Indications
AuclanityL 500/125mg drug is indicated for infections caused by sensitive bacteria in the following cases:
Severe upper respiratory infections: tonsillitis, sinusitis, otitis media have been treated with common antibiotics but does not decrease.
Lower respiratory infections by H.influenzae strains and Branhamella CatVrbalis produce beta lactamase: Acute bronchitis and acute batch of chronic bronchitis, staging pneumonia in the community.
Heavy urinary tract infections by E. coli strains, Klebsiella and Enerobacter produce: cystitis, urethritis, pyelonephritis.
Skin and soft tissue infections: pimples, abscesses, wound infections.
Bone and joint infections: Bone inflammation.
Dental infections: Total abscess.
Other infections: Obstetrics and gynecology, abdominal cavity.
Clavulanic acid due to the fermentation of Streptomyces Clavuligerus, which has a beta - lactam structure similar to penicilin, has the ability to inhibit beta - lactamase due to the majority of Gram -negative bacteria and Staphylococcus. Especially, it has a strong inhibition effect of beta - lactamase transmitted via plasmid that resists penicillins and cephalosporins. Clavulanic acid itself has very weak antibacterial effects.
Clavulanic acid helps amoxicillin not destroyed by beta-lactamase, and expanding the antibacterial spectrum of amoxicillin effectively to many common bacteria that have resisted amoxicillin, resistant penicillins and other cephalosporins.
Pharmacokology
Pharmacological group: antibiotics.
Amoxicillin is a semi -synthetic antibiotic belonging to the Beta - Lactam family that has a broad bactericidal spectrum for many gram -positive and gram -negative bacteria due to inhibition of bacterial cell wall inhibitors. But because Amoxicilin is easily decomposed by beta - lactamase, therefore does not work for bacterial strains producing these enzymes (many of En
pharmacokinetic
amoxicillin and clavulanic acid both absorbed easily by oral. The concentration of these 2 substances in serum reaches maximum after 1-2 hours of taking the medicine. With a dose of 500mg will have 8 - 9μg/ml of amoxicillin and about 3μg/ml of clavulanic acid in serum.
After 1 hour oral 20mg/kg amoxicillin + 5mg/kg Clavulanic acid, there will be an average of 8.7μg/ml of amoxicillin and 3μg/ml of clavulanic acid in serum. The absorption of the drug is not affected by food and it is best to drink right before the meal.
Amoxicillin's oral bioactivity is 90% and of Clavulanic acid is 75%. The half -life of amoxicillin in serum 1-2 hours and of Clavulanic acid is about 1 hour. 55 - 70% amoxicillin and 30-40% Clavulanic acid are discharged in urine in the form of activity. Probenecid extends Amoxicillin's elimination time but does not affect the elimination of clavulanic acid. Before taking AuclanityL 500/125mg Tipharco Treatment of infection (2 blisters x 10 tablets)
How to use
oral AuclanityL 500/125mg at the beginning of eating.
Treatment must not exceed 14 days without examination.
Dosage
Dosage: Calculated by the content of amoxicillin in the drug.
adults and children 40kg or more
Take 1 tablet (500/125 mg)/time, 8 hours/time.
Elderly
No dose adjustments unless creatinine clearance ≤ 30ml/min.
Patients with renal failure
Creatinine clearance:
> 30 ml/minute: No dose adjustment.
10-30 ml/min: Take 1 capsule (500/125mg), 12 hours/time.
Hematopalyst
Take 1 capsule (500/125mg), every 24 hours, during the appraisal process, the dose may be repeated after the appraisal process (due to the concentration of amoxicillin and clavulanic acid in the blood will be reduced).
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Symptoms
Overdose of AuclanityL 500/125mg causes a disaster, because it is well tolerated even at high doses. However, general reactions occur depending on the hypersensitivity of each individual. The risk is definitely increased by potassium when taking very high doses and clavulanic acid is used in the form of potassium salt. Gastric - intestinal disorders and electrolyte metabolic disorders can also occur. Renal failure may be encountered (in people with renal failure or high doses).
Handling
Can use the method of hemolysis so that the drug is from circulation. Treatment of digestive disorders and attention to adjusting water - electrolytes.
What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
Digestive: diarrhea , nausea, vomiting and vomiting are associated with clavulanic acid doses (taking the dose of 250mg clavulanic acid risk to 40% compared to the dose of 125mg).
Skin: Ban Ban.
Uncommon, 1/1000 Blood: Eosin hypernagus. Liver: hepatitis and jaundice, increased transamine. Can be heavy and last for a few months. Other: Candida vaginitis, headache, fever, fatigue. Rare, ADR Systemic: Anaphylaxis reactions, edema. Blood: A mitigation of platelets, leukopenia, hemolytic anemia. digestive: fake colitis. Skin: Stevens - Johnson syndrome, diverse erythema, dermatitis, peeling, epidermal necrosis due to poisoning. kidney: interstitial nephritis. Central nervous system: agitation, anxiety, behavior change, confusion, convulsions, dizziness, insomnia and hyperactivity. Instructions on how to handle ADR When experiencing side effects of AuclanityL 500/125mg, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Auclanityl 500/125mg drug contraindicated in the following cases:
Hypersensitivity to any ingredients of the drug.
History of jaundice or liver dysfunction due to amoxicillin, clavulanic acid.
allergies to the beta-lactam group (penicillin and cephalosporin).
Be cautious when using
Should ask carefully what is related to a history of hypersensitivity reaction to penicillin, cephalosporin or other beta-lactam before the beginning of treatment with amoxicillin/clavulanic acid (see the anti-contrary and unwanted effect).
There have been reports on severe hypersensitivity reactions and sometimes fatal (anaphylaxis) in patients treated with penicillin. These reactions often occur in patients with a history of hypersensitivity to penicillin and in people with allergies. If allergic reactions occur, amoxicillin/clavulanic acid must be stopped and appropriate replacement.
In the case of an infection that is shown by a bacterium sensitive to amoxicillin, it is necessary to consider to convert from amoxicillin/clavulanic acid to amoxicillin use according to the official instructions.
This drug is not suitable for patients at high risk of reducing sensitivity or resistance to beta-lactam antibiotics without intermediaries beta-lactamase, which are easily inhibited by clavulanic acid. Do not use to treat S.pneumoniae resistant penicillin.
Convulsions may occur in patients with renal function or high doses (see unwanted effect).
Avoid using drugs if suspected of mononulopathy hyperllying bacteria due to the appearance of measles rash related to the future when using amoxicillin.
Concomitant use of Allopurinol during treatment with amoxicillin may increase the likelihood of skin allergies.
For long -lasting use can sometimes cause non -sensitive bacteria.
When the treatment is started if a large -scale rash appears, it may be a symptom of an out -of -the -acute pus. This reaction requires immediately stopping the drug and contraindicated with amoxicillin.
The drug should be used carefully in patients with signs of liver failure (see how to use and dosage).
The liver event has been reported mainly in men and elderly patients and for prolonged treatment. These events are very rare in children. Signs and symptoms usually occur during or after treatment but in some cases may be unclear until the treatment is terminated for a few weeks. All these symptoms will usually recover. Events in the liver may be serious and in a rare case that can lead to deaths. These are almost always occurring in patients with serious or simultaneous use with the drug that can affect the liver (see unwanted effect).
Palm colitis has been reported when using antibiotics and with mild to life -threatening severity (see unwanted effect). Therefore, this diagnostic consideration is very important in patients with diarrhea during or after taking any antibiotic. So when colitis occurs but related to antibiotics, should stop treatment immediately, patients should consult a doctor for appropriate treatment. Contraindicated anti -motion drugs in this situation.
Must periodically check the hematology, liver and kidney function during treatment.
There have been reports on abnormal extension of prothrombin time (Inr increased) but rarely in patients using Auclanityl and oral anticoagulants. Should follow appropriately when anticoagulants are prescribed simultaneously. The dose of oral anticoagulants may need to be adjusted to maintain the desired anticoagulant level (see drug interactions and unwanted effects).
Should adjust the dose reduction in patients with renal failure.
Observed the urinary crystal in patients who reduce urine secretion but very rare, mainly when patients use infusion. The patient should be recommended to maintain enough water and the amount of urine excreted during the high doses of amoxicillin use to reduce the possibility of the occurrence of urinary amoxicillin crystals. In patients with bladder catheter, regular tests should be maintained to avoid congestion (see overdose).
The presence of clavulanic acid in the drug can cause nonsense cohesion of IgG and albumin by red blood cell membranes that lead to fake Coombs test.
There have been reports on positive test results when performing test bio-rad laboratories platelia Aspergillus EIA in patients using amoxicillin/clavulanic acid that detects immune Aspergillus. There has been a report on the reaction between Non-Aspergillus Polysaccharides and Polyfuranoses with Aspergillus EIA Test, so the positive test results in patients who are using AuclanityL should be carefully considered and redefined by other analytical methods.
The ability to drive and operate machinery
The effect of AuclanityL 500/125mg drug on work (operators, train cars, higher -working people and other cases). The drug can cause headaches, fatigue, dizziness, convulsions, insomnia should be cautious when used for people who are driving and operating machinery.
Pregnancy
Avoid the use of drugs for pregnant women, especially in the first 3 months, except for the necessary cases prescribed by a physician.
Breastfeeding period
During breastfeeding can use preparations. The drug is not harmful to breastfed children unless there is a risk of sensitivity due to a very small amount of medicine in milk.
Medicinal interaction
The drug can cause prolonged bleeding time and blood clotting. Therefore, it is necessary to be careful for patients who are being treated with anticoagulants (Warfarin).
nifedipine increases amoxicillin absorption.
Patients with hyperuricemia when using allopurinol along with amoxicillin will increase the ability of amoxicillin.
There may be antagonists between amoxicillin and bactericidal stimulants such as fusidic acid, chloramphenicol, tetracyclin.
The drug can reduce the effectiveness of oral contraceptives, so it is necessary to in advance for the patient.
Simultaneous use of probenecid reduces the secretion of amoxicillin in the kidneys, increasing the concentration of amoxicillin in the blood.
Storage
Leave a cool place, avoid light, temperature below 30⁰C.
To be out of reach of children.
