Uses
Indications:
Augxicine 500 mg/62.5 mg is indicated in the following cases:
Amoxicillin and potassium clavulanate preparations are used for treatment in a short time (less than 14 days) cases of infections caused by the products produced by beta - lactamase are not responded to treatment with poisonous aminopenicilin and sensitive to drugs:
Lower respiratory infections: The outbreak of chronic bronchitis, pneumonia sufficient in the community.
Bone and joint infections: Bone inflammation.
Pharmacology
Amoxicillin is a semi -synthetic penicillin (beta antibiotic - lactam), inhibiting one or more enzymes (often called penicillin bonding proteins, PBP) in the bacterial peptidoglycan biosynthesis of bacteria, this is an indispensable ingredient in bacteria. The inhibition of peptidoglycan biosynthesis leads to weakened bacterial cell wall walls, which often lead to cell destruction and death.
Amoxicillin is easily decomposed by beta - lactamase generated by anti -drug bacteria and thus the amoxicillin spectroscopy used alone does not include bacteria that produce these enzymes.
Clavulanic Acid is a beta - lactam related structural with penicillin. It indifferent to some beta - lactamase enzymes thus prevents Amoxicillin inactivity. Clavulanic Acid alone has no useful antibacterial effect on clinical.
Common sensitive bacteria:
Two main resistance mechanisms with amoxicillin/ clavulanic acid are: inactivated by Beta - Lactamases secretion is not inhibited by Clavulanic Acid, including B, C and D. PBP changes, reducing the affection of antibacterial agents to the target. The permeability of bacteria or pumping mechanism is the cause or contributing to the resistance of bacteria, especially in gram -negative bacteria.
Some strains have drug resistance:
Bacteria resistant to drugs:
Other bacteria: Chlamydophila Pneumoniae, Chlamydophila Psittaci, Coxiella Burnetii, Mycoplasma Pneumoniae.
Dynamic pharmacokinetics
absorption
Amoxicillin and Clavulanic Acid, are completely separated in water solutions in physiological pH. Both ingredients are easily absorbed by oral. Absorption of amoxicillin and clavulanic acid optimizes when taken at the beginning of the meal. After drinking, bioavailability of amoxicillin and clavulanic acid is about 70%. The concentration absorption chart of the two components is the same in plasma and the time to reach the maximum drug concentration (TMAX) in each case is about an hour.
Distribution
About 22 - 30% Clavulanic acid and 17-20% amoxicillin binds to plasma proteins. Amoxicillin's distribution voltage is about 0.3 - 0.4 l/kg and of clavulanic acid is about 0.2 l/kg.
After drinking, both amoxicillin and clavulanic acid are distributed into the lungs, pleural fluid and peritoneal fluid, passing through the placenta. A small amount of pills found in sputum, saliva as well as in breast milk. When the meninges are not inflamed, the concentration of drugs in cerebrospinal fluid is very low, but the larger amount is achieved when the meninges are inflamed.
Metabolism
Amoxicillin is partially excreted in urine into penicilloic acid, which is not active in equivalent to 10-25% of the initial dose. Clavulanic acid is metabolized in the body and eliminated through feces and urine and in the form of carbon dioxid in emissions.
Elimination
Amoxicillin is excreted mainly through the kidneys, while clavulanic acid is excreted through both kidney - and no kidney mechanisms. Amoxicillin/Clavulanic acid has an average half -life of about an hour and an average creatinine clearance of about 25 l/hour in people with normal kidney function. About 60 - 70% amoxicillin - and about 40 - 65% Clavulanic acid are discharged in urine in the form of unchanged within 6 hours after taking single doses of amoxicillin and potassium clavulanate.
Probenecid extends the excretion time of amoxicillin but does not affect the elimination of clavulanic acid.
Before taking Augxicine medicine 500mg/62.5mg Jersey treat tonsillitis, sinusitis (10 packs x 1000mg)
How to use
Used by oral, drink at the beginning of eating to minimize the phenomenon of non -intolerance of the stomach - intestines.
Do not take the drug for more than 14 days without checking, reviewing the treatment. The dosage is usually calculated by the content of the total amoxicillin and clavulanic acid.
Put the powder in 10-15ml of cool boiled water (about 2-3 teaspoons), stir and drink immediately.
Dosage
Adults:
Dosage is calculated by age or mg/kg/day.
No dose adjustment, dose as adults. If signs of renal failure, dose adjustment should be adjusted by patients with renal failure.
kidney failure
No dose adjustment in patients with creatinine clearance> 30 ml/min.
In patients with clearance
Hematopalyst
This drug should only be used for patients with creatinine clearance> 30 ml/min.
Hepatic failure
Be cautious when prescribing this drug, should periodically check the liver function.
Not enough data to give recommended dose.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Symptoms
Abdominal pain, vomiting and diarrhea, water and electrolyte balance disorders, convulsions, rash, agitation or drowsiness. Crystal pisses in some cases that lead to kidney failure has been reported.
Handling
Need to stop the drug immediately. Symptomatic treatment and support. It is necessary to provide enough water and electrolytes to the body to maintain the urinary tract and reduce the risk of crystalline urination. Can use hemolysis method to remove amoxicillin and clavulanic acid from circulation.
What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
When using Augxicine often has unwanted effects (ADR).
Common (ADR> 1/100)
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Too hypersensitivity to the beta - lactam group (penicillin, cephalosporin, carbapenem or monobactam) or any ingredients of the drug.
Patients with a history of jaundice/liver dysfunction due to amoxicillin and potassium clavulanate (due to clavulanic acid that increases the risk of cholestasis in the liver).
Precautions when used
The ability to drive and operate machinery
Caution should be cautious when driving or operating machinery due to drugs can cause unwanted effects: allergic reactions, dizziness, headache, convulsions, fatigue.
Pregnancy
Experimental research on the reproductive process of mice proved when using oral preparations and injections at a dose larger than 10 times the dosage for humans did not cause deformities. However, because there is little experience in using preparations for pregnant women, it is necessary to avoid using drugs in pregnant women, especially in the first 3 months, except for the necessary cases prescribed by a physician.
Breastfeeding period
During breastfeeding can use preparations. The drug is not harmful to breastfeeding, unless there is a risk of sensitivity due to a very small amount of medicine in milk.
Medicinal interaction
Storage
Leave a cool dry place, avoid light, avoid moisture, temperature below 30⁰C.
