Uses
Indications
Azithromycin 250mg (H/1VI x6V) Armephaco 1x6 is indicated in cases of infections caused by bacteria sensitive to azithromycin such as:
Skin and soft tissue infections.
Pharmacology
azithromycin is a bactericidal antibiotic belonging to the macrolid group. The protein synthesis of bacterial cells is attached to the ribosom subunit unit, preventing the transition of peptidyl-Arnt from the reception position to the position, so the new aminoacyl-karn cannot enter the reception position, so the amino acids cannot be attached to the peptide chain being established.
Effective spectrum:
azithromycin is a new antibiotic with a broad spectrum, but it also has cross -resistance to erythromycin, so it is necessary to consider carefully when using azithromycin due to spreading macrolid -resistant bacteria in Vietnam.
Azithromycin works well on Gram -positive bacteria such as Streptococcus, Pneumococcus, Staphylococcus aureus. Studies conducted in Vietnam show that these species are about 40%resistant to macrolid groups, so the ability to use azithromycin is limited. Some other bacteria strains are also very sensitive to azithromycin such as: corynebacterium diphtheriae, Clostridium perfringens, peptostreptoccus and propionibacterium acnes.
Erythromycin anti -Erythromycin strains can also be resistant to both azithromycin as gram -positive strains, including enteroccocus and most staphylcoccus strains of methicilin are completely resistant to azithromycin.
Azithromycin works well in gram -negative bacteria such as: Haemophilis Influenzae, Parainfluenzae and Ducreyi, Moraxella Pneumophilia, Bordetella Pertussis and Parapertussis, Neisseria Gonorrhoeea and Campylobacter sp.
In addition, this antibiotic is also effective with Listeria monocytogenes, Mycobacterium avium, Mycoplasma pneumoniae and Hominis, ureaplasma ureaticum, Toxoplasma Gondii, Chlamydia Trachomatis and Chalamydia Pneumoniae, Treponema Pallidum and Borrelia Burgdorferi.
Azithromycin has a moderate effect on Gram -negative bacteria such as E. Coli, Salmonella Enteritis, and Salmonella Typhi, Entobacter, Acromonas Hydrophilia, Klebsiella.
Azithromycin resistant grams are Proteus, Serratia, Pseudomonas Aeruginosa and Morganella.
In general, azithromycin acts slightly weaker on Gram -positive bacteria than erythromycin but stronger on some Gram -negative bacteria including Haemophilus.
pharmacokinetics
absorption:
After drinking, Azithromycin is widely distributed in the body, bioavailability is about 40% and reaches the maximum plasma concentration within 2 to 3 hours. Food reduces the ability to absorb Azithromycin by about 50%.
Distribution:
The drug is distributed mainly in tissues such as lungs, tonsils, prostate glands, granulocytes and macrophages, higher than in the blood (about 50 times the maximum concentration found in plasma). The concentration of the drug in the central nervous system is very low.
Era:
Small amount of azithromycin is reduced to methyl in the liver and is exposed to bile partially in the formless form and partly in the form of metabolism. About 6% of oral doses except for urine within 72 hours in the form of unchanged. The sale time of the drug is from 2 to 4 days.
Before taking Azithromycin 250 armephaco treatment for infection (1 blister x 6 tablets)
How to useazithromycin used once daily, drink 1 hour before meals or 2 hours after eating.
Dosage
Adults:
Treatment of sexually transmitted diseases such as cervicitis, urethritis caused by chlamydia trachomatis infection with a single dose of 1 g.
Other indications (bronchitis, pneumonia, sore throat, skin infection and soft tissue): Total dose of 6 Azithromicin tablets divided into 3 days (3 -day therapy), 2 tablets a day or the first day of taking a dose of 500 mg and taking the next 4 days with a single dose of 250 mg/day.
Children:
The suggested dose for children on the first day is 10 mg/kg body weight, followed by the 2nd to the 5th day of 5 mg/kg/time x 1 time/day.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Azithromycin 250mg (h/1vi x6v) Armephaco 1x6 you can experience unwanted effects (ADR).
Common, ADR> 1/100
Uncommon, 1/1000 Rare, ADR skin: eagle. Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Azithromycin 250mg (H/1VI x6V) ARMEPHACO 1X6 Contraindications in the following cases:
Do not use for people with hypersensitivity to azithromycin or any antibiotic belonging to the macrolid group.
Be cautious when using
be careful when using azithromycin and other macrolids because of the ability to cause allergies such as vasodilation and anaphylaxis are very dangerous (although less likely).
As with other antibiotics during the use of the drug, the signs of superinfection must be observed by bacteria that are not sensitive to drugs including fungi.
It is necessary to adjust the appropriate dose for patients with renal disease with creatinine clearance ratio less than 40 ml/min.
Do not use this drug for people with liver disease because of the liver excretion.
The ability to drive and operate machinery
There is no evidence of the impact of the drug on the ability to drive and operate machinery.
Pregnancy
There is no research data on pregnancy. Azithromycin should only be used when there are no other appropriate drugs.
The period of breastfeeding
There is no data to study the excretion ability of azithromycin by breast milk. Azithromycin should only be used when there are no other appropriate drugs.
Medicinal interaction
Conditioner of chicken fungus: Unserted with azithromycin with chicken spurs derivatives because of its ability to cause poisoning.
Antacislets: When needed, azithromycin is only used at least 1 hour before or after 2 hours after eating.
Carbamazepin: In pharmacokinetics research in healthy volunteers, no significant effect on carbamazepine concentration or their metabolic products in plasma.
cimetidin: pharmacokinetics of azithromycin is not affected if taking a dose of cimetidin before using azithromycin 2 hours.
cyclosporin: Some antibiotics of macrolid group interfere with the metabolism of cyclosporin, so it is necessary to monitor the concentration and adjust the dose of cyclosporin.
Digoxin: For some patients, azithromycin can significantly affect the metabolism of digoxin in the intestine. Therefore, when using these two drugs simultaneously, it is necessary to monitor the concentration of digoxin because it has the ability to increase the content of digoxin.
Methylprednisolon: Research on healthy volunteers has proved that azithromycin has no significant impact on the pharmacokinetics of Methylprednisolon.
Theophylin: There has not been any effect on pharmacokinetics when two drugs azithromycin and theophylin are used on healthy volunteers, but in general, theoophylin should be monitored when using these two drugs for patients.
Warfarin: When studying pharmacokinetics on healthy people using a single dose of 15 mg Warfarin, azithromycin does not affect anticoagulant effects. These 2 drugs can be used at the same time but still need to monitor the patient's blood clotting time.
Storage
In a cool dry place, avoid light, temperatures below 30 ° C.
