Uses
Indications
Agifamcin 300 drug is indicated in the following cases:
Rifampicin is also used in the treatment of severe staph infections such as endocarditis, pneumonia, bacterial infection and myelitis. When resistant to other antibiotics, rifampicin is used with fusidic acid.
Rifampicin is not cross -resistant to antibiotics and other tuberculosis drugs, but the anti -drug strains grow very fast when using lone rifampicin and abuse. Therefore, it is necessary to use rifampicin very strictly to ensure successful treatment.
The mechanism of action of rifampicin is unlike other antibiotics. Rifampicin inhibits the RNA synthetic enzyme activity depending on the DNA of Mycobacterium bacteria and other bacteria by creating sustainable complexes - enzymes.
pharmacokinetic
absorption
Rifampicin is quickly absorbed through the gastrointestinal tract. When taking the dose of 600 mg, after 2–4 hours, the peak concentration in plasma is 7–9 micrograms/ml. Food slows down and reduces drug absorption.
Distribution
The drug is widely distributed into the tissue and fluid, diffusing into the cerebrospinal fluid when the meninges are inflamed. The drug is placed in both placenta and breast milk. The integral is equal to 0.93–1.6 l/kg. Link with plasmaprotein 80%.
Metabolism
Rifampicin metabolizes in the liver. The drug is quickly reduced to acetyl into active metabolites (25-O-Desacethyl-Rifampicin).
Other metabolites that have been identified as rifampin quinon, desacethyl-rifampin quinon and 3-formyl-rifampin.
Elimination
rifampicin excreted through bile, feces and urine and undergoing the gut -liver cycle. 60–65% of dosage dosage.
About 10%, excreted drugs in the form of unchanged urine, 15% are metabolites that are active and 7% are 3-formyl derivatives no longer active.
Rifampicin's sale time at the beginning is 3–5 hours when used repeated, the selling time is reduced to 2–3 hours. Prolonged sale time in people with liver failure.
Before taking Agifamcin 300 Agimexpharm treats all tuberculosis, leprosy (10 blisters x 10 tablets)

Cyeus treatment
Must coordinate with other leprosy medications such as Dapson and Clofazimin.
With a group of patients with many bacteria used continuously 24 months on the following regimen:
Adults aged 15 and older: Rifampicin 600 mg 1 time/month and Clofazimin 300 mg 1 time/month or 50 mg/day x 1 time/week and dapson 100 mg x 1 time/day.
With a group of people with less bacteria used for 6 months as follows:
From 6 to 14 years old: Rifampicin 300–450mg x 1 time/month and Dapson 50–100 mg x 1 time/day.
Prophylactic meningitis
Do Haemophilus Influenzae:

Adults: 600 mg x 2 times/day x 2 days. Treatment of staphylococcus aureus methicillin resistance Vancomycin intravenously 1 g of 1 g of 1 g, accompanied by oral Rifampicin 600 mg x 12 hours/time and oral sodium fusidate 500 mg x 8 hours/time. The treatment is 2–6 weeks, depending on the infection. Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? Take activated carbon to increase the removal of the gastrointestinal tract. The positive urinary will increase the elimination of the drug. Hematoma can be effective in some cases. What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Uncommon, 1/1000 Rare, ADR urinary: severe renal failure. Instructions on how to handle ADR Systemic immune response such as chills, fever, rare during continuous treatment. In the event of a banner or hemorrhage or suddenly reducing the kidney function, it is necessary to stop rifampicin immediately (common in interruption treatment). Reduce kidney function before using rifampicin does not hinder treatment, but it is necessary to reduce the dose (apart drinks). Stop using the drug and notify the doctor the unwanted effects when using the drug.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Agifamcin drugs contraindicated in the following cases:
Porphyrin metabolism disorders in sensitive people, due to a mechanism related to the enzyme induction Cytochrome P450 in the liver.
Precautions when using
for people with liver failure, must monitor liver function during treatment. Rifampicin induction enzyme should be particularly cautious when taking drugs for patients with potential porphyrin metabolism due to Delta-Amino Levulinic acid activation process. Because the enzyme system in premature babies and newborn babies is incomplete, they only use rifampicin for these objects when really necessary.
Use Rifampicin in combination with Isoniazid and Pyrazinamide will increase the toxicity of the liver. Need to consider the risk of complications and treatment needs.
Must notice the patient in advance that feces, urine, saliva, tears, sweat and other fluids of the body will be red while taking the drug to avoid unnecessary worries. Contact lenses can catch permanent color.
The ability to drive and operate machinery
Medicines that cause headaches, dizziness, fatigue, so be careful to use for drivers and machinery operation.
Pregnancy
If using rifampicin for women who are pregnant in the last 3 months, can bleed due to decrease in prothrombin - Mother and infant. So to avoid bleeding, use more vitamin K for pregnant mothers, after birth and babies.
Animal experiments show that Rifampicin is likely to cause bone deformities. So Rifampicin is only for pregnant women when they have carefully considered risks and needs.
Breastfeeding period
rifampicin eliminated through breast milk, but almost no risk to the baby.
Drug interaction
Rifampicin induction enzyme cytochrome P450 should increase metabolism and excretion. Therefore, reducing the effects of some drugs when used simultaneously.
Drugs should be avoided in combination with rifampicin: isradipin, nifedipin and nimodipin.
The following drugs when in combination with rifampicin need to adjust the dose: contraceptive pill, cyclosporin, diazepam, digoxin, anticoagulant drug dicoumarol, disopyramide, doxycycline, phenytoin, glucocorticoids, haloperidol, ketoconazole, erythromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin, clarithromycin Chloramphenicol, Theophyllline, Verapamil ...
Some drugs when used with rifampicin will reduce the absorption of rifampicin such as antacids, benfonite, clofazimin ... fix by drinking separately by 8–12 hours apart.
In addition, isoniazid and liver toxic drugs when used in combination with rifampicin will increase the risk of toxicity to the liver, especially liver failure.
