Uses
indications
Aronatboston is indicated for treatment and preventing osteoporosis in postmenopausal women, preventing fractures, including hip joints, wrists and vertebrae (broken vertebrae).
Pharmacokology
In animal studies, Alendronat is a bisphosphonate mounted mainly on bone pockets, especially under the cancellation of the cell, inhibiting the activity of the cell cancellation without direct effect on the bone creation process. Because bones and osteoporosis always go hand in hand, the bone formation process decreases but slower than the bone resorption process leads to an increase in bone mass. When in contact with alendronat, the normal bone will push Alendronat into the substrate, where they have no pharmacological activity.
Alendronic acid daily after menopause (Alendronic Acid) with different doses that produce biochemical changes showing bone inhibition depending on the dose, including reducing calcium urinary and indicators of bone collagen decreased in urine.
Alendronat inhibits the progression of osteoporosis regardless of age, racial, basic rotation speed of bones, kidney function and when used with other common drugs.
Pharmacokinetics
absorption
Alendronat's oral bioavailability in women is 0.64% equivalent to men (0.6%), with a dose of 5 - 70 mg when used after 1 night without eating and 2 hours before standard breakfast. When Alendronat is used an hour or half an hour before standard breakfast, bioavailability will decrease equivalent (about 40%). Thus, Alendronat is effective when drinking at least half an hour before the first meal of the day.
Drinking Alendronat with coffee or orange juice will reduce about 60% of the bioavailability of alendronat.
Distribution
Research on rats shows that alendronat is temporarily distributed into soft tissues after intravenous injection of 1 mg/kg, but then quickly distributed into the bone or eliminated through the urine. The distribution volume is in medium stable state, except for bones, at least 28 liters in humans. Alendronate is attached to plasma proteins in humans about 78%.
Metabolism
There is no evidence that alendronat has been metabolized in humans.
Elimination
The clearance of the kidneys in the kidney is 71 ml/min and the whole body does not exceed 200 ml/min.
Alendronat does not eliminate through acid or basic transportation systems in the kidneys of rats, so there is no interaction with the excretion of other drugs through this transport system in humans. Pre -clinical studies show that the drug does not deposit in the bone, but is quickly exported to the urine.
There is no clinical information but may be in patients with renal impairment that accumulates alendronate in the slightly high bone (see the dose part).
Before taking Aronatboston 70mg Boston treats and prevents osteoporosis in postmenopausal women (1 blister x 2 tablets)
How to use
Take the pills at least 30 minutes before eating, taking or taking the first day of the day with normal water.
To quickly put the drug down to the stomach, reduce the potential for local and esophagus irritation:
Dosage
recommended dose
Each time taking 1 tablet 70 mg, only once a week.
Children
drugs are not for children because they have not been studied.
Elderly
No need to adjust the dose for the elderly or mild to moderate renal failure (creatinine purification from 35 - 60 ml/minute). Do not recommend using Alendronat for people with severe renal failure.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
Overdose
There is no special information on overdose treatment with Alendronat. Overdose can cause blood calcium loss, hypoclapiasia, adverse effects in the gastrointestinal tract such as digestive disorders, heartburn, esophagitis, inflammation or stomach ulcers.
How to handle
Need to drink milk or antacids to combine with alendonat. Due to the risk of esophageal irritation, do not cause vomiting and patients need to be completely vertical.
What to do when you forget 1 dose? Do not take two capsules in the same day, but return to drink a single tablet every week on the day of the week as the original chosen calendar.
Side Effects
When using Aronatboston, you may experience unwanted effects (ADR).
In clinical studies, harmful reactions due to the use of alendronat are usually mild and generally do not need to stop the drug. Mainly unwanted effects in the digestive tract.
Common, ADR> 1/100
Uncommon, 1/1000 Instructions on how to handle ADR Notify the doctor with unwanted effects when using the drug.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
contraindicated
Aronatboston drugs are contraindicated in the following cases:
Norms such as narrowing or losing elasticity of the esophagus leads to slow emptiness of the esophagus.
Be cautious when using
need to be alert to any signs or symptoms that signal the esophageal reaction and need to stop taking the drug, if it is difficult to swallow, swallow pain, pain behind the sternum, heartburn appears or worsen. There have been reports on harmful reactions in the esophagus, such as esophagitis, esophagus and esophageal ulcer, rarely leading to stenosis or perforation of esophagus in patients treated with alendronat.
Be cautious with people with the above gastrointestinal problems such as: difficulty swallowing, esophagus, inflammation or stomach ulcers, duodenum because the drug can irritate the above gastrointestinal mucosa and make it worse.
People with tooth extractions and/or local infections have been seen for a long time (although rarely) when using oral bisphosphonate can cause local jaw bone necrosis.
Do not use medication for patients with creatinine clearance
It is necessary to consider the causes of osteoporosis other than the deficiency of estrogen, high age and use glucocorticoid.
Must treat blood calcium loss and metabolic disorders of other minerals before treatment with alendronat.
The ability to drive and operate machinery
Caution when taking drugs for drivers or operating machinery.
pregnancy
aronatboston has not been studied in pregnant women, so it is not used for these cases.
The period of breastfeeding
aronatboston has not been studied in nursing women, so it is not used for these cases.
Drug interaction
estrogen: Safe and effective of simultaneous use of hormone replacement therapy and alendronat for postmenopausal women have not been determined, so it is not recommended this simultaneous usage.
Calcium supplements and anti -acid drugs can reduce the absorption of alendronat. Therefore, patients have to wait at least half an hour after taking Alendronat to use any other drug.
Aspirin: In clinical studies, the proportion of unwanted reactions in the gastrointestinal tract, increased in patients with simultaneous use of alendronat, dose greater than 10 mg/day, with aspirin compounds.
Non -steroid anti -inflammatory (NSAID): Alendronat can be used in patients who are taking NSAID drugs. However, NSAID drugs often cause gastrointestinal irritation, so they must be cautious when used with Alendronat.
Storage
Store in a dry place, below 30 ° C, avoid light.
