Uses

Indications

Amlodipine 5mg drug is indicated/used in the following cases:

  • Treatment of hypertension.
  • Treatment of stable angina. The cell membrane in the myocardial cells and blood vessel muscles.

Amlodipine's blood pressure lowering mechanism is due to the effect of direct relaxation of blood vessel muscles. Amlodipine's exact mechanism has not been fully determined, but Amlodipine reduces the overall anemia burden due to the following two effects: (1) Amlodipine relaxes peripheral arteries and thus reduces the overall peripheral resistance to the heart (reducing the burden). Because the heart rate does not change, this burden reduces the heart to reduce energy consumption and oxygen needs of the heart muscle. (2) Amlodipine action mechanism may also include the relaxation of the main coronary arteries and coronary arteries, both in anemia and normal areas. This vasodilation increases the supply of oxygen to the heart muscle in patients with coronary artery spasms (Prinzmetal angina or angina variables) and reduces coronary spasms for smoking.

In patients with hypertension, the daily dose significantly reduces clinical blood pressure both in the lying position and stands for 24 hours. Due to the slow starting characteristic, Amlodipin avoids acute hypotension when used.

In patients with angina daily use amlodipine increases the overall exertion time, increases the starting time of angina pain and increases the difference of the ST segment to 1mm, reducing both the frequency of angina and the number of nitroglycerin tablets must be used.

Amlodipine is not related to any adverse effects on metabolic or changes in lipids and can be used in patients with asthma, diabetes and gout.

    pharmacokinetic

    absorption

After taking treatment doses, Amlodipin is well absorbed with peak concentration in plasma reaching about 6 to 12 hours after drinking. Absolute bioavailability is estimated at 64 - 80%. The peak concentration in the blood reached after 6 - 12 hours of taking the drug. Amlodipine absorption is not affected by food.

    Distribution

The distribution volume is 21 liters/kg, about 97.5% amlodipine in the circulation attached to plasma proteins.

    Metabolism

amlodipine is metabolized mostly in the liver into non -active metabolites with 10% of the original active ingredient.

    Elimination

Amlodipin's plasma semi -discharged time ranges from 35 to 50 hours and is suitable for daily dosage daily. 60% of metabolites are eliminated through urine.

    How to use

    Oral drugs.

    Dosage

    Adults:

Hypertension and angina, the normal starting dose is 5 mg/time/day, may increase to a maximum dose of 10 mg/time/day depending on the response of each patient.

No need to adjust the dose of amlodipine in case of use in combination with thiazid diuretics, beta inhibitors or enzyme inhibitors Angiotensin.

    Use in children:

Children 6 - 17 years old treat high blood pressure: 2.5 - 5 mg/time/day. The dose exceeds 5 mg daily has not been studied in patients with this age.

Children under 6 years old: No research data.

    Used in the elderly:

Amlodipin is equally tolerated in both elderly and young patients when using the drug in the same dosage in the elderly and young. Should be able to use the usual dose regimen for the elderly.

Used in patients with liver function:

The recommended dose is not set in patients with mild to moderate liver failure. Amlodipine pharmacokinetics have not been studied in patients with severe liver failure. Amlodipine should be started at the lowest dose and should be careful to use the drug when starting treatment and when increasing the dose in patients with severe liver failure.

Used in patients with renal failure:

The change of amlodipine concentration in plasma is not correlated with the degree of renal failure. Although recommended to use amlodipine with normal doses for these patients.

Note: There is no special requirement on drug treatment after use.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose?

    Overdose

The data suggests that overdose in large quantities can cause peripheral vessels and may be accompanied by tachycardia. There have been reports on prolonged blood pressure drops and including death shock.

    Handling

Amlodipine absorption is significantly reduced when indicating activated carbon at that time or until 2 hours after taking Amlodipin 10 mg in healthy volunteers. In some cases, the stomach may need. In cases of severe amlodipine hypotension, the positive cardiovascular support measures are required, including regular monitoring of cardiovascular and respiratory functions, high limbs and pay attention to the volume of circulation and urine.

Can use a vasoconstrictor to recover blood vessel tone and blood pressure, as long as the patient does not have the contraindication to use the drug. Calcium gluconate can be injured to reverse the effect of calcium channel blockers. Because amlodipine is heavily connected to plasma proteins, the divergence does not bring results in this case.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

The safety and effectiveness of amlodipine in hypertension is not yet set.

Used in patients with heart failure

In a long -term research with placebo (Praise - 2) using amlodipine in patients with heart failure due to anemia, III and IV according to NYHA's classification (New York -US Heart Association), Amlodipine is reported to be related to an increase in pulmonary edema ratio. However, there is no significant difference in the proportion of patients with severe heart failure in the amlodipine group compared to the placebo group.

Used in patients with impaired liver function

Like all other calcium channel blockers, Amlodipine's waste time is prolonged in patients with liver function impairment and recommended doses for these patients have not been set up. Therefore amlodipin needs to be used cautiously in this patient.

    Elderly

Be careful when increasing the dose in the elderly.

    Patients with renal failure

can use amlodipine with normal doses for these patients. The change of amlodipine concentration in plasma is not related to the level of renal failure. Amlodipine is not excluded from plasma by a cup.

The ability to drive and operate machinery

Amlodipine may have a small or moderate effect on the ability to drive and operate machinery. In patients with dizziness, headache, fatigue or nausea can affect driving and operating machinery. Be careful to use drugs for drivers and operate machinery at the beginning.

    Pregnancy

Amlodipine safety in pregnant women has not been set. Amlodipine is not toxic in animal reproductive studies, except for slowing labor and prolonging mouse delivery time with a dose of 50 times higher than the recommended dose for humans. Therefore, Amlodipine should only be used during pregnancy when there is no safer replacement measure and when the disease itself contains greater risks for mothers and embryos.

    Breastfeeding period

There is no research document confirming whether Amlodipin is excreted in breast milk or not. Based on the benefits of breastfeeding and the benefits of amlodipine treatment for the mother to decide the use of amlodipine.

    Drug interaction

Amlodipin has been shown to be safe for thiazid diuretics, alpha receptor inhibitors, beta receptor inhibitors, angiotensin transferring enzymes, long -lasting nitrate, nitroglycerin under the tongue, anti -inflammatory drugs, antibiotics and antibiotics Blood.

Intro data from plasma studies have shown that amlodipin does not affect the cohesion of plasma proteins of research drugs (digoxin, phenytoin, warfarin or indomethacin)

In the following studies, there are no significant changes in pharmacokinetics of amlodipine or other drugs in the study when used in combination.

The effect of other drugs on amlodipin

    CYP3A4 inhibitors

Use amlodipine simultaneously with strong or medium CYP3A4 inhibitors (protease inhibitors, Azol antifungal drugs, macrolid antibiotics such as erythromycin or clarithromycin, verapamil or diltiazem) can increase the appearance of amlodipine, leading to an increased risk of blood pressure lowering blood pressure. The clinical change of these pharmacokinetic changes can be more evident in the elderly. Therefore, clinical monitoring and dose adjustment.

    Clarithromycin

is CYP3A4 inhibitor. That increases the risk of hypotension in patients using clarithromycin along with amlodipine, need to closely monitor patients when using simultaneously with amlodipine with clarithromycin.

    CYP3A4 induction drug

There is no data on the effects of the CYP3A4 induction drug on Amlodipine. The simultaneous use of CYP3A4 induction drugs (e.g. Rifampicin, Hypericum Perforatum) can reduce amlodipine levels in plasma. Amlodipin should be used cautiously with CYP3A4 induction drugs.

    dantrolene (transmission)

In animals, ventricular vibration and cardiovascular collapse are related to hyperkalemia after using Verapamil and Dantrolene intravenously. Due to the risk of hyperkalemia, it is recommended that avoiding simultaneous use of calcium channel blockers such as amlodipine in patients who are prone to malignant body hyperplasia and in the treatment of malignant body temperature.

    cimetidine

Simultaneous use amlodipine and cimetidine does not change the pharmacokinetics of amlodipine.

    Grapefruit juice beam

Do not use amlodipine simultaneously with grapefruit or grapefruit juice because bioavailability can increase in some patients leading to increased hypotension.

    Sildenafil

Use a single dose of 100mg of Sildenafil in objects with idiopathic hypertension does not affect the pharmacokinetic parameters of Amlodipin. When used in combination with amlodipine and sildenafil, each drug has its own lowering effect.

    Aluminum salt/magnesium salt (antacids)

Concentrated use of antacids is aluminum salt and magnesium salt with a single dose of amlodipine does not significantly affect the pharmacokinetics of amlodipine.

The effect of amlodipine on other drugs

    Hematoplasty drugs

Amlodipine increases the hypotension effect of other antihypertensive drugs simvastatin. Simultaneous use of 10mg of Amlodipin with 80mg of simmvastatin results increases 77% of Simvastatin concentration compared to only Simvastatin. The limit of simvastatin dose in patients using amlodipine is 20mg daily.

    Atorvastatin

Soonered 10mg of amlodipin with 80mg atorvastatin does not significantly change the pharmacokinetic parameters of Atorvastatin in a state of plasma concentration.

    digoxin

Simultaneous use of amlodipine with digoxin does not change the concentration of digoxin in plasma or the renal clearance of digoxin in normal volunteers.

    Ethanol (alcohol)

Use single -dose or multi -dose 10mg of amlodipine does not significantly affect the pharmacokinetics of ethanol.

    indomethacin

In vitro data from human plasma studies has shown that amlodipine does not affect the cohesion of the plasma protein indomethacin

    phenytoin

amlodipin does not affect the cohesion of phenytoin plasma proteins

    warfarin

Simultaneous use amlodipine with warfarin does not change the time of prothrombin inhibitors of warfarin.

    cyclosporin

Pharmacy studies with cyclosporin have shown that Amlodipin does not significantly change the pharmacokinetics of cyclosporin. There has been no newspaper interaction between amlodipine and cyclosporin on healthy volunteers and other patients except for patients with kidney implants.

    tacrolimus

There is a risk of increasing tacrolimus concentration when combined with amlodipine. To avoid the toxicity of tacrolimus, use amlodipine in patients using tacrolimus to monitor the concentration of tacrolimus in the blood and adjust the doses of tacrolimus to suit.
