Uses

indications

Agiclovir 400mg drug indicated in the following cases:

  • Treatment of herpes Simplex infection in the skin and mucosa, including genital herpes onset and relapse (excluding heavy HSV infection in children with immunodeficiency). Treatment of chickenpox and herpes zoster (shingles).

    ATC code: J05AB01.

Aciclovir is a nucleosid (acycloguanosin) that has a selective effect on the herpes virus cell.

To have the effect of aciclovir must be phosphoryl turned into activity form as aciclovir triphosphate. At the beginning, aciclovir is converted into aciclovir monophosphate thanks to the enzyme of the virus, thymidinkinase, then transformed into aciclovir diphosphate and triphosphate by some other cell enzymes.

Aciclovir Triphosphate inhibits the virus's DNA synthesis and the multiplication of the virus without affecting the metabolism of normal cells.

The most powerful aciclovir effect on Herpes Simplex Typ 1 virus (HSV-1) and worse in Herpes Simplex Typ 2 virus (HSV-2), Varicella Zoster virus (VZV), the weakest effect on Cytomegalovirus (CMV). Clinically not seeing aciclovir effectively on patients infected CMV.

Epstein Barr virus effect is still unknown. During treatment, some anti -drugs and herpes simplex viruses appeared in non -destructive lymph nodes.

Aciclovir has a good effect in the treatment of severe HSV-1 virus, the mortality rate may be reduced from 70% to 20%. In some patients who are cured, serious complications also decrease. Aciclovir takes a high doses of up to 10 mg/kg body weight, every 8 hours, used for 10-14 days. The encephalitis-the meninges is lighter because HSV-2 can also treat well with aciclovir.

    pharmacokinetics

    absorption:

  • Biological by oral by aciclovir is about 20% (15 - 30%).
  • Food does not affect the absorption of the drug.
  • Aciclovir is widely distributed in body fluids and organs such as brain, kidneys, lungs, intestines, liver, spleen, muscle, uterus, mucosa and vaginal fluid, tears, aquatic fluid, semen, cerebrospinal fluid. Time to reach the peak concentration in plasma after drinking from 1.5 to 2 hours.
  • In patients with normal kidney function, half -life eliminates about 2-3 hours, in patients with chronic kidney failure increases and can reach 19.5 hours in patients with east. (30 - 90% of the dose) is eliminated through the kidneys in unchanged form.

    How to use

Take medicine after meals with plenty of water.

    Dosage

    adults

    Treatment of Herpes Simplex infection:

  • Take 200 mg/time x 5 times/day, each time 4 hours apart by the night.
  • usually 5 days of treatment, in severe contamination cases, the treatment time may last longer.

Should start treatment as soon as possible as soon as infection, for recurrence, it is best to start treatment in the million dong phase or when starting to appear.

Prevention of recurrent Herpes Simplex in patients with normal immunity:

  • Take 200 mg/time x 4 times/day, each time 6 hours apart. Fruit.
  • Take 200 mg/time x 4 times/day, each time 6 hours apart. Beans and herpes zoster:
  • Take 800 mg/time x 5 times/day, each time 4 hours apart, no drug in the evening. The treatment time should last 7 days. Herpes Zoster treatment will bring better results if started as soon as possible after starting a rash.
  • Chickenpox treatment in patients with normal immunity should start within 24 hours after the rash.
  • children

Treatment of Herpes Simplex infection and herpes Simplex infection in immunodeficiency patients:

  • Children ≥ 2 years: Use in adult dose.
  • Children The dose may be more accurate than 20 mg/kg body weight (not exceeding 800 mg) four times daily.

There is no information on the prevention of herpes simplex or treating herpes zoster infection in children with normal immunity.

    * If the dose is different from the content of 400 mg or 200 mg or a child is difficult to take the tablet, it is necessary to consult a doctor or pharmacist for instructions on the use of preparations with appropriate content or dosage.

    Special subjects

    Elderly:

  • Should consider the impaired renal function in the elderly and the dosage must be adjusted accordingly (according to the dosage in the renal failure below).

Be careful when using aciclovir in patients with renal function, the dose must be adjusted according to the clearinine clearance, so patients drink enough water.

Treatment caused by herpes Simplex: Dosage is like normal people, for patients with severe renal impairment (creatinine clearance below 10 ml/minute) should adjust the dose of 200 mg/time x 2 times/day, each 12 hours apart.

Treatment of herpes zoster infection, should adjust the dose according to the level of renal failure as follows:

  • Medium renal failure, Creatinine clearance 10 - 25 ml/min: 800 mg x 3 times/day, each time 8 hours apart. The concentration in the renal tubules exceeds 2.5 mg/ml, or when high serum creatinine, kidney failure, stimulating state, restlessness, tremor, convulsions, chest drums, high blood pressure, difficulty urinating.

Management: Hematopathy of patients until kidney function recovers, stops drugs, gives water and electrolytes.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

  • Side Effects

The harmful reactions are grouped by frequency: very common (ADR ≥ 1/10), common (1/100 ≤ ADR

When taking Aciclovir may experience unwanted effects as follows:

    Common

  • Mental and nervous disorders: headache, dizziness.
  • Skin and subcutaneous tissue: urticaria, increasing the frequency of spreading hair loss. Increasing spread of hair loss is related to a number of diseases and drugs, not sure due to the use of aciclovir.
  • Rare:

  • The immune system: Anaphylaxis.
  • Blood and lymphatic system: Anemia, leukemia, thrombocytopenia. These manifestations are often recovered and often noticeable in patients with renal impairment or other factors.
  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    contraindicated

agiclovir 400mg contraindicated for patients with hypersensitivity to aciclovir or any component of the drug.

    Be cautious when using

Need to provide enough water, especially for high doses of aciclovir.

Patients with severe renal failure and elderly patients:

  • Aciclovir is eliminated through the kidneys, so the dose must be reduced in patients with renal impairment (see the dose part).
  • In cases of reporting, these reactions are generally out of use when stopping the drug.
  • Patients with severe immunodeficiency: When treated with prolonged or repeated aciclovir can lead to selection of viral strains to reduce sensitivity, resulting in unable to continue treating with aciclovir.

Attention should be maintained enough for patients with high doses of aciclovir to avoid kidney poisoning.

The risk of renal failure increases if used simultaneously with other drugs toxic to the kidneys.

The existing data from clinical studies is not enough to conclude that aciclovir treatment reduces the rate of chickenpox -related complications in patients with normal immunity.

Be cautious when taking children's medicine:

  • Aciclovir is used orally used in children mainly to treat non -serious HSV infections in the skin and mucosa.
  • To treat newborns and severe HSV infection in children with immunodeficiency using intravenous aciclovir (IV).
  • This drug contains lactose: Patients with rare genetic problems in tolerance Galactose, Lapp lactase or glucose-Galactose are not used by this drug.

The effect of the drug on driving, operating machinery

No research has shown the effect of aciclovir on driving and operating machinery. Moreover, it is impossible to predict the adverse effects on these activities from the pharmaceutical properties of the active ingredient.

However, be cautious because the drug can cause some unwanted effects on the nervous system such as headache, dizziness, hallucinations, sleeping chicken (often occurring in patients with high doses or kidney failure), patients should certainly not be affected by drugs before driving or using machines.

Use drugs for pregnant and lactating women

    Pregnancy:

  • The existing figures on the use of aciclovir in pregnant women are still limited, and there is no adequate test and good control. Therefore should be careful, should only use aciclovir for pregnant women when the benefits of treatment are more than risks that may occur with the fetus.

    Breastfeeding period:

  • The drug is excreted by oral milk. This concentration can be put into breastfeeding with aciclovir doses up to 0.3 mg/kg/day.

    Interactive drug

Interaction of drugs with other drugs and other types of interactions:

  • Probenecid: Increases half a lifetime in plasma and AUC of aciclovir, reduces excretion through urine and clearance of aciclovir. Aciclovir. Aciclovir.

Cavalry of the drug: Due to no studies on the correspondence of the drug, not mixing this drug with other drugs.

  • Storage

Leave a cool place, avoid light, temperature below 30⁰C.
