Uses

indications

Azarga drug is indicated in the following cases:

  • Reduce internal pressure for adults with glaucoma (IOP) or Glocom opened angle that the treatment does not effectively lower the glaucoma.

Brinzolamid is a strong carbon dioxide inhibitor (CA-II) in humans, this iso-enzyme is mainly in the eye. The inhibition of anhydrase carbon dioxide in the eyelashes (eyelashes) of the eye reduces the secretion of the aquarium, by slowing down the formation of bicarbonate ions, thereby reducing the transport of sodium and aquarium.

Timolol is an unsterious adrenergic blocker, no intrinsic sympathetic activity, direct inhibition of myocardial or membrane stability

Two active ingredients have different mechanisms, but when combined, it will reduce the glaucoma mainly by reducing the tomb secretion than using only one substance.

    Dynamic pharmacokinetics

Absorption: After the eye is small, Azarga's eyes, the active ingredient Brinzolamid and Timolol are absorbed through the cornea and in the overall circulation.

Distribution: Brinzolamid's plasma protein bonding ratio is average (about 60%). Brinzolamid is concentrated mainly in red blood cells due to high-grinning drugs, connected to Ca-II and at a lesser level than with Ca-I.

Timolol can be measured in a human aquatic fluid after using a small Timolol eye solution and in plasma until 12 hours after using Azagar eye.

Metabolism: Metabolism of brinzolamid related to n-dealkylation, o-dealkylation and its N-Propyl chain oxidation. The main metabolites of Brinzolamid in humans are N-Desethyl Brinzolamide, which is mainly related to CYP3A4.

Timolol is metabolized in two lines: creating an ethanolamine string on Thiadiazol ring and creating an ethanolic string on nitrogen morpholin and a second string similar to the carbonyl group attached to nitrogen. Timolol metabolized mainly thanks to CYP2D6.

Elimination: Brinzolamid is excreted mainly in urine and fertilizer at the equivalent ratio, 32% and 29% respectively. About 20% of the dosage excreted through urine is metabolic.

Timolol and its metabolites are excreted mainly through the kidneys, of which 20% of the doses of Timolol are eliminated in the urine in the form of constant, the rest is in the form of metabolites. Timolol's waste time after using Azarga eye is 4.8 hours.

  • Before taking Azarga Alcon eye drops reduce internal pressure (5ml)

    How to use

Azarga drugs are only used for eye drops.

Instruct patients to shake the vial carefully before using.

To avoid infection of the small tip of the pipe and the drug chaos, be careful not to let the small head of the vial touch the eyelids and the areas around the eyes, or any surface. Instruct patients to close the lid of the vial when not in use.

After opening the lid, if the attachment guaranteed to be dropped, it is necessary to remove it before taking the drug.

After the patient's eyes should press the turbulent tube or close the eyes, the whole body absorption of the drug will decrease. This will help reduce the unwanted effects of the whole body and increase the local effects of the drug.

If using more than one eye drops, the drugs should be used at least 5 minutes apart.

    Dosage

Put 1 drop of a small Azarga eye mixture into the conjunctiva bag of the eye twice daily.

Children: It is not recommended to use small eye mixes for children under 18 years of age due to safety and effectiveness of treatment.

Patients with liver failure, kidney failure: No studies of Azarga eye media or Timolol 5 mg/ml eye drops are performed in patients with liver or renal failure. No need to adjust the dose in patients with hepatic or medium and medium liver failure patients.

Contraindicated for patients with severe renal impairment.

    Elderly patients:

There is no difference in safety and effectiveness of treatment observed between the elderly patients and other adult patients.

The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?

Due to Brinzolamid, electrolyte imbalance, progress to acidosis and effects on the nervous system may occur. Need to monitor serum electricity (especially blood potassium) and pH.

How to handle: Need to support support such as washing eyes with warm water and symptomatic treatment. Studies have shown that Timolol is not excreted through dialysis.

What to do when you forget 1 dose? Do not get more than a drop once, 2 times daily to the eye need treatment.

When using Azarga eye shrine to replace another glaucom treatment, it is necessary to stop using the replacement drug and start taking Azarga on the next day.

  • Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Azarga eye drops are not used in the following cases:

  • Hypersensitivity to the active ingredients, with any excipients of the drug or with sulphonamids. Heavy allergies and bronchials are overly responding, hypersensitivity to other beta blockers.

    Caution when using

    Caution:

Brinzolamid and Timolol are absorbed in the body. Because the Timolol component has the effect of beta-adrenergic blockers, there may be unwanted effects on the heart, lungs and other unwanted effects as observed with beta-adrenergic blockers using body sugar.

There may be hypersensitivity reactions that are often encountered with sulphonamid derivatives. It is necessary to stop taking the drug if there are signs of serious reactions or hypersensitivity reactions.

Acid-base balance disorders may be encountered when taking oral carbonic inhibitors. Caution used in patients at risk of renal failure due to the risk of metabolic acidosis.

ANHYDRASE carbonic inhibitors can affect hydration in the cornea causing compensation and cornea edema. Therefore, careful supervision of patients with corneal damage (patients with diabetes or corneal dystrophy).

    Heart disorders:

In patients with cardiovascular disease (such as coronary artery disease, prinzmetal angina, heart failure) and hypotension, should be carefully evaluated to treat beta blockers and should consider treating other active ingredients. Must monitor the signs of worsening of these diseases and adverse effects.

    circuit disorders:

Be cautious when treating drugs for patients with disturbance/severe peripheral circulatory disorders (such as severe forms of Raynaud or Raynaud's syndrome).

    Respiratory disorders:

Caution for patients with asthma due to respiratory reactions, including death.

    Hypoglycemia of diabetes:

Be cautious when taking beta blockers for spontaneous hypoglycemic patients or unstable diabetes due to beta blockers can hide signs and symptoms of acute hypoglycemia.

    hyperthyroidism:

Beta blockers can also hide the signs of hyperthyroidism.

    weak muscle:

Beta-adrenergic blockers can cause weakness in the symptoms of myasthenia gravis (for example, pilot, eyelash collapse and body weakness).

    Other beta inhibitors:

Effects on intraocular pressure or beta blockers can appear when Timolol is used for patients being treated with beta blockers. Should closely monitor the response of these patients.

    Hypersensitivity reaction:

While taking beta blockers, patients with a history of the response or a history of the hypersensitivity reaction to some allergens may react more strongly to the repetitive challenge with these allergens and may not respond to the adrenalin doses of communication used to treat hypersensitivity reactions.

    Tearing the vein:

Black Mac has been reported with the treatment of aquaculture inhibitors (such as Timolol, Acetazolamid) after a reincarnation surgery.

    surgical anesthesia:

Beta blockers can inhibit the effects of Beta -body dominance as Adrenalin. Should notify the anesthetic doctor when the patient uses Timolol.

    Effect on the eye:

Be cautious when using the drug for these patients and monitor the internal pressure closely.

    contact lenses:

The drug contains benzalkonium chloride can cause irritation and color change of soft contact lenses. Avoid contact with soft contact lenses. Patients need to be instructed to remove contact lenses before the Azarga eye is small and wait at least 15 minutes before wearing again.

The ability to drive and operate machinery

temporary blurred vision or other visual disorders may affect the ability to drive or operate machinery. If blurred vision appears after small eyes, the patient must wait until he can see it clearly before driving or operating machinery.

ANHYDRASE CARBONDRASS may reduce the ability to perform jobs that need mental alertness and/or need physical coordination.

The period of pregnancy and lactation

should not be used during pregnancy unless it is really necessary. However, if you use a small Azarga eye discharge during pregnancy to birth, you should carefully monitor your child during the first days of birth.

    Breastfeeding period

Brinzolamid is used to be secreted into milk. It is unknown whether small eye brinzolamid can be secreted into human milk. Beta blockers are excreted into breast milk that can cause serious unwanted effects on breastfeeding.

    Drug interactions

Azarga eye brushes contain brinzolamid, an anhydrase carbonic inhibitor and although used in the eye, the drug is still absorbed in the body. Acid-base balance disorders have been reported with oral carbon dioxide inhibitors. The risk of interaction should be considered in oral carbon dice.

Sudden hypertension reaction when clonidine stops can be more serious when using beta blockers.

The risk of beta blocking effects (such as reduced heart rate, depression) has been reported when combined with CYP2D6 inhibitors (such as Quinidin, Fluoxetin, Paroxetin) and Timolol.

Beta blockers can increase the hypoglycemic effect of anti -diabetic drugs.

can have a synergistic effect that leads to significant hypotension and/or slow heartbeat slowly when simultaneously used with beta-blocked eye blockers with calcium channel blockers, beta-adrenergic blockers, anti-arrhythmic drugs (including amiodaron), or Digitalis cardiac glycosides, oral sympathetic drugs.

Ps stretching by simultaneous use of Beta and Adrenalin eye blockers (Epinephrin).

Beta blockers can reduce response to adrenalin used to treat hypersensitivity reactions. It is necessary to be especially cautious in patients with a history of allergies or anaphylaxis.
