Indications
Treatment of infections caused by sensitive bacteria in the following cases:
Severe upper respiratory infections: tonsillitis, sinusitis, otitis media have been treated with common antibiotics but does not decrease.
Lower respiratory infections by H.influenzae strains and Branhamella CatVrbalis produce beta - lactamase: Acute bronchitis and acute batch of chronic bronchitis, staging pneumonia in the community.
Urinary tract heavy bacterial infections by strains E. coli, klebsiella and Enerobacter produce: cystitis, urethritis, pyelonephritis.
Skin and soft tissue infections: pimples, abscesses, wound infections.
Bone and joint infections: Bone inflammation.
Pharmacokological
Amoxicillin is a semi -synthetic antibiotic belonging to the Beta - Lactam family with wide bactericidal spectrum for many gram -positive and gram -negative bacteria due to the synthetic inhibition of bacterial cell wall. But because Amoxicilin is easily decomposed by beta - lactamase, therefore does not work for bacterial strains producing these enzymes (many of En
Clavulanic acid due to the fermentation of Streptomyces Clavuligerus, which has a beta - lactam structure similar to penicilin, has the ability to inhibit beta - lactamase due to the majority of Gram -negative bacteria and Staphylococcus. Especially, it has a strong inhibition effect of beta - lactamase transmitted via plasmid that resists penicillins and cephalosporins. Clavulanic acid itself has very weak antibacterial effects.
Clavulanic acid helps amoxicillin not to be destroyed by beta - lactamase, and expand the antibacterial spectrum of amoxicillin effectively to many common bacteria that have resisted amoxicillin, resistant penicillins and cephalosporins.
pharmacokinetic
amoxicillin and clavulanic acid are easily absorbed by oral. The concentration of these 2 substances in the serum is maximum after 1-2 hours of taking the medicine. With a dose of 500mg will have 8 - 94g/ml of amoxicillin and about 3kg/ml of clavulanic acid in serum. After 1 hour oral 20mg/kg amoxicillin + 5mg/kg Clavulanic acid, there will be an average of 8.7kg/ml of amoxicillin and 3ng/ml of clavulanic acid in serum. The absorption of the drug is not affected by food and it is best to drink right before meals.
The oral bioactivity of amoxicillin is 90% and of Clavulanic acid is 75%. Half a lifetime of amoxicillin in serum 1-2 hours and of Clavulanic acid is about 1 hour.55 - 70% amoxicillin and 30-40% clavulanic acid are discharged through urine in the form of activity. Probenecid extends the excretion time of amoxicillin but does not affect the elimination of clavulanic acid.
Before taking Auclanityl 875/125mg Tipharco treatment for infections (2 blisters x 7 tablets)
How to use
Take at the beginning of eating.
Dosage
Treatment must not exceed 14 days without examination.
Dosage: calculated by amoxicillin content in the drug.
Adults and children from 40kg or more: Take 1 tablet (875/125 mg)/time, 12 hours/time.
Elderly: No dose adjustments unless creatinine clearance
kidney failure:
What to do when overdose? However, general reactions occur depending on the hypersensitivity of each individual. The risk is definitely hyperkalemia when taking a very high dose because clavulanic acid is used in the form of potassium salt. Gastric - intestinal disorders and electrolyte metabolic disorders may also occur. Can be encountered with kidney failure or convulsions (in people with renal failure or high doses).
Management: Can use blood decomposition methods to get the drug from circulation. Treatment of digestive disorders and attention to adjusting water - electrolytes.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using AuclanityL 875/125mg, you may experience unwanted effects (ADR).
Infections and parasites
Gastrointestinal disorders
Liver disorders
Should stop treatment if there is any hypersensitivity dermatitis (see caution).
Magic and urinary disorders
Instructions on how to handle ADR
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Contraindicated
Auclanityl drug contraindicated for the following cases:
History of jaundice or liver dysfunction due to amoxicillin, clavulanic acid.
Be cautious when using
Should ask carefully what is related to a history of hypersensitivity reaction to penicillin, cephalosporin or other beta - lactam before the beginning of treatment with amoxicillin/clavulanic acid.
There have been reports on severe hypersensitivity reactions and sometimes fatal (anaphylaxis) in patients treated with penicillin. These reactions often occur in patients with a history of hypersensitivity to penicillin and in people with allergies. If the allergic reaction occurs, it is necessary to stop treating with amoxicilin/clavulanic acid and have appropriate alternative therapy.
In the case of an infection, it is shown to be caused by an amoxicillin -sensitive bacteria, which must be considered for conversion from amoxicillin/clavulanic acid to amoxicillin use according to the official instructions.
This drug is not suitable for patients at high risk of reducing sensitivity or resistance to beta-lactam antibiotics without intermediaries beta-lactamase, which are easily inhibited by clavulanic acid. Should not be used to treat S. Pneumoniae resistant penicillin.
Convulsions can occur in patients with renal function or high doses.
Avoid using drugs if suspected of mononucleosis is caused by the appearance of the measles rash related to this condition after using amoxicillin.
Simultaneous use of allopurinol during treatment with amoxicillin may increase the likelihood of skin allergies.
For long -lasting use can sometimes cause non -sensitive bacteria.
When starting to treat if a large -scale rash appears, it may be a symptom of an acute bodied pus (AGEP). This reaction requires immediately stop Auclanityl and contraindicated to use amoxicillin.
The drug should be used cautiously in patients with signs of liver failure.
The liver event has been reported mainly in men and elderly patients and for prolonged treatment. These events are very rare in children. Signs and symptoms usually occur during or after treatment but in some cases may not be clear until the treatment is terminated for a few weeks.
All these symptoms will usually recover. Events in the liver may be serious and in a rare case that can lead to deaths. These things almost always occur in patients with serious or simultaneous diseases with the drug that can affect the liver.
Palm of fake colitis has been reported when using antibiotics and with mild to life -threatening severity. Therefore, this diagnostic consideration is very important in patients with diarrhea during or after taking any antibiotic. So when colitis occurs but related to antibiotics, should stop treatment immediately, patients should consult a doctor for appropriate treatment. Contraindicated anti -motion drugs in this situation.
Must periodically check the hematology, liver and kidney function during treatment.
There have been reports on abnormal extension of prothrombin time (Inr increased) but rarely in patients using Auclanityl and oral anticoagulants. Should follow appropriately when anticoagulants are prescribed simultaneously. The dose of anticoagulant drugs may be needed to maintain the desired anticoagulant level.
Should adjust the dose reduction in patients with renal failure.
Observed the urinary crystal in patients who reduce urine secretion but very rare, mainly when patients use infusion. The patient should be recommended to maintain enough water and the amount of urine excreted during the time of using high doses of amoxicillin to reduce the possibility of the occurrence of urinary amoxicillin crystals. In patients with bladder catheter, should be maintained regularly to avoid obstruction.
During the treatment period with amoxicillin, the enzyme glucose oxidase should be used whenever the test is looking for the presence of glucose in the urine because of the fake positive results that can occur with non -enzyme methods.
The presence of clavulanic acid in the drug can cause nonsense cohesion of IgG and albumin by red blood cell membranes that lead to fake positive tests.
There have been reports on positive test results when performing test bio - rad Laboratories Platelia Aspergillus EIA in patients using amoxicillin/clavulanic acid that detects immune Aspergillus. There has been a report on the reaction between Non-Aspergillus Polysaccharides and Polyfuranoses with Aspergillus EIA Test, so the positive test results in patients who are using AuclanityL should be carefully considered and redefined by other analytical methods.
The ability to drive and operate machinery
The drug can cause headaches, fatigue, dizziness, convulsions, insomnia, so cautious when used for people who are driving and operating machinery.
Pregnancy
Avoid the use of drugs for pregnant women, especially in the first 3 months, except for the necessary cases prescribed by a physician.
Breastfeeding period
During breastfeeding can use preparations. The drug is not harmful to breastfed children unless there is a risk of sensitivity due to a very small amount of medicine in milk.
Medicinal interaction
The drug can cause prolonged bleeding time and blood clotting. Therefore, it is necessary to be careful for patients who are being treated with anticoagulants (warfarin).
nifedipine increases amoxicillin absorption.
Patients with hyperuricemia when using alopurinol along with amoxicillin will increase the ability of amoxicillin.
There may be antagonists between amoxicillin and bactericidal stimulants such as fusidic acid, chloramphenicol, tetracyclin.
The drug can reduce the effectiveness of oral contraceptives, so it is necessary to notify the patient in advance.
Amoxicillin can reduce the secretion of methotrexate, causing increased toxicity on the gastrointestinal tract and hematopoietic system.
Simultaneous use of probenecid reduces the secretion of amoxicillin in the renal tubules, increasing the concentration of amoxicillin in the blood.
