Pharmacokological
Fexofenadin is a second -generation antihistamine drug, with specific antagonistic and selective effects on peripheral H1 receptors.
The drug is a metabolic substance that is active of terfenadin but no longer toxic to the heart due to not inhibiting potassium channel associated with myocardial cell red.
Fexofenadin has no significant effect on acetylcholine antagonism, dopamine antagonism and has no effect of inhibiting receptors of Alpha1 - or beta -adrenergic. At the treatment dose, the drug does not cause sleep or affects the central nervous system.
The drug has a fast and prolonged effect due to the slow -attached drug into the H1 receptor, forming a sustainable complex and separating slowly.
pharmacokinetics
Good absorption drug when used oral. After taking 1 60mg tablet, the peak concentration in the blood is about 142 nanogam/ml, achieved after 2 to 3 hours. Food reduces the peak concentration in plasma by about 17% but does not slow down the time to reach the peak concentration of the drug.
The ratio of the plasma protein of the drug is 60 to 70% mainly with albumin and alpha1 - Glycoprotein acid. The distribution volume is 5.4 - 5.8 liters/kg. It is unclear whether the drug is through the placenta or excretion in breast milk, but when using terfenadin has detected fexofenadin is the metabolic substance of terfenadin in breast milk. Fexofenadin does not pass the brain barrier.
approximately 5% of the dose of the drug is metabolized. About 0.5 to 1.5% are metabolized in the liver thanks to the cytochrom P450 enzyme system into non -active substances. 3.5% are converted into methyl ester derivatives, mainly thanks to the bacterial system.
Half of Fexofenadin elimination is about 14.4 hours, longer in renal failure. The drug is excreted mainly in feces (about 80%), 11 to 12% of the dose is eliminated in the urine in the form of constant.

The effect of the drug on the ability to drive and operate machinery
Although Fexofenadin is less drowsy, still need to be cautious when driving or operating machinery.
Drug interaction
erythromycin and ketoconazole increases the concentration and area under the fexofenadin -level concentration curve in the blood, the mechanism may be due to increased absorption and reduction of this drug elimination. However, the interaction does not mean clinically.
Antacomicisted with aluminum, Magnesi if used simultaneously with fexofenadin will reduce the absorption of the drug, so they must use these drugs about 2 hours apart.
