Uses
indications
Carvedilol drug is indicated for treatment in the following cases:
Carvedilol has two main mechanisms of anti -hypertension.
A mechanism of action is vasodilation, mainly due to the selection of Alphal receptor selection, thus reducing peripheral blood vessel resistance.
Carvedilol also has a non -selective beta receptor.
Effects on vasodilation and receptor blockers occur in the same dosage level. The effect of mild calcium channel blockers. Carvedilol does not have the effect of the beta movement, but only has a weak effect. Clinical research has shown that the combination of alphal and beta receptor blockers causes the heart frequency to decrease or not change when resting, maintaining the number of volume and blood flow in the kidney and peripheral.
Dynamic pharmacokinetics
Biology of Carvedilol on average 20 - 25% because of incomplete absorption and initial strong transformation.
After taking a dose of about 1-3 hours, the plasma concentration is maximum. The concentration in plasma increases linearly with the dose, within the willow recommended. The distribution volume is about 2 liters/kg. Normal plasma clearance is about 590 ml/min.
Three metabolites have the active active receptor of beta, but the vasodilation effect is weak. However, the concentration of these metabolites is low, and therefore does not contribute to the effect of the drug.
The half -life of Carvedilol elimination is 6 - 7 hours after drinking. A small percentage of about 15% of oral dose is excreted through the kidney. Before taking Carvedilol 6.25 Savi treatment for hypertension, congestive heart failure (3 blisters x 10 tablets)
How to use
oral medication. To reduce the potential for hypoglycemia, Carvedilol is recommended to drink with food.
In addition, the manufacturer suggests that it can reduce vasodilation manifestations in patients using the enzyme inhibitors transferred by taking Carvedilol 2 hours before taking the enzyme inhibitors.
Dosage
Dosage for hypertension treatment
The first dose of 12.5 mg (2 Savi Carvedilol 6.25 tablets), 1 time; increased to 25 mg after 2 days, drink 1 time.
Another way, the first dose of 6.25 mg oral twice a day, after 1 to 2 weeks increased to 12.5 mg, taken twice a day. If necessary, the dose may increase, apart at least 2 weeks apart, up to a maximum of 50 mg, 1 time, or divided into several doses.
For the elderly, 12.5 mg orally once daily can be effective.
Dosage to treat angina
The first dose of 12.5 mg, taken 2 times a day; After 2 days increasing to 25 mg, 2 times a day.
Dosage to treat heart failure
Take the dose of 6.25 mg orally once a day for 2 weeks. After that, the dose may increase, if tolerated, up to 6.25 mg, taken 2 times a day. The dose may increase if tolerant, at least 2 weeks apart, the maximum dose is recommended 25 mg, taken twice a day, for patients weighing less than 85 kg, or 50 mg, taken twice a day, for people weighing over 85 kg.
Before starting the carvedilol therapy for congestion heart failure, the patient is taking a heart glycoside, diuretics, and/or medications inhibitors, which must be stable with the dose of those drugs. The risk of loss of heart failure and/or decreased severe severe blood pressure in the first 30 days of treatment.
Dosage to treat idiopathic heart disease
Dosage recommended 6.25 - 25.0 mg, taken 2 times a day.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
The most common heart symptoms include slow blood pressure lower heart rate.
Atrial block - ventricular, ventricular disorders in ventricular and heart stun may occur when serious overdose, especially with membrane inhibitors (e.g. propranolol). Impact on the central nervous system including convulsions, coma and respiratory stops common with propranolol and with
fat -soluble drugs and other membrane inhibitors.
Therapy includes treatment of seizures, hypoglycemia, hyperkalemia and hypoglycemia.
Slow heart rate and decrease in anti -Atropine, isoproterenol, or with a pacemaker that can use glucagon. QRS is expanded due to membrane inhibitor canxicity that can use hypertonic sodium bicarbonate. For multi -dose activated carbon, hemorrhage can only help eliminate the blockage - beta with small distribution, with half a long life, or low internal clearance (Acebutolol, Atenolol, Nadolol, Sotalol).
In an emergency, call the 115 emergency center immediately or go to the nearest local health station.
What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
Most of the unwanted effects are temporary and after a while. Most of these effects occur at the beginning of treatment. Unwanted effects are mainly related to pharmacological mechanisms and dose. The most common is dizziness (about 10%) and headache (about 5%).
Common: ADR> 1/100
Instructions on how to handle ADR
can minimize the risk of slow heart rate and other undesirable effects by starting treatment at low doses, then gradually increasing the dose, careful monitoring of diastolic blood pressure and heart frequency, and taking carvedilol with food.
Need to reduce the dose if the circuit frequency drops below 55 hours of dam. Avoid stopping the drug suddenly.
Patients must avoid standing up suddenly or standing still for a long time; Need to rest if you see dizziness or go, and consult a physician about the dose reduction.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Carvedilol drug is contraindicated in the following cases:
Be cautious when using
Caution must be used with Carvedilol in patients with congestive heart failure treatment with digitalis, diuretics, or angiotensin transferring enzyme inhibitors because of the atrial - ventricle may be slowed down.
Be careful in patients with diabetes with no or difficult to control, because beta receptor blockers can cover symptoms of blood glucose.
Must stop treatment when there is signs of liver damage.
Must be cautious in people with peripheral blood vessels, people with anesthesia, people with increased thyroid function.
If the patient does not tolerate other anti -hypertension drugs, caution can be used with very small doses of carvedilol for people with bronchospasm.
Avoid stopping the drug suddenly, to stop the drug for 1 to 2 weeks.
Consider the risk of arrhythmia, if using carvedilol simultaneously with anesthetic smell. The risk must be considered if combined with carvedilol with anti -arrhythmia group I.
Not yet determined the safety and effectiveness of Carvedilol in children.
Using drugs for women during pregnancy and lactation
The case of pregnancy
Carvedilol has caused clinical harms in the fetus. Only use this medication during pregnancy if the expected benefit is greater than the risk that may occur and as usual, not used in the last three months of pregnancy or almost giving birth.
Unwanted effects for fetuses such as slow heartbeat, lowering blood pressure, respiratory inhibition, reducing blood glucose and reducing body temperature in infants can be used by pregnancy.
Cases of breastfeeding
Carvedilol can excrete in breast milk.
There is no risk of unwanted effect in breastfeeding.
The effect of the drug on the ability to drive and operate machinery
should be careful to use the drug while driving or operating machinery because the drug causes side effects (dizziness, posture hypotension).
Drug interaction
reduces the effects of drugs
Rifampicin can reduce plasma concentrations of carvedilol to 70%.
The effect of other beta blockers decreases when combined with aluminum salts, barbiturates, calcium salts, cholestyramin, colestipol, non -selective blockbeant A, penicillin (ampicillin), salicylate, and sulfinyrazon due to reduced biodiversity and plasma concentration.
Increases the effects of drugs
Carvedilol can increase the effects of anti -herbal drugs, calcium channel blockers, digoxin.
Carvedilol's interaction with clonidin can lead to an increase in blood pressure and reduce heart rate.
cimetidine increases the effects and bioavailability of carvedilol.
Other drugs that are capable of increasing the concentration and effects of carvedilol include quinidin, fluoxetin, paroxetin, and propafenon because these drugs inhibit CYP2D6.
Carvedilol increases the concentration of digoxin about 20% when taken at the same time.
Storage
Leave a cool place, avoid light, temperature below 30⁰C.
