Uses

Indications

Binex Mentone 500 mg is indicated in the treatment of acute and chronic symptoms of chronic osteoarthritis and rheumatoid arthritis (inflammation, stiffness and joint pain).

Pharmacokological

Nababetone belongs to nonsteroidal anti -inflammatory groups that have anti -inflammatory, analgesic and antipyretic effects. The drug has the ability to inhibit prostaglandin biosynthesis, which is intermediate.

Nabumetone is a precursor, after being transformed through the liver, it will produce 6 tethoxy-2-naphthylacetc acid (6MNA) which is an active ingredient with the ability to inhibit prostaglandin synthesis.

pharmacokinetics

After drinking, about 80% of the dose of nababetone marks the found in the urine. Nabumetone is not found in plasma after being absorbed, Nabumetone is quickly transformed into a major active metabolic, 6 methoxy-2-naphthuylacetic acid (VNA). After taking a dose of 1000mg, about 35% is converted into 6mna and 50% converted into unknown substances and they are eliminated in urine.

6MNA is connected to plasma proteins about 99%.

Used with food will increase the absorption rate and the appearance of BMNA in plasma but does not affect the conversion of nababetone to 6MNA. The peak concentration of 6MNA in plasma increased by about 1/3.

Used with the ATACID resistance containing aluminum does not significantly affect the bioavailability of 6MNA.

6MNA is metabolized through the liver to form inactive metabolites and are eliminated in two forms of free and combined form. About 75% of the marked dose is found in urine after 48 hours. After taking the dose of 1000mg - 2000mg, the average plasma clearance is 20-30ml/min and the waste time is about 24 hours.

How to use

Binex Mentone is recommended by oral.

Dosage

  • Adults
  • The starting dose is usually 2 tablets (1g), drink once a day in the evening before going to bed. Some patients may need a dose of up to 1.5 or 2g to reduce symptoms. Dosage can be adjusted depending on the patient.

  • Elderly people
  • Like other drugs, Nabumetone concentration is usually higher than in the blood of the elderly. The daily recommendation does not exceed 1g, in most cases, the dose of 1 tablet (500mg/day) has shown a good response in these objects.

    Side Effects

When using Binex Mentone 500 mg tablets of Binex manufacturing company (USA), with the main ingredient of Nabumetone. Binex Mentone is a drug to treat osteoarthritis, rheumatoid arthritis and the conditions that need anti -inflammatory drugs, you may experience unwanted effects (ADR)

Digestive system: Sometimes indigestion, nausea, vomiting, abdominal pain, constipation, flatulence, diarrhea, in vapor, esophagitis, peptic ulcer, potential or massive gastrointestinal bleeding. Rarely gastric perforation, rectal inflammation, gastritis. Usually the above consequences are more serious in older patients.

Hematology: Occasionally cause anemia, blood formula disorders (leukocytes), leukemia, thrombocytopenia. If used simultaneously with toxic drugs on the bone marrow, especially methotrexate will be a favorable factor for blood cell decline.

Skin: Sometimes it causes stomatitis, itching, skin rash, urticaria, rarely sensitive to light, diverse erythema, blistering such as Stevens Johnsan syndrome, epidermal necrotic poisoning.

Respiratory: In some patients, after taking aspirin and other non -steroid anti -inflammatory drugs including this drug, acute asthma attacks.

Central nervous system: Sometimes headache, dizziness, tinnitus, falling asleep, fatigue ...

Cardiovascular: Sometimes edema, hypertension, tachycardia, blushing.

Liver: temporarily increases serum transaminase or bilirubin. Very rarely with hepatitis.

Kidney: Double when disorders of kidney function parameters (increased serum creatining or urea in urine, rarely causes kidney failure.

vision disorders: Sometimes conjunctivitis, visual disorders.

Allergic reactions: rarely causing concept, increased sensitivity reaction including anaphylactic reaction.

Notice immediately to the doctor when encountering the unwanted effects of the drug.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Binex Mentone drugs are contraindicated in the following cases:

  • Patients with a history of hypersensitivity to Nabumetone or any ingredients of the drug.
  • Patients with cross -sensitivity with aspirin and other non -steroids anti -inflammatory drugs: Patients who have shown signs of asthma, nasal polyps, angiography or urticaria after taking aspirin and other nonsteroidal anti -inflammatory drugs.
  • Patients with progressive stomach - duodenal ulcer.
  • Patients with severe liver failure.

  • Patients with severe kidney failure must not be hemorrhagic.
  • Children under 15 years old.
  • Pregnant and lactating women.

    Precautions when used

such as other nonsteroidal anti -inflammatory anti -inflammatory, need to be cautious when taking this drug in patients with the above gastrointestinal pathology or being treated with anticoagulants. Need to monitor carefully in abnormal manifestations in the stomach - intestines. Should stop taking the drug if the signs of ulcer or gastrointestinal bleeding.

As most of other nonsteroidal anti -inflammatory drugs, sometimes the drug increases serum transaminase or other liver function indicators, in patients with signs of impaired liver function or unusual liver testing, so the progression and severity of liver function should be determined during the medication period. There have been some serious reactions on the liver such as jaundice, hepatitis when using anti -inflammatory drugs - steroids. Although very rare, if the abnormalities in the testing of liver function are not lost and become worse or the appearance of systemic signs (eosinophilia, rash ...), then stop using nabumetone. Because Nabumetone's biological transformation into 6mnc is dependent on liver function, the biological change can be reduced in patients with severe liver failure. So be cautious when using Nabumetone for patients with severe liver failure.

The combination treatment of anti -inflammatory drugs and painkillers only treat symptoms, not treatment.

When taking this medication to treat chronic symptoms, note the following factors:

When taking the drug for a long time, regular tests should be checked (liver function test, blood formula ...) If there are abnormal signs, appropriate handling measures such as dose reduction or stop use of drugs.

When treating acute symptoms, note the following points:

  • It is necessary to consider carefully in the case of acute inflammation, pain and fever. Keeping water and covering the report in some patients using Nabumetone. Therefore, like other nonsteroidal anti -inflammatory drugs, need to be cautious when taking drugs in patients with congested heart failure, high blood pressure.
  • The ability to drive and operate machinery

The drug does not affect the ability to drive and operate machinery.

    Pregnancy

using NSAIDs during pregnancy can slow labor, prolong pregnancy.

NSAID can stop the development of fetal arteriosus, persistent lung pressure in infants, increasing the rate of miscarriage. Therefore should not be used for pregnant women. Pregnant women should avoid using NSAIDs unless really necessary.

    Breastfeeding period

    Medicinal interaction

In clinical trials, the metabolic substance of Nabumetone is EVN strongly connected to the protein protein that can replace other drugs at the position mounted on the protein. Be cautious when using nababetone and warfarin for having an interaction between warfarin and other nonsteroidal anti -inflammatory drugs.

Nabaetone sharing with aluminum antacids does not affect BMNA's bioavailability. When used with food or milk, Nabumetone absorbs faster but the total amount of 6NNA in the blood does not change.

ACE inhibitors (ACE): Studies show that NSAIDs reduce the effects of enzyme inhibitors.

aspirin. When using Nabumetone simultaneously with NSAIDs, the rate of association with the protein of the drug is reduced, although the clearance of Nabumetone does not change. Clinical manifestations have not been statistically, but the fact that other NSAIDs are not indicated simultaneously with Nabumetone due to the ability to increase side effects.

Diuretics: Clinical studies have shown that Nabumetone can reduce the effects of diuretics to increase sodium discharge such as Furosemide and Thiazide in some patients. At the same time, because Nabumetone also inhibits the formation of prostaglandin in the kidney, so when used in combination with NSAIDs, patients need to closely monitor the symptoms of kidney failure as well as ensure the effectiveness of diuretics.

Lithium: NSAIDs increase lithium concentration in plasma and reduce lithium's kidney clearance. The average lithium drug concentration increased by about 15% and the renal clearance decreased by about 20%. These effects are due to inhibition of the renal prostaglandin formation of NSAIDs. Therefore, if NSAIDS and Lithium use simultaneously, it is necessary to closely monitor the symptoms of lithium poisoning.

Methotrexate: NSAIDs are reported to compete on the elimination process with methotrexate, so it can cause toxicity of methotrexate. Be cautious when using narnebutione with methotrexate.
