Uses

indications

Augmentin 625 drug is indicated:

Treatment of sensitive bacterial infections:

Infectious upper respiratory infections (including ear - nose - throat): Acute tonsillitis, acute sinusitis, acute otitis media.

Lower respiratory infections: Acute bronchitis, lobe pneumonia, bronchitis.

Urinary infections: cystitis, urethritis, nephritis - pyelonephritis.

Skin and soft tissue infections: boils, abscesses, cellular inflammation, wound infections.

Bone bacterial infection: Bone inflammation.

Tooth infections: Severe alveolar abscess with spreading cellar inflammation or tooth infection does not meet the original antibiotics.

Pharmacokology

Amoxicillin is a semi -synthetic penicillin (Lactam - Lactam) inhibiting 1 or more enzymes (penicillin -mounted proteins, Pbps) in the synthesis of bacterial peptidoglycan, which is an indispensable structure component of bacterial cell walls. Therefore, it leads to the weakening of cell walls, then the separation and cell death.

Amoxicillin is easily degenerated by beta - lactamase produced by resistance bacteria and therefore the activity of single amoxicillin does not include bacteria producing these enzymes.

Clavulanic acid is a beta - lactam with a parentillin -related structure. It indifferent to some beta - lactamase enzymes thus prevents Amoxicillin inactivity. Single Clavulanic acid does not produce clinical useful antibacterial effects.

often sensitive bacteria:

Aerobic grams: Bacillus anthracis, Enterococcus Faecalis, Listeria Monocytogenes, Nocardia Asteroides, Streptococcus Pyogenes, Streptococcus Agalactiae, Streptococcus spp. (other bacteria β hemolysis), Staphylococcus aureus (sensitive to methicillin), Staphylococcus saproPhyticus (methicillin sensitive), staphylococcus aureocular enzyme coagulase (methicillin sensitive).

Aerobic grams: Bordetella pertussis, Haemophilus influenzae, Haemophilus parainfluenzae, Helicobacter Pylori, Moraxella Catatrhalis, Neisseria Gonorrhoeae, Pasteurella Multocida, Vibrio Cholerae.Other: Borrelia Burgdorferi, Leptospira ITTEROHAemorrhagiae, Treponema Pallidum.

Anaerobic Gram: Clostridium spp.

Anaerobic gram: bacteroides fragilis, bacteroides spp., capnocytophaga spp.

Bacteria can be resistant to drugs:

Escherichia Coli, Klebsiella Oxytoca, Klebsiella Pneumoniae, Klebsiella spp., Proteus Mirabilis, Proteus Vulgaris, Proteus spp., Salmonella spp Pneumoniae, Viridans streptococcus.

resistant bacteria:

Acinetobacter spp., Citrobacter Freundii, Entobacter spp Chlamydia Pneumoniae, Chlamydia Psittaci, Chlamydia spp., Coxiella Burnetii, Mycoplasma spp.

pharmacokinetics

absorption:

amoxicillin and clavulanic acid are completely dissociated in water solutions in physiological pH. Both are well absorbed and fast through oral. Birth of amoxicillin and clavulanic acid after drinking about 70%. The characteristics of 2 active ingredients in plasma are the same and the time to reach the peak concentration of each active ingredient is nearly 1 hour.

Distribution:

About 25% Clavulanic acid and 18% amoxicillin are total in plasma links to protein. Amoxicillin's apparent distribution volume is about 0.3 - 0.4 l/kg and clavulanic acid about 0.2 l/kg.

After intravenous injection, both active ingredients are found in gallbladder, abdominal tissue, skin, fat, muscle tissue, fluid, abdominal fluid, bile, pus. Amoxicillin does not distribute much in cerebrospinal fluid. Amoxicillin, clavulanic acid through the placenta and breast milk (little).

Metabolism:

Amoxicillin eliminates urine in the form of inactivated penicilloic acid (equivalent to 10-25% of the initial dose). Clavulanic acid metabolizes a lot in humans, excreted through urine and feces in the form of CO2 in exhaled gas.

Era:

Amoxicillin excreted the main kidney, clavulanic acid excreted through both the kidneys and outside the kidneys. The drug has an average 1 hour waste and the average body clearance is about 25 l/hour. 50 - 85% amoxicillin, 27 - 60% of clavulanic acid discharged through urine for 24 hours. Clavulanic acid is most waste after the first 2 hours of medication.

Before taking Augmentin 625mg GSK medicine for infections (2 blisters x 7 tablets)

How to use

Instructions on how to handle ADR

When experiencing side effects of Augmentin 625, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

Warnings

Contraindicated

Augmentin 625 contraindications in the following cases:

Patients with a history of hypersensitivity to beta - lactam (penicillin, cephalosporin).

Patients with hypersensitivity to any component of the drug.

Patients with a history of jaundice, liver dysfunction related to Amoxicillin - Clavulanat.

Be cautious when using

carefully ask the history of hypersensitivity penicillin, cephalosporin and beta - lactam other before treatment with augmentin 625.

Avoid using Augmentin 625 if suspected of mononulopathy hyperlemen due to the appearance of measles rash related to this situation when using amoxicillin.

Sometimes used for proliferation of non -sensitive bacteria.

Palm of fake colitis can occur from mild to life -threatening. Therefore, this diagnostic consideration is very important in patients with diarrhea during or after antibiotics. If diarrhea, prolonged or abdominal cramps, stop the drug immediately and check more.

Rarely reports jaundice, which may be heavy but often recovered. Symptoms may not be clear up to 6 weeks after stopping treatment. Use Augmentin 625 cautious in people with liver dysfunction.

Convulsions can occur in patients with renal function or high doses. Dose adjustment in patients with renal failure.

Very rarely see the urinary crystal in patients reducing urine secretion (mainly in patients using infusion). Advise the patient to maintain enough water and the amount of urine excreted during the time of using amoxicillin high doses to reduce the possibility of the appearance of amoxicillin crystals.

The ability to drive and operate machinery

No research. However, unwanted effects (allergies, dizziness, convulsions) can affect the ability to drive and operate machinery.

Pregnancy

Augmentin 625 may be associated with an increased risk of necrotizing bowelitis in newborns. Pregnancy should be avoided, especially in the first 3 months, unless the doctor is indicated.

The period of breastfeeding

very little excretion pills in breast milk. Except for the risk of sensitivity, no adverse effects on breastfeeding. Augmentin 625 can be used during breastfeeding.

Drug interaction

Probenecid reduces amoxicillin secretion through the renal tubules. Concomitant use with Augmentin 625 can cause increased and prolonged the concentration of amoxicillin in the blood but does not affect clavulanate. Do not recommend this combination.

Simultaneous use of allopurinol and amoxicillin may increase the likelihood of allergic reactions on the skin.

Augmentin 625 can affect the intestinal bacteria, reducing the reabsorption of estrogen, reducing the effectiveness of oral contraceptives used in combination.

rarely increase Inr in patients with acenocoumarol or warfarin combining augmentin. Should track prothrombin, INR when starting or stopping using augmentin.

In patients who are taking Mycophenolate Mofetil, there may be a decrease in the dose (pre - dose concentration) of metabolic substances with mycophenolic acid activity (MPA) about 50% after starting to use augmentin. The change of concentration before the dose may not exactly show the overall MPa exposure level.

penicillin can reduce methotrexate secretion, toxic.
