Uses

Indications

Calcium - D drugs are indicated to use calcium supplements in the following cases: The body is deficient in calcium, growing children, pregnant or lactating women, elderly people with bone porous, people under corticosteroids.

Pharmacokological

Calcium is an essential cation to maintain the function of the nervous system, muscle, bone and absorbent properties through cell membranes, capillaries. Calcium is present in the bone with the corresponding mineral content of about 40% of bone mass.

The main biological function of vitamin D is to maintain normal calcium and phosphorus concentrations in serum, by increasing the absorption of these substances from food in the small intestine. Activation of Colecalciferol mobilizes calcium from bones into the blood, boosts the reabsorption of phosphate in the renal tubules and directly acts on bone formation cells, stimulates bone growth.

pharmacokinetic

calcium is absorbed about 30% of the dosage through the gastrointestinal tract. 99% of calcium in the body concentrated in bones and teeth. About 50% of the total amount of blood calcium, with 10% in the form of activation associated with citrate, phosphate and other anions, about 40% are linked to protein, especially albumin. Calcium is eliminated through feces, urine and sweat. Excretion through the kidney depends on glomerular filtration and calcium reabsorption in the renal tubules.

Vitamin D, good absorption through the gastrointestinal tract. The bile is essential for the absorption of vitamin D in the intestine. After absorption, Colecalciferol (Vitamin D3) into the bloodstream through the microorganism of the white mach, then combined mainly with a special alpha-globulin (protein with vitamin D). The metabolites (hydroxylation) of Colecalciferol are also circulating in the blood combined with Alpha-Globulin. In the liver, Colecalciferol is hydroxylation in the mitochondria into 25-hydroxycoleciferol. This compound is hydroxylation in the kidneys thanks to the vitamin D-1-hydroxylase enzyme into a metabolic substance with 1.25-dihydroxycoleciferol. Continue to transform more in the kidney forming a derivative of 1.24.25-trihydroxy. Vitamin D, and metabolites mainly excreted in bile and fertilizer, only a small amount through urine.

Before taking Calcium - D Mekophar supplements with calcium (10 blisters x 10 tablets)

How to use

Drugs used by oral, can be taken with or without food.

There is no special requirement on drug treatment before and after use.

Dosage

Adults and children> 12 years old: Take 1 tablet at a time, 3 times a day.

Children 6 - 12 years old: Take 1 tablet each time, 2 times a day.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose?

Overdose

High doses may have symptoms of hypercalcemia.

Early symptoms of hypercalcemia include: muscle weakness, fatigue, chicken sleep, headache, anorexia, dry mouth, mouth with metal flavor, nausea, vomiting, abdominal pain, constipation, dizziness, tinnitus, eye coordination, rash, reducing muscle tone, muscle pain, bone pain.

Symptoms of late as a result of hypercalcemia: kidney calcification, kidney stones, kidney damage (urination, night urination, drinking plenty of water, concentrated urine).

Handling

Stop supplementing drugs and calcium, maintain a low -calcium diet, drink water or intravenous fluid. If necessary, use corticosteroids or calcium diuretics such as Furosemid, acidethacrynic to reduce serum calcium concentration. Can give hemolysis or peritoneal.

If you first drink, give your stomach or cause vomiting. If the drug has passed the stomach, drink mineral oil to promote elimination through feces.

After blood calcium returns to normal, it can be treated again, with lower doses.

Actively monitor for timely management measures.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Side Effects

When using calcium - D you may experience unwanted effects (ADR).

Common, ADR> 1/100

  • No report.
  • Uncommon, 1/1000

  • Metabolic disorders: hypercalcemia, hypercalciuria.
  • Rare, 1/10,000

  • Gastrointestinal disorders: constipation, flatulence, nausea, abdominal pain, diarrhea.
  • Metabolic disorders: Milk - Alkaline syndrome.

Risk of hypercalcemia and hyperactive blood phosphorus:

  • Symptoms: Anorexia, headache , nausea, vomiting, abdominal pain, constipation.
  • Regularly determine serum calcium concentration, should be maintained at 9 -10mg/Decilid (4.5 - 5mg/liter). Serum calcium concentration usually does not exceed 11mg/Decilite.
  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Calcium - D contraindications in the following cases:

  • Hypersensitivity to one of the ingredients of the drug.

    Precautions when used

Caution should be used for patients with kidney failure, kidney stones, heart disease, atherosclerosis. In case of hypercalcemia or signs of kidney function damage, dose reduction or stop use of drugs.

Dosage reduction or temporary stop use of calcium concentration in urine exceeds 7.5mmol/24 hours.

Be cautious when taking medication for people who are taking cardiac glycosides, thiazid diuretic because of hypercalcemia causing arrhythmia in these objects.

Using drugs in patients with immobile osteoporosis increases the risk of hypercalcemia.

Observe the phosphate concentration in serum during treatment, to reduce the risk of peanut calcification.

Monitoring blood calcium levels regularly, especially initially and when there is a suspected poisoning symptom.

Preparations containing lactose, cautious when used for patients with rare genetic disorders about Nap Galactose, Lapp Lactase deficiency or Glucose-Galactose

The ability to drive and operate machinery

There is no research data showing the effect of drugs on driving and operating machinery.

    Pregnancy

can use drugs during pregnancy in case of calcium and vitamin D3 deficiency. The daily dose must not exceed 1500mg of calcium and 600 IU of Vitamin D3. Animal research shows a toxic effect on reproductive process when using high doses of vitamin D. It is necessary to avoid overdose of vitamin D and calcium in pregnant women due to prolonged hypercalcemia, which can lead to intellectual and physical development retardation in the fetus. There is no sign that vitamin D, causing teratogenicity in humans when used at the dose of treatment.

    Breastfeeding period

The drug can be used during breastfeeding. Calcium and vitamin D are excreted into breast milk. Therefore, attention should be paid when using more vitamin D preparations for children.

    Medicinal interaction

    Interaction of drugs:

Avoid combined with thiazid, cardiac gangsters, cardiac glycosides and verapamil due to increased blood calcium.

Corticosteroids reduce calcium collection. If you have to use these drugs simultaneously, increase the dose of calcium - d.

Calcium carbonate interferes with the absorption of preparations containing tetracyclin, so it is advisable to use tetracyclin 2 hours ago or 4-6 hours after using calcium - d.

Do not simultaneously use vitamin D with cholestyramin or colestipol hydrochloride, due to reducing the absorption of vitamin D in the intestine.

Excessive use of mineral oil can hinder the absorption of vitamin D in the intestine.

Absorption of quinolon antibiotics is reduced if used simultaneously with calcium, should take these drugs 2 hours before or 6 hours after using calcium - d.

Calcium salt reduces the absorption of zinc ions, so the drugs should be used at least 2 hours apart.

Do not simultaneously use vitamin D with rifampicin, phenobarbital, phenytoin, due to increased vitamin D conversion into non -active substances.

Oxalic acid (found in spinach, sour tamarind, great Hoang) and phytic acid (found in toads) can inhibit calcium absorption by creating with calcium ions into insoluble compounds. Patients should not use calcium -containing products within 2 hours after using foods rich in oxalic acid and phytic acid.

    The cavalry of the drug:

Due to the absence of studies on the correlation of the drug, not mixing this drug with other drugs.
