Uses

Indications

Bisilkon Bidiphar 10 G is indicated for skin diseases that respond to corticosteroids when there is complication of bacterial infections (sensitive to gentamicin) and fungi (sensitive to clotrimazol) or when suspected due to infection. Bisilkon is suitable for treatment of leaking eczema.

Pharmacokological

bisilkon contains betamethason, a glucocorticoid that clearly shows the general characteristics of corticosteroids; Clotrimazol is an imidazol group antifungal; Gentamicin is an aminoglycoside antibiotic.

clotrimazol

Links to phospholipids in the fungal cell membrane, changing the permeability of the membrane, causing loss of intracellular essential substances that lead to the destruction of mushroom cells.

Effect spectrum: In vitro, clotrimazol have the effect of restraining and killing fungi, depending on the concentration, with Trichophyton Rugbrum, Trichophyton Mentagrophytop, Epidermophyton Floccosum, Microsporum Canis and Candida species. It also works on some gram -positive bacteria (Staphylococcus aureus, Streptococcus pyogenes), Gram -negative bacteria (bacteroides, gardnerella vaginalis), trichomonas.

The on -site effect of the drug surface depends not only on the lesions and mechanisms of the drug, but also on the viscosity, no water and the acidity of the preparation.

betamethason

is a synthetic corticosteroid, which has a very strong glucocorticoid effect, accompanied by a negligible metabolic effect on water salt. Betamethasone has anti -inflammatory, anti -rheumatism, anti -allergy and high doses that inhibit immunosuppressive effects. Suitable for diseases that keep the country unfavorable.

gentamicin

An antibiotic belonging to the aminoglycoside group has bactericidal effects through inhibiting bacterial protein biosynthesis. Drugs on sensitive bacterial cells through positive transportation process depend on oxygen. In the cell, the drug is attached to the subunit of 50S of sensitive bacterial ribosom, the result causes the bacterial cell membrane to be disabled and thereby inhibit cells to grow.

Gentamicin sensitive bacteria strains include: Streptococci (Beta hemolytic, alpha hemolytic group A), S. Aureus (positive coagulase, negative coagulase and some penicillinase production strains) and Gram -negative bacteria, Pseudomonas Aeruginosa, Aerobacter Aerogenes, Escherichia COLI, Prodotus, Production Vulgaris, Kiebsiella Pneumoniae.

pharmacokinetics

clotrimazol

Very little absorbed into the blood: 6 hours after applying the cream and the 1% clotrimazol solution has radioactive isotope on the skin intact and on the skin of acute inflammation, concentration. Clotrimazol varies from 100 mcg/ml in the horny layer to 0.5 - 1 mcg/ml in the spine layer and 0.1 mcg/ml in the subcutaneous fat layer.

betamethason

easy to absorb when used on the spot. When applied, especially if the skin is torn or when the skin is torn, there may be a amount of betamethasone that is absorbed enough for the whole body effect. Betamethasone is quickly distributed into all tissues in the body. During the circulation, Betamethason binds to plasma proteins about 60%, mainly with globulin, with little with albumin. Selling time: 36 - 54 hours. The metabolism is mainly through the liver and excreted mainly through the kidney with the metabolic rate of less than 5%.

gentamicin

No pharmacokinetic information of the drug in scientific documents has not been seen. However, the drug can absorb the whole body after use on the spot on the skin of the skin loss of skin, burns, wounds and cavities of the body eliminate bladder and joints.

Before taking Bisilkon Bidiphar skin cream for skin diseases, bacterial infections (10g)

How to use

Use topical skin.

Dosage

Rinse the infected skin, blot dry and apply enough thin layer of the drug to the affected skin 2 times/day, in the morning and evening. For effective treatment, Bisilkon should be applied regularly.

Different treatment time depends on the degree, position of the disease and the patient's response. However, when applying the drug for 3-4 weeks without showing signs of improvement, it is advisable to stop the drug and examine again.

What to do when overdose?

Symptoms

Overdose or prolonged local corticosteroids can inhibit the pituitary -adrenal function, leading to secondary adrenal insufficiency and cause signs of adrenal radio hyperplasia, including Cushing. The expression of overdose due to clotrimazol use rarely occurs.

Gentamicin's single overdose usually does not cause symptoms. However, excessive or prolonged use can lead to excessive growth of lesions caused by non -sensitive microorganisms.

How to handle

Symptomatic treatment is necessary. Symptoms of acute overdose corticosteroids often recover. Treatment of electrolyte imbalance if needed. In the case of chronic poisoning, stop use slowly corticosteroids is recommended. If the excessive growth of non -sensitive microorganisms occurs, stop using Bisilkon and conduct appropriate treatment. Notify the doctor the unwanted effects when using the drug.

What to do when you forget the dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

Side Effects

When using Bisilkon Bidiphar 10 g, you may experience unwanted effects (ADR).

The adverse reactions rarely occur; Including skin discoloration, reducing pigmentation, hot burning, erythema, rust and itching.

Of the nearly 1,000 patients treated on the spot skin diseases with clotrimazol, about 95% of well tolerated. The adverse reactions reported include: pain, blistering, peeling, edema, urticaria and skin stimulation.

Gentamicin treatment may cause fleeting stimulation and often do not require treatment.

Local adverse reactions have been reported with the use of topical corticosteroids (especially sealed areas) such as hot, itching, irritation, dry skin, folliculitis, hair, acne -like rash, pigmentation, dermatitis around the mouth, allergic contact dermatitis, blisters, secondary infections, skin atrophy, millet atrophy.

topical corticosteroids that can inhibit the hypothalamus - pituitary - adrenal gland (HPA) with recovery, Cushing syndrome, hyperglycemia, diabetes and osteoporosis in some patients. In some corticosteroid users with wide area or in an ĐI -ĐID, the inhibition of the hypothalamus - pituitary - adrenal glands must be checked regularly through the concentration of cortisol in the blood, free cortisol in urine or testing ACTH.

Should be avoided for long -term treatment, especially sealed because the drug can cause side effects like using oral corticosteroids.

Simultaneous use with systemic aminoglycosides can be toxic from the accumulation of antibiotics. Except for special cases, avoid continuous use for long days and in a tight condition. Special note the above information for babies and children.

Do not use to treat systemic fungal infections; If irritated or sensitive, the medication must be stopped and treated appropriately; Must treat the drug enough time as prescribed even though the symptoms are relieved; Tell the physician if there is a sign that increases irritation in the topical area (red, itching, burns, blisters, swelling), signs of hypersensitivity. Avoid sources of infection or re -infection.

    Drug interaction

    betamethason

paracetamol: corticosteroid liver enzyme, which can be increased to form a toxic metabolite with the liver.

3 -round antidepressants: These drugs do not reduce and may increase mental disorders caused by corticosteroids; Do not use these antidepressants to treat the above unwanted effects.

Oral or insulin anti -diabetic drugs: increase blood glucose levels, so the dose should be adjusted. Glycosid Digitalis: Increases the possibility of arrhythmia or toxicity of digitalis and hypotension.

Phenobarbiton, Phenytoin, Rifampicin or Ephedrin: can increase metabolism of corticosteroids and reduce their treatment effects. Corticosteroids simultaneously with anticoagulant drugs can increase or decrease anticoagulants.

Non -steroidal anti -inflammatory or alcoholic drugs in combination with glucocorticoid may increase the occurrence and severity of the gastrointestinal ulcer. Corticosteroids may increase the concentration of salicylate in the blood. Must be cautious when coordinating with aspirin in case of reducing blood prothrombin.

When using bethamethasone may increase the concentration or effects of cholinesterase inhibitors, amphotericin B, cyclosporin, strap diuretic, natalizumab, thiazid diuretic. In contrast, some shared drugs will increase the concentration or effect of betamethasone such as antifungal drugs belonging to azol derivatives, calcium channel blockers, quinolon antibiotics, macrolid, tratuzumab, estrogen.

    clotrimazol

There has been no notice of the effect of copper or antagonism between clotrimazol and nystatin, amphotericin bhay flucytosin and the Candida albicans.

    gentamicin

There is no gentamicin drug interaction when using the skin on the skin in scientific documents. However, the drug can absorb the whole body after use on the spot on the skin of the skin loss of skin, burns, wounds and cavities of the body eliminate bladder and joints, so it can cause interaction of the system such as: Increasing the effects of Abobotulinumtoxin A, the derivatives of bisphosphonat, carboplate, colistimethate, cycloscedin Onabotulinumtoxin A, rimabotulinnumtoxin B. Inhibit the activity of d-galactosidase, so interact with Agalsidase Alpha and Agalsidase Beta; BCG vaccine, Gali nitrate, typhoid vaccine. Hypercalcemia can occur in patients treated with aminoglycosides and bisphosphonat.

Reducing excretion through the kidneys of zalcitabin.

Increased risk of kidney toxicity or hearing when combined with kidney toxic drugs (such as other aminoglycosides, vancomycin, some drugs of the cephalosporin family) or hearing toxicity (such as ethacrynic acid, ...). This risk also increases when used simultaneously with neurotransmitter inhibitors. The use of anti -vomiting drugs such as Dimenhydrinat can cover the first symptoms of vestibular poisoning.
