Uses

indications

Aginalxic drug is indicated in the following cases:

  • The lower urinary tract infection has no complications due to gram -negative bacteria, except for Pseudomonas. More to treat Shigella infections. Enerobacter is often sensitive to drugs. However, drug resistance occurred.

Pseudomonas Aeruginosa, Gram -positive bacteria (Enterococcus and Staphylococcus), anaerobic bacteria are often resistant to Nalidixic acid.

Most of the acute and chronic urinary tract infections are caused by gram -negative intestinal bacteria. Therefore, nalidixic acid is often used to treat urinary tract infections.

Enterococcus and Staphylococcus saprophyticus, the main cause of urinary tract infections, resistance to Nalidixic acid.

Nalidixic acid does not affect intestinal bacteria, this is important to keep the ecological balance of the intestinal bacteria.

Nalidixic acid interferes with the copy process of bacterial DNA by inhibiting the activity of DNA Gyrase (Topoisomerase).

Anti -drug: resistant to drugs occurs quickly, sometimes within the first few days of treatment, but not spreading or through intermediaries R - Plasmid. Cross -resistant to oxolinic acid and cinoxacin. Dysentental and anti -typhoid chloramphenicol/sulfamethoxazol/ampicillin is still sensitive to nalidixic acid.

    Dynamic pharmacokinetics

Nalixic acid absorbs quickly and almost completely from the digestive tract and reaches the peak concentration of 20 - 50 micrograms/ml, 2 hours after taking the dose of 1g. Half a life of plasma about 1 - 2.5 hours.

Nalidixic acid converts partially into nalidixic hydroxy acid, has antibacterial effects like nalidixic acid and corresponding to about 30% of the effect of the drug in the blood. About 93% of nalidixic acid and 63% of nalidixic hydroxy acid binds to plasma proteins.

Both nalidixic acid and nalidixic hydroxy acid metabolize rapidly into glucuronid and dicarboxylic derivatives. Often only detected inactive metabolites are carboxynalidixic acid in the urine.

Nalidixic acid and metabolites are quickly excreted through urine within 24 hours. About 80-90% of the drug excreted in the urine is the metabolites that do not work, but the concentration of urine in the urine of the drug is unchanged and the metabolites work at about 25 - 250 micrograms/ml, after taking the dose of 1g (most sensitive bacteria are inhibited at 16 micrograms/ml).

Hydroxy acid Nalidixic accounts for 80 - 85% of the effect in urine. Probenecid reduces the excretion of drugs through urine. Nalidixic acid through the placenta and breast milk very little. About 4% of the dose excreted through feces.

    How to use

Aginalxic drug used oral. Take medicine when hungry, 1 hour before eating.

    Dosage

Children from 3 months to 12 years: 50 - 55 mg/kg/day, divided into 4 times. If the treatment is prolonged, the dose of 30-33 mg/kg/day should be used. If the child does not take the tablet, use the appropriate form for children.

Adults: Take 2 capsules/time x 4 times/day, use for at least 7 days, if the treatment continues to last for more than 2 weeks, a dose must be reduced to half.

If creatinine clearance is over 20 ml/min, the normal dose can be used.

If the creatinine clearance is less than 20 ml/minute serum creatinine above 360 micromol/liter, the dose for 24 hours must be considered to reduce to 2 g.

Despite the antibacterial effect of nalidixic acid is not affected by urine pH, simultaneously used with sodium bicarbonate or sodium citrate may increase the concentration of nalidixic acid in urine.

When combined with nalidixic acid and sodium citrate, the dose of nalidixic acid for adults is 1 tablet/time x 3 times/day, used for 3 days.

Because drug resistance occurs quickly, nalidixic acid should be replaced without the effect of bacterial culture in 48 -hour urine.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?

Treatment: Gastrointestinal lavage when using medication. If the drug has been absorbed, the infusion should be used and used supports such as oxygen and artificial respiration. Anti -seizure therapy can be indicated in very serious cases.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using Aginalxic drugs, you may experience unwanted effects (ADR).

There is no exact proportion of beneficial reactions of drugs based on clinical trials. However, nalidixic acid is often well tolerated and the reaction is often mild.

    Common

    : ADR> 1/100

  • Body: headache.
  • digestive: nausea, vomiting, diarrhea.
  • Eyes: blurred vision, double look, non -standard color.
  • Skin: Light poisoning reaction to blisters in case of sun exposure or after treatment.

    Uncommon: 1/1000

  • Central nervous system: Increase of special intracranial pressure in young children.
  • skin: urticaria, itching, contrast.

    Rare: ADR

  • Central nerve: Confusion, hallucinations, nightmares.
  • Body: Anaphylaxis. blood: platelet decreased, leukopenia, Eosin hyperpens, hemolytic anemia, especially in people with a deficiency of Glucose 6 phosphate dehydrogenase.

    Other: Evana, joint pain.

Instructions on how to handle ADR

Stop taking the drug if the patient shows allergies, anxiety, hallucinations, and convulsions. Use Adrenalin, Glucocorticoid, oxygen when people are sensitive to drugs.

Notify the doctor with unwanted effects when using the drug.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    contraindicated

Aginalxic drug contraindicated in the following cases:

  • Kidney failure, hematopoietic disorders (anemia), epilepsy, increased intracranial pressure.
  • Children under 3 months of age.
  • Hypersensitivity to nalidixic acid or other quinolons.

  • Not used for pregnant women.
  • Be cautious when used

The risk of special drug accumulation is found in the case of reducing kidney function, liver function and G-6-PD enzyme deficiency. Avoid using drugs for children under 3 months old because nalidixic acid and related drugs cause degeneration of joints carrying body weight in adult animals. Avoid direct sunlight during treatment.

The ability to drive and operate machinery

The drug can cause headaches, hallucinations and seizures, so be cautious for trainers and operate machinery.

    Pregnancy

Nalidixic acid passes through the placenta. There are no complete studies and have been closely checked in humans. However, nalidixic acid and related compounds have caused joint diseases in young animals, so nalidixic acid should not be used during pregnancy.

    breastfeeding period

Nalidixic acid excreted into breast milk with low concentrations. There are young children with G6PD enzyme deficient in hemolytic anemia. However, most of them have no problem. Nalidixic acid can be used for women who are breastfeeding.

    Medicine interaction

Theophylin concentration in plasma increases when used simultaneously with nalidixic acid. Nalidixic acid can increase caffeine levels due to the metabolism of caffeine.

Nalidixic acid increases the effects of warfarin and derivatives; Nalidixic acid also increases cyclosporin levels in plasma.

Stomach antacids containing magnesi, aluminum, calcium, sucralfate and 2 or 3 valence cations such as zinc, iron can reduce absorption of nalidixic acid, leading to reducing the concentration of nalidixic acid in urine a lot. Nitrofurantoin reduces the treatment effect of nalidixic acid.

    Storage

Leave a cool place, avoid light, temperature below 30⁰C.
