Uses

indications

Boncium 1250 mg Gracure 3x10 is indicated in the following cases:

Preventing and treating calcium and vitamin D3 in adults such as osteoporosis, pregnant mothers, nursing mothers.

Pharmacokology

calcium is the fifth element in the body. Mostly in the bone. The skeletal system contains about 90% of calcium in the body.

Calcium salt plays an important role in regulating the permeability of the cell membrane for sodium and potassium ions, the integrity of the mucosa, the adhesion of the cell. Increased calcium concentration decreases permeability and vice versa. Calcium carbonate is the form of the highest calcium content (40%).

Vitamin D3, along with parathyroid hormones and calcitonin regulates the calcium concentration in serum. The main biological function of Vitamin D3 is to maintain normal calcium and phosphor levels in plasma by increasing the effectiveness of the absorption of minerals from the diet, in the small intestine.

pharmacokinetics

calcium is absorbed in the upper part of the small intestine. In healthy adults, the amount of absorption is about 1⁄3 of the amount of food. Vitamin D3 increases the absorption of calcium and phosphate from the intestine and mobilizes calcium into the bone. Vitamin D3 stimulates the absorption of phosphate from the intestine and this leads to the effect of vitamin D3 on calcium transport.

In plasma, 40% of calcium in the form of protein, 10% dispersed and created complexes with anion such as citrate and phosphate, the rest dispersed in the form of calcium ions, phosphate present in plasma, intracellular fluid, collagen and bone tissue.

Calcium is excreted through the digestive system such as saliva, bile and pancreatic juice to discharge through stools, calcium is also significantly excreted through breast milk and sweat, calcium is eliminated through urine and is associated with sodium excretion. Calcium is re -collected in the tube close to the influence of PTH and in the distance below the influence of vitamin D3.

Vitamin D3 is well absorbed through the gastrointestinal tract from the small intestine. The bile is necessary for absorption of vitamin D3 in the intestine. Because vitamin D3 is soluble in lipids, it is concentrated in micro -microorganisms, and is absorbed in the lymphatic system; approximately 80% of vitamin D3 used by oral is absorbed by this mechanism. Vitamin D3 and its metabolites rotate in the blood associated with specific alpha globulin. Half -life in the plasma of Vitamin D3 is 19-25 hours, but the drug is stored for a long time in fat tissue.

Vitamin D3 is hydroxylation in the liver forming 25-hydroxycoleciferol. This substance continues to be hydroxylated in the kidneys to form 1.25-dihydroxycolecolecerol active substances and the derivatives of 1.24,25-trihydroxy.

The liver is the place where Vitamin D3 converts 25 - OHD, this substance is linked to protein and the circulation in the blood. In fact, 25-OHD has higher affinity with protein than the mother compound. Deading 25 - Hydroxy has half a lifetime of 19 days and is the main form of vitamin D3 in the blood. The concentration in the stable state of 25 -oHD is 15 - 50 nanogam/ml.

Vitamin D3 and its metabolites are excluded mainly through bile and feces, only a small amount appears in the urine.

How to use

Boncium 1250 mg Gracure 3x10 is an oral medication.

Dosage

1 capsule 2 times/day.

Do not use for more than 1 month without a doctor's instructions.

No dose adjustments in patients with hepatic failure.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?

Expression

may occur in vitamin D3 when treating high or prolonged doses or when increased in response to normal doses of vitamin D3 and will lead to clinical manifestations of calcium metabolic disorders. Some children may increase the reaction with a small amount of vitamin D3. In adults, Vitamin D3 can be used by an overdose of vitamin D3 in the case of parathyroidism or Vitamin D3 with a strange dosage. Poisoning may also occur after the wrong dose of adult vitamin D3.

The amount of vitamin D3 causes great enhancement of vitamin D3 changes from one person to another. Typically, people with normal and normal adjacent functions are normal with vitamin D3 continuously drinking 50000 units of vitamin D3/day or more daily, may be poisoned with vitamin D3. Vitamin D3 is especially dangerous for those who are using digitalis, because the toxicity of cardiac glycosides increases when blood calcium hypercalcemia.

Signs and initial symptoms of Vitamin D3 poisoning are signs and symptoms of hypercalcemia (see the unwanted effect).

Handling

Stop drugs, stop calcium supplements, maintain a diet with low calcium, drink plenty of water or infusion. If necessary, corticosteroids can be used or other drugs, especially diuretics increase calcium (such as Furosemid and ethacrynic acid), to reduce carcinomic concentration in serum. Can use artificial kidney dialysis or abdominal separation to release free calcium from the body.

If vitamin D3 poisoning is just taken, it can be prevented from absorbing vitamin D3 by causing vomiting or gastric lavage. If the drug has passed the stomach, mineral oil treatment can promote vitamin D3 elimination. Because the metabolites 25 - OH of Colecalciferol are accumulated in the body, blood calcium hypercalc can last for 2 months or longer, after long -term treatment with large doses of these drugs.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

Side Effects

When using Boncium 1250 mg gracure 3x10, you may experience unwanted effects (ADR).

Rare digestive disorders.

High doses can lead to hypercalcemia and toxicity of Vitamin D3 to have some unwanted effects as follows:

Common, ADR> 1/100

  • Neurological: Weak, tired, sleep, headache 1/100
  • urinary - genital: sex reduction, renal calcium infection, renal dysfunction (leading to multi -urinary, night urine, thirst, reducing the proportion of urine, proteinuria).

    Rare, ADR

  • Cardiovascular: Hypertension, arrhythmia. Reducing alkaline enzyme concentration in serum.

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Boncium 1250 mg Gracure 3x10 contraindicated in the following cases:

  • Sensitive to any ingredients of the drug.
  • Patients with renal impairment, hypercalcemia and hyperthyroidism, hypercalcium.

People with kidney stones, toxicity of vitamin D.

    Be cautious when using

    Concentrated use with ceftriaxone antibiotics.

Avoid using if there is a hypercalcemia or in the case of kidney stones.

Be careful when using calcium salts for patients with renal impairment, heart disease or sarcoidosid disease.

Do not use calcium preparations with digoxin.

Monitoring blood calcium, calcium and kidney function when used for a long time; The risk of increasing acupuncture calcium in patients with immobile osteoporosis; Do not use with other preparations containing calcium and vitamin D.

The ability to drive and operate machinery

does not affect driving and operating machinery.

    Pregnancy

drugs used for women during pregnancy.

If using vitamin D3 at a larger dosage than the recommended daily daily dose (RDA) for normal pregnant people (400 ĐVQT), the risk may occur, so it should not be used for vitamin D3 at a larger dose than RDA for pregnant women. Aortic valve stenosis has occurred, kidney disease and mental retardation and/or developmental retardation when there is prolonged hypercalcemia in young children and babies that their mother has hypercalcemia during pregnancy. Hypercalcemia during pregnancy can reduce the concentration of parathyroid hormones in newborns leading to blood calcium, seizures and epilepsy.

If the diet is not enough vitamin D3 or lack of exposure to ultraviolet radiation, vitamin D3 should be supplemented to the dose of RDA during pregnancy.

    Breastfeeding period

The drug is used for women in breastfeeding period.

Vitamin D3 secretes milk, so do not use vitamin D3 at a larger dose than RDA for nursing mothers. Vitamin D3 should be used, if the diet is not enough vitamin D3 or lack of exposure to ultraviolet radiation.

Be cautious when supplementing with vitamin D3 breastfeeding.

    Interactive drug

The following drugs inhibit calcium excretion of calcium: thiazid, clopamid, ciprofloxacin, chlorthalidon, anti -convulsions.

Calcium reduces the absorption of Demeclocyclin, Doxyciclin, Metacyclin, Minocyclin, Quinolones, iron and other essential minerals.

Calcium increases the toxicity of cardiac glycosides because of increased blood calcium levels will increase the inhibition effect of Na ” - K” - Atpase of the cardiac glycoside.

glucocorticoid, phenytoin reduces the absorption of calcium through the gastrointestinal tract. Diet with phytat, oxalate reduces calcium absorption because it forms difficult complexes to absorb.

Do not simultaneously use vitamin D3 with cholestyramine or colestipol hydrochloride, as it can lead to reduced absorption of vitamin D3 in the intestine.

Excessive use of mineral oil can hinder the absorption of vitamin D3 in the intestine.

Simultaneous treatment of vitamin D3 with diuretic gums for parathyroid disabilities can lead to hypercalcemia.

Do not simultaneously use vitamin D3 with phenobarbital and/or phenytoin because these drugs can reduce the concentration of 25-hydroxy-canCalciferol in plasma and increase vitamin D3 convert into non-active substances.

Do not simultaneously use vitamin D3 with corticosteroid corticosteroids that hinder the effect of vitamin D3.

Do not simultaneously use vitamin D3 with cardiac glycosides due to hypercalcemia, leading to arrhythmia.
