Uses

Indications

Ampicillin 500mg drugs are indicated in the following cases:

  • Treatment of upper respiratory tract infections caused by Streptococcus Pyogenes, Streptococcus Pneumoniae, Haemophilus Influenzae.
  • Gonorrhea treatment due to gonococcus has not been resistant to penicillins, often ampicillin or ampicillin + problemecid (taking advantage of interactions reduces penicillin elimination).
  • Treatment of Listeria infection (or Listeria monocytogenes).

    Pharmacokology

ampicilin is a beta-lactam antibiotic, penicillin group A. Similar to penicillin, ampicilin that affects the multiplication of bacteria, inhibits peptidoglycan synthesis of bacteric cell membranes.

    antibacterial spectrum

Ampicilin is a broad -spectrum antibiotic that works on both gram -positive and gram -negative bacteria: Streptococcus, Pneumococcus and Staphylococcus without penicillinase. However, the effect on the streptococcus beta and pneumococcus is lower than the benzyl penicillin.

Ampicilin also works on meningococcus and gonococcus. Ampicilin has a good effect on some gram -negative bacteria such as E. Coli, Proteus Mirabilis, Shigella, Salmonella. With antibacterial spectroscopy and pharmacokinetic properties, ampicillin is often chosen as a treatment for urinary tract infections, cholecystitis and cholangitis, endocarditis, gastritis - gastritis, gonorrhea, meningitis.

Ampicilin does not work on some gram -negative bacteria such as: Pseudomonas, Klebsiella, Proteus. Ampicilin also does not work on anaerobic bacteria (bacteroides) causing abscess and infection wounds in the abdominal cavity. In severe bacterial infections from the intestine or vagina, ampicilin is replaced by a combination of clindamycin + aminoglycosides or metronidazol + aminoglycosides or metronidazol + cephalosporin.

The following is the sensitivity of some bacteria for ampicillin (according to the clinical medicine Institute of Tropical Diseases, 2000 - 2003):

    pharmacokinetics

oral dose of 0.5g, after 2 hours there will be peak concentration in serum about 3mg/liter. In pregnant women, the serum concentration is only 50% compared to non -pregnant women, because in pregnant women, ampicillin has a much larger distribution and ampicillin diffuses through the placenta during the fetal circulation and into amniotic fluid.

ampicilin does not pass the blood - brain barrier. In pus meningitis, due to the inflammation of the blood - the brain and the permeability changes, the ampicillin is usually penetrated into the central nervous system enough to have the effect, and the medium concentration of the medication in the cerebrospinal fluid reaches 10 - 35% of the serum concentration.

Ampicilin is excreted quickly as Benzył penicillin, mainly through the renal tubules (80%) and bile ducts. For gonorrhea, it is possible to take a single dose of ampicillin along with probenecid, this drug reduces ampicillin elimination through the renal tubules.

About 20% ampicillin binds to plasma proteins.

ampicilin excrete small amounts through milk. Ampicillin is 40% eliminated when bloody.

Before taking Ampicillin Capsules BP 500mg Brawn treats infection (10 blisters x 10 tablets)

    How to use

Take oral, avoid taking medicine for meals, drink 30 minutes before meals or 2 hours after meals because food affects the efficiency of ampicilin.

    Dosage

Ampicillin dose depends on the type of disease, severity, age and kidney function of the patient. The dose is reduced in severe kidney failure.

    Adults

Oral dose is usually 1g, 4g/day, divided evenly every 6 hours. With serious illness, can drink 6 - 12 g/day.

To treat uncomplicated gonorrhea due to ampicilin sensitive strains

Take the dose of 2.0 - 3.5g, combined with 1g of Probenecid, take a single dose. For women can repeat once if needed.

    Treatment time

depends on the type and level of infection. For most infections, except gonorrhea, continue treating at least 48 - 72 hours after the patient is over.

    Renal failure

Creatinine clearance 30ml/min or higher: No need to change the usual doses in adults.

Creatinine clearance 10ml/min or below: Give usual dose every 8 hours. Patients with dialysis must take an additional dose of ampicillin after each time of appraisal.

    Urinary tract infections

    Doser 50 -100mg/kg/day, 6 hours/time.

    Children

Children should use other dosage forms that are more suitable for the dose.

    Respiratory or skin infections

    Children weigh no more than 40 kg: 25 - 50mg/kg/day equally divided, 6 hours/time.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when using overdose? If an overdose occurs, medication must be discontinued and used to support symptomatic treatment.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using Ampicillin 500mg, you may experience unwanted effects (ADR).

    Common, ADR> 1/100

  • Digestive: diarrhea.
  • Skin: Red rash).

    Uncommon, 1/1000

  • Blood: Anemia, thrombocytopenia, Eosin hyperpens, leukopenia and grain leukocytes.
  • Gastrointestinal tract: Gastritis, stomatitis, nausea, vomiting, colitis, fake colitis, diarrhea.
  • skin: urticaria.

    Rare, ADR

  • Systemic: Anaphylaxis reactions, flaking dermatitis and diverse erythema.
  • The "late" skin allergic reaction is more common during ampicillin treatment than when treated with other penicillins (2 - 3%). If a person with mononulocytosis is treated with ampicillin, 7-12 days after the beginning of treatment, almost 100% of the patients often have very strong red rash. These reactions are also seen in patients with mononoculatoculus hypernagination caused by cytomegalovirus and lymphocytes. Patients with single and single leukemia have responded well to other penicillins (except ampicilin esters such as pivampicillin, bacampicillin).
  • Interstitial nephritis, increase AST.
  • Instructions on how to handle ADR

Notify the doctor with unwanted effects when used.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Ampicillin 500mg contraindicated drug in the following cases:

  • Patients with penicillin.
  • Be cautious when used

cross allergy with penicillin or cephalosporin. Do not use or absolutely cautious use ampicilin for people who have been sensitive to cephalosporin.

If the long -term treatment time is required, the liver and kidney must be periodically checked. Previously investigated to see if patients were allergic to penicillin, cephalosporin and other allergies. If the allergic reaction occurs such as: urticaria, anaphylaxis, Steven - Johnson syndrome, it is necessary to immediately stop the ampicillin therapy and immediately indicated treatment with epinephrin (adrenalin) and never be treated with penicillin and cephalosporin anymore.

The ability to drive and operate machinery

No report on the effect of drugs on driving and operating machinery.

    Pregnancy

There is no harmful reaction to the fetus.

    Breastfeeding period

ampicillin into milk but does not cause harmful reactions for breastfed babies if used in normal treatment.

    Drug interaction

Food reduces the speed and level of ampicillin absorption.

Penicilin both interact with methotrexate and probenecid. Ampicillin interacts with allopurinol. In patients using ampicillin or amoxicillin along with allopurinol, the ability to redness increases. Bacterial antibiotics such as chloramphenicol, tetracylin, erythromycin reduces the bactericidal ability of ampicillin and amoxicilin.

Ampicillin may falsify the results of the glucose test using copper sulphat (such as Benedict, Clinitest).

    Storage

    Store at a temperature not exceeding 30 ° C.
