Uses

indications

Carduran 2mg drug is indicated in the following cases:

  • Treatment of hypertension: Doxazosin is indicated in the treatment of hypertension and can be used alone to control blood pressure in most patients with hypertension. In patients who are not well controlled by a mere antihypertensive agent, Doxazosin can be used in combination with other drugs such as thiazid diuretic, sympathetic beta blockers, calcium channel blockers or conversion inhibitors. Doxazosin may be used in patients with prostate enlargement with or not with hypertension. While blood pressure changes are clinically insignificant in normal blood pressure patients with prostate fibroids, patients with hypertension with prostate fibroids can be effectively treated with both diseases with monon treatment.

Use doxazosin for hypertension patients to reduce blood pressure significantly clinically due to reducing body blood vessel resistance. This effect is thought to be due to selective inhibition on the sympathetic Alpha receptor on the wall of the circuit. With a single dose of the day, Doxazosin significantly reduces blood pressure throughout the day and 24 hours after drinking.

Blood pressure decreases slowly and reaches up to 2 to 6 hours of medication. In patients with hypertension, blood pressure during treatment with dexazosin is the same in both lying and standing positions. Unlike other non -selective alpha receptors, greasy drugs are not observed during long -term treatment with Doxazosin. Less see the phenomenon of increasing plasma renin activity and tachycardia on long -term treatment cases.

Doxazosin has a beneficial effect on blood lipids, significantly increasing the high density lipoprotein ratio (Hight - HDL)/total cholesterol, decreased triglycerides and total cholesterol. Therefore, the drug has an advantage over diuretics and receptor blockers of sympathetic beta, drugs that have adverse effects on blood lipid indicators. Based on the relationship that has been established between hypertension and lipid disorders and coronary diseases, this beneficial effect of treatment with doxazosin on both blood pressure and blood lipids will help reduce the risk of developing coronary diseases.

DOXAZOSIN therapy shows reducing left ventricular hypertrophy, inhibiting platelet gathering and enhancing the ability to activate the active substance plasminogen tissue. In addition, doxazosin improves insulin sensitivity in less sensitive patients.

Doxazosin has been shown to have no unwanted effect on metabolism and appropriate when used in diabetics, bronchial asthma, left ventricular failure, gout.

A In vitro study showed that the antioxidant properties of metabolic forms of 6/4 and 7 - hydroxy of doxazosin at 5 micromol concentrations.

In a verified clinical study in hypertension patients, doxazosin treatment has been associated with improving erectile dysfunction. In addition, patients who drink doxazosin have been reported to be less than cases of new erectile dysfunction compared to patients who take other antihypertensive drugs.

    Prostate fibroids

The use of doxazosin for patients with prostate enlargement significantly improves urinary tract function and other symptoms. Effects on prostate enlargement are thought to be due to the selective inhibition of the sympathetic alpha receptor on the support tissue and the prostate envelope and the bladder neck.

DOXAZOSIN has been shown to inhibit the division 1A of the sympathetic alpha receptor, which is a group that accounts for more than 70% of the doxazosin in prostate fibroid patients.

Doxazosin has also been shown to be effective and safe for long -term treatment for prostate enlargement (for example, up to 48 months).

    Pharmacokinetics

    absorption

After taking the treatment dose, Doxazosin is well absorbed, the blood concentration in the blood is achieved after about two hours.

    Metabolism/excretion

Divide divided into two stages with a half -life of 22 hours. This is the basis for the only dosage of the day. Doxazosin is metabolized in the body, only less than 5% doxazosin excreted in constant form.

Dynamic studies in patients with renal failure have shown that there is no significant difference in pharmacokinetics of doxazosin in patients with renal impairment and patients with normal renal function.

There are only very few data on the use of doxazosin in patients with liver failure and the effects of drugs that have been known to have the effect on liver metabolism (such as cimetidine). In a clinical study in 12 subjects with average liver failure, the single dose of Doxazosin increased 43% AUC and reduced 40% of the clearance of oral dose. As well as for all liver metabolic drugs, be cautious when using doxazosin in patients with liver function changes.

About 98% doxazosin attached to plasma proteins. Doxazosin is metabolized mainly by the methyl reducing reaction of the methoxy group at the ortho position and the hydroxylation reaction.

doxazosin is metabolized in the liver. In vitro studies show that the main excretion path is through CYP 3A4; However, the transformed paths through CYP 2D6 and CYP 2C9 also participate in the elimination process, but at a lower level.

    Prelise Clinical Safety Data

    Cancer ability

Use Doxazosin lasting up to 24 months) with a maximum tolerance of 40 mg/kg/day in mice and 120 mg/kg/day in mice showing no evidence of cancer likely. The highest dose is used for rats and mice that are associated with AUC (the method of measuring the system concentration), the corresponding AUC value is 3 and 4 times higher than the AUC of the person at 16 mg/day.

The ability to cause gene mutations

Research on gene mutations shows that there is no effect on the exchange or related to drug use at both chromosomes and under chromosomes.

    Perfect fertility

Studies on white mice show that the fertility of the male decreases when treated with Doxazosin at a dose of 20mg/kg/day (does not occur at a dose of 5 mg or 10 mg/kg/day), about 4 times the AUC value achieved in humans at a dose of 12mg/day. This effect is turned upside down for 2 weeks if not taken. There is no report on any effects of doxazosin for male fertility.

    How to use

take the tablet with a glass of water. Can be taken in the morning or evening.

    Dosage

    adults

    Hypertension

Doxazosin dose may vary from 1 mg to 16 mg per day. Therapy should start at a dose of 1 mg once a day for 1 or 2 weeks to minimize the risk of posture and/or fainting.

may then increase the dose to 2 mg once daily for 1 or 2 weeks. If necessary, daily dose may increase to 4 mg, 8 mg, and 16 mg with similar dose adjustments based on the patient's response to achieve desired lower pressure. The usual dose is 2 mg to 4 mg once daily.

    Prostate fibroids

The starting dose is recommended for doxazosin 1 mg once a day to minimize the risk of posture and/or fainting. Depending on the flow of urine and symptoms of prostate enlargement in each patient, the dose may be increased to 2 mg, then 4 mg and may increase to the maximum dose recommended by 8 mg. The recommended dose time period is from 1 to 2 weeks. The usual dose is 2 mg to 4 mg once daily.

    used in the elderly

Recommendation of the usual dosage as in adults.

Used in patients with renal failure

Due to the pharmacokinetics of doxazosin does not change in patients with renal failure and there is no evidence that doxazosin aggravates the existing renal failure in advance, so the usual dosage can be used in these patients.

    used in children

Safety and effectiveness of doxazosin for children has not been set up.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do

do when overdose? Other supportive methods should be done if appropriate on each specific case. Doxazosin is connected to high protein, not indicating dialysis.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

This drug is only used by a doctor. Before using the drug, you need to read the user manual carefully and refer to the information below.

    contraindicated

    doxazosin contraindicated for:

  • Patients who already know are hypersensitivity to Quinazolin, Doxazosin, or any excipients of the drug.
  • Patients with a history of hypotension.

Patients with prostate fibroids accompanied by urinary tract congestion, chronic urinary tract infections or bladder stones.

  • During breastfeeding (for indications for hypertension treatment).
  • Patients with low blood pressure (for the prescription of prostate fibroids).

  • Contraindicated use of monon treatment doxazosin for patients with anuria with or without renal failure.
  • Precautions when using

    Hypotenary pressure posture/fainting

Like all sympathetic alpha blockers, the phenomenon of hypotension occurs on a very small percentage of patients, manifested by dizziness, fatigue or rare consciousness (fainting) especially at the beginning of treatment.

When starting to treat with any sympathetic alpha blockers should guide patients on how to avoid symptoms caused by posture hypotension and treatments if they progress. It is necessary to warn patients to avoid situations that may cause injury if the phenomenon of dizziness or fatigue occurs when they start to take the drug.

    Acute cardiovascular conditions

Like any anti -hypertension vasodilators, need to be cautious in medical practice when using doxazosin for patients with acute cardiovascular conditions as follows:

  • pulmonary edema due to aortic stenosis or mitral valve.
  • Heart failure due to an increase in cardiac weight.
  • Heart failure must be caused by pulmonary embolism or pericardial effusion.

  • Left ventricular failure accompanied by low filling pressure.
  • Simultaneously used with phosphodiesterase enzyme inhibitors 5

Be cautious when indicated simultaneously doxazosin with a phosphodiesterase type 5 (PDE - 5) inhibitor because this can lead to symptoms of hypotension in some patients.

    Liver function impairment

Like any drugs that are completely metabolized through the liver, the use of Doxazosin needs to be cautious in patients with signs of liver function impairment.

    Soft iris syndrome during surgery

has noticed soft iris syndrome during surgery (Intraoperative Floppy Iris Syndrome - FIS, a variant of small pupils syndrome), which has been recorded in cataract surgery in some patients who are taking or have used sympathetic alpha blockers. Due to the soft iraculous syndrome during surgery, it may increase complications during the surgery, it is necessary to tell the doctor that the patient has or is using sympathetic alpha blockers before surgery.

    Penis pain

The erection and long penis tip have been reported when using sympathetic alpha blockers including doxazosin during the circulation process. In the case of an erection that lasts more than 4 hours, patients need to find medical assistance immediately. If the penis pain is not treated immediately, the penis and impotence may occur permanently.

    Lapp Lapp Lactase

Patients with rare genetic problems such as galactose intolerance, lactase deficiency or glucose - galactose absorption disorders should not use this drug.

The ability to drive and operate machinery

The ability to participate in activities such as driving or operating machinery may be impaired, especially when starting treatment with Doxazosin.

    Pregnancy

for the indication for hypertension treatment: Because there is no adequate and well -controlled research on pregnant women, carduran safety when used during pregnancy has not been set. Therefore, during pregnancy, only carduran only when the doctor thinks that treatment benefits are greater than the potential risks. Although not observing teratogenic effects on animal tests, the ability to survive the embryo decreases in animals when using very high doses. These doses are approximately 300 times the recommended maximum dose in humans.

    The period of breastfeeding

Contraindications to use doxazosin during breastfeeding due to animal studies showed that doxazosin accumulated in breast breasts, and there is no information about the excretion of breast milk of breastfeeding women. Carduran's clinical safety during breastfeeding has not been established, thus contraindicated to use carduran for breastfeeding mothers.

Mothers should stop breastfeeding when they need to be treated with dexazosin.

    Drug interaction

Timeless with PDE - 5 inhibitors

Be cautious when indicated simultaneously doxazosin with a phosphodiesterase type 5 (PDE - 5) inhibitor because this can lead to symptoms of hypotension in some patients.

    CYP3A4 inhibitors

In vitro studies show that Doxazosin is a substrate of CYP 3A4, cautious when using simultaneous doxazosin with a powerful CYP 3A4 inhibitor, such as Clarithromycin, Indinavir, Itraconazol, Ketoconazole, Nefazodon, Nelfinavir, Ritonavir, SaQuavin or TelithromyCin or Telithromycin voriconazole.

    Other drugs

Most of the plasma doxazosin in plasma are associated with protein (98%). In vitro data on human plasma indicates that Doxazosin does not affect the binding of protein of digoxin, warfarin, phenytoin, or indomethacin. Clinically used experience has shown that Doxazosin does not have any clinically adverse drug interactions with thiazid, Furosemid, sympathetic beta blockers, nonsteroidal anti -inflammatory drugs, antibiotics, oral hypoglycemic drugs, urinary uric acid or anticoagulants.

In an open, random label study, there is a place where to be placebo on 22 healthy male volunteers, when using a single dose of 1 mg Doxazosin on the first day of the 4 -day cimetidine (400 mg, twice a day) increases 10% AUC (area under the curve) of the average of Doxazosin, and there is no change in the average statistical time for the average of the average of the average of the average of the average of the average of the average of the average of the average of the average of the average of the average of the duration of the average of the average of the duration of the average of the average of the average of the average of the average of the average of the average of the Doxazosin average.

The average increase of 10% of AUC for doxazosin when used with cimetidin is still in the deviation that may occur between individuals (27%) for the average AUC of doxazosin when used with placebo.

    Storage
