Indications

BICEBID drug indications for treatment in the following cases:

  • Inhabited urinary tract infections caused by Escherichia Coli or Proteus Mirabilis and some cases of uncomplicated urinary tract infections caused by gram -negative bacilli such as Citrobacter spp., Enterobacter spp., Klebsiella spp. Betalactamase strains), Moraxella CatVrhalis (including beta-lactamase strains), Streptococcus pyogenes. Moraxella Catnrhalis. Gonorrhea has no complications caused by Neisseria Gonorrhoeae (including beta-lactamase strains), typhoid disease caused by Salmomella Typhi (including multi-resistant strains), sensitive shigella dysentery (including ampicilin-resistant strains).
  • Pharmacy

Cefixim is a semi -synthetic antibiotic of the 3rd generation cephalosporin group, with bactericidal effect. Cefixim prevents the growth and division of bacteria by attaching to target proteins (penicillin -mounted proteins) that inhibit mucopeptide synthesis in bacterial cell walls. Cefixim has high durability with the hydrolysis of beta-lactamase encoded by the gene located on plasmid and chromosom. Sustainability with beta-lactamase of Cefixim is higher than Cefaclor, Cefoxitin, Cefuroxim, Cephalexin, Cephadin.

cefixim works both invitro and clinically with most of the strains of the following bacteria:

Gram -positive bacteria: Streptococcus pneumoniae, Streptococcus pyrogenes.

Gram-negative bacteria: Haemophilus influenzae (secretive or no betalactamase), Moraxella Catatrhalis (most beta-lactamase secretion), Escherichia coli, Proteus Mirabilis, Neisseria Gonorrhoeae (secretive or non-penicilinsae).

cefixim also works to invitro with the majority of the strains of the following bacteria, but the clinical effect has not been determined:

    Gram -positive bacteria: Streptococcus agalactiae.

Gram-negative bacteria such as: Haemophilus parainfluenzae (secretive or no beta-lactamase secretion), Proteus vulgaris, Klebsiella Pneumoniae, Klebsiella Oxytoca, Pasteurella Multocida, Providencia spp., Salmonella spp., Shigella SCP Citrobacter Diversus, Serratia Marcescens. Cefixim has no activity for Enterococcus, Staphylococcus, Pseudomonas Aeruginosa and most Bacteroides and Clostridia strains.

    pharmacokinetics

After taking a single dose of Cefixim, about 30-50% of the dose is absorbed in the digestive tract.

The absorption of the drug does not depend on the meal, but the absorption rate is relatively slow, especially when taken with the meal. The time to reach the peak concentration of the drug in plasma after taken is 2 - 6 hours. Half life in plasma is about 3-4 hours and may last for renal failure. About 65% Cefixim in the blood is attached to plasma proteins.

The drug can pass the placenta, reaching relatively high concentrations in bile and urine.

The drug is excreted mainly through the kidneys, about 20% of oral doses are eliminated in the form of unchanged urine within 24 hours and about 60% of the oral dose is excreted through the kidneys, partly eliminated through feces.

Treatment time depends on the type of infection: uncomplicated urinary tract infections or respiratory infections on the treatment period of 5 - 10 days, lower respiratory tract or otitis media 10-14 days.

Adjust the dose for kidney failure: Adults have creatinine clearance from 21 - 60 ml/minute cefixim dose 300 mg/day, if creatinine clearance

To treat uncomplicated gonorrhea caused by Neisseria Gonorrhoeae (including beta-lactamase strains): use a dose of 400 mg (single-time, combined with an effective antibiotic with chlamydia that is likely to be infected). Higher doses (1 time 800 mg) have also been used to treat gonorrhea. When kidney failure needs to adjust the dose accordingly.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    Contraindicated

Contraindicated Bicebid in the following cases:

  • Patients with a history of hypersensitivity to cefixim or other antibiotics of Cephalosporin group, who has a history of anaphylaxis due to penicillin.
  • Be cautious when using

Sensitive people with penicillin, cephalosporin, cephamycin.

People with a history of gastrointestinal diseases and colitis, especially when used for prolonged use because of the risk of excessive growth of anti -drug bacteria, especially Clostridium difficile causing severe diarrhea, need to stop the drug and treat with other antibiotics (metronidazol, vancomycin, ...). However, diarrhea for the first 1-2 days due to medication, if light does not need to stop the drug.

People with kidney failure and hemorrhage must reduce the dose. Must monitor kidney and blood function during treatment, especially when taking the drug for a long time, with high doses.

Children younger than 6 months old, pregnant women and during breastfeeding.

Elderly people suffer from liver failure, renal failure (creatinine clearance

Use drugs for women during pregnancy and lactation

The drug can be given the placenta but there is no complete research on pregnant women, so it is only used if it is really necessary.

Lactating women should be used cautiously, can suspend breastfeeding during medication.

The effect of the drug on the ability to drive and operate machinery

The drug can cause headaches, dizziness, so it is necessary to be cautious when driving and operating the machine.

    Drug interaction

Probenecid increases the peak and AUC concentration of Cefixim, reduces the clearance of the kidney and the distribution of the drug.

nifedipin when taken with cefixim increases the bioavailability of cefixim, manifests itself by increasing peak concentration and AUC.

The drug increases the concentration of carbamazepin in the blood when used simultaneously.

warfarin or other anticoagulants: when used with cefixim, it increases prothrombin time, accompanied by bleeding or not.

Positive urinary glucose reactions when testing with Benedict or Fehling solution, Sulfate copper tablet but does not occur with test -based tests of glucose oxidase.

There have been a report on the direct Coombs test for positive results during treatment with cephalosporin.

    Storage

In a dry place, the temperature does not exceed 300C, avoiding light.
