabolite, N-acetyl-P-Benzoquinonimin (NAPQI). Napqi is detoxified by glutathion and eliminated into urine and/ or bile.

When the metabolic is not connected to Glutathion, it will be toxic to liver cells and cell necrosis. Acetaminophen is often safe when used for treatment, because the amount of Napqi is formed relatively low and glutathion formed in liver cells that are sufficiently associated with NAPQI. However, when overdose or sometimes with the common doses used in some sensitive people (such as malnutrition, drug interaction, alcoholism, genetics), Napqi concentration can be accumulated toxic in the liver.

    pharmacokinetics

    Absorption:

Acetaminophen when taken will be absorbed quickly and completely. The peak concentration of plasma is about 10-60 minutes after drinking.

    Distribution:

fast and uniform in most body tissues, about 25% acetaminophen in the blood combined with plasma proteins.

    Metabolism:

Acetaminophen is metabolized mainly in the liver in two main paths in the liver: in conjunction with glucuronic acid and associated with sulfuric acid; Combined with sulfuric acid quickly saturated when taking a higher dose but still within the scope of treatment.

The saturation of the glucoronid chemical process only appears when the dose is higher, toxic to the liver.

A small part (less than 4%) is trips by Cytochrom P450 to form a high reaction intermediate (N-acetyl Benzoquinoneimin), under normal usage conditions, this intermediate will be detoxified by glutathion reduction and eliminated in urine after connected with cystein and mercapturic acid. However, when poisoning with high doses of acetaminophen, the amount of toxic metabolites increased.

    Era:

Acetaminophen metabolites are mainly eliminated in urine, in adults, about 90%of the dose is excreted for 24 hours, mainly in the form of glucuronid complex (about 60%) and sulfate complex (about 30%). Less than 5% are eliminated in constant form.

Selling time in plasma is about 2 hours.

    How to use

Acefalgan 500 long tablets for oral, should be taken during or after meals.

    Dosage

    Adults: 1 - 2 capsules x 3-4 times/day, the distance between 2 drinks is at least 4 hours apart, no more than 8 tablets/day.

Children over 12 years: 1 tablet/time, the distance between 2 drinks is at least 4 hours apart, no more than 6 tablets/day.

Children from 8 to 12 years: 1 tablet/time, the distance between 2 drinks is at least 6 hours apart, no more than 4 tablets/day.

Children from 5 - 8 years old: 1/2 tablets/time, the distance between 2 drinks is at least 4 hours apart, no more than 3 tablets/day.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical expert

    What to do when overdose?

Overdose of 10g (150 mg/kg of weight in children) can cause liver cell cancellation, which can lead to absolutely non -recovery necrosis, with manifestation of liver function, brain disease can lead to coma and death. Transaminase in the liver increases, increased bilirubin, decreasing prothrombin.

    Emergency treatment in the hospital:

  • Bowel washing and drinking activated carbon.
  • Use N-acetylcystein, acetaminophen detoxification, intravenously or drink if possible before the 10th hour.

  • without n-acetylcystein, methionine may be used. Also can use activated carbon and/or salt bleach, they have the ability to reduce acetaminophen absorption
  • Help breathing.
  • What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

    Be cautious when used

Be careful in patients with anemia before, impaired liver and kidney function. Overdose or long -term treatment can cause liver necrosis, kidney failure

Drinking plenty of alcohol can cause increased toxicity to the liver of acetaminophen, should avoid or limit drinking alcohol.

For drugs containing acetaminophen: Doctors need to warn patients on signs of serious skin reactions such as Steven-Johnson syndrome (SJS), toxic skin necrosis syndrome (Ten) or Lyell syndrome, acute overseas acne syndrome (AGEP).

    Drug interaction

long -term oral acetaminophen -doses increases the anticoagulant effect of COUMARIN and derivatives.

It is necessary to pay attention to the possibility of serious heat lowering in patients using simultaneously phenothiazine and cooling therapy (such as acetaminophen).

Anti -convulsions (Phenytoin, Barbiturat, Carbamazepin), Isoniazid increases the toxic liver toxicity of acetaminophen.

Probenecid may reduce the elimination of acetaminophen and increase the half -life of plasma of acetaminophen.

Isoniazid and tuberculosis drugs increase the toxicity of acetaminophen for the liver.

metoclopramid, domperidon increases the absorption speed of acetaminophen, cholestyramin reduces the absorption rate of acetaminophen

drinking and long -term alcohol increases acetaminophen's liver toxicity.
