Indications

Ceclor drugs are indicated in the following cases:

Treatment of acute bronchitis and exacerbations of chronic bronchitis caused by s.pneumonia, H.influenzae (including β - lactamase strains), H.Parainfluenzae, M.Catarrhalis (including β - lactamase strains) and S.Aureus.

Treatment of sore throat and tonsillitis caused by s.pyogenes (streptococcal group A). (Penicillin is a drug often chosen for treatment and prevention of streptococcal infections, including rheumatism prophylaxis. In general, Ceclor effectively treats streptococcal infections in the pharyngeal area;

Treatment of pneumonia caused by s.pneumonia, H.influenzae (including β - lactamase strains) and M.Catatrhalis (including β - lactamase strains).

Treatment of sinusitis caused by s.pneumonia (only strains sensitive to penicillin), H.influenzae (including β - lactamase strains) and M.Catrhalis (including β - lactamase strains).

Treatment of inferior urinary tract infections including cystitis and asymptomatic urinary bacteria caused by E.coli, K.pneumoniae, P.Mirabilis and S.Saprophyticus.

Treatment of skin infections and skin structure caused by s.pyogenes (streptococcal group A), S.Aureus (including β - lactamase strains) and s.epidermidis (including β - lactamase strains).

Conduct microorganisms to identify pathogenic bacteria and Cefaclor sensitivity. It is possible to start treating immediately while waiting for these results, but once there is antibiotic results, it must be changed accordingly.

Pharmacokological

In In vitro, the bactericidal properties of Ceclor is caused by Cefaclor. In vitro tests show that cephalosporin has bactericidal effects due to inhibiting cell wall synthesis. Cefaclor is not affected by the enzyme β - lactamase, so cefaclor killes bacteria secreting β - lactamase resistant to penicillin and some cephalosporins. Ceclor has proven bactericidal effects both clinically and in vitro for the following bacteria:

Gram -positive bacteria

  • Staphylococcus aureus (including β - lactamase strains); Pneumoniae;
  • Streptococcus Pyogenes (Group A).

    Gram -negative bacteria

  • Haemophilus parainfluenzae;
  • Haemophilus influenzae (including β - lactamase strains); Coli;
  • Klebsiella Pneumoniae;

    Gram -negative bacteria

  • citrobacter diversus;
  • Neissiria gonorrhoeeae.
  • propionibacterium ances; Enterococcus, Enerobacter Sp, Proteus Indol positive and Serratia sp resistance to cefaclor.

    pharmacokinetics

    Ceclor is well absorbed through the digestive tract after drinking. Although Ceclor can take it at full or hungry, but taken at full medicine will be better absorbed. Take the medicine after 1 hour, the bioavailability of Ceclor is over 90% compared to Cefaclor (drinking at hunger), the average peak concentration in the serum of Ceclor (measuring both at full and hungry) is lower and reaches slower from 40 to 90 minutes. The shared H2 inhibitors do not limit the absorption of Ceclor. Aluminum antacids or aluminum hydroxide or hydroxide are taken for 1 hour after using Ceclor, does not affect the speed but reduces 17% of Ceclor's absorption level.

    After taking the dose of 375mg, 500mg and 750mg, the average peak concentration in serum is 4.8 and 11 μg/ml, respectively after 2.5 to 3 hours. There is no record of the accumulation of drugs when taking the dose 2 times daily.

    Average half -life in normal plasma is about 1 hour (from 0.6 - 0.9 hours) and does not depend on the dose. In the elderly (over 65 years old) with normal seral creatinine, the peak concentration of the drug in plasma may be higher and the area under the curve (AUC) may be affected by a mitigation of kidney function but not clinically significant. Therefore, it is not necessary to adjust the dose in the elderly with normal renal function. There is no evidence of ceclor metabolism.

    How to use

    Ceclor can drink when hungry or full. However, drinking at full medicine is better absorbed. The pill is not cut, crushed or chewed.

    Dosage

    Dosage recommended in sore throat, tonsillitis, skin infections and skin structures are 375mg, use 2 times for 24 hours.

    For lower urinary tract infections, the recommended dose is 375mg, use 2 times for 24 hours or 500mg, used once a day.

    Dosage recommended in bronchitis is 375mg or 500mg, use 2 times for 24 hours.

    For patients with pneumonia and sinusitis, the recommended dose is 750mg, use 2 times in 24 hours.

    In the treatment of infections caused by S.Pyogenes (streptococcal group A), it is recommended to use Ceclor for at least 10 days.

    Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose? The level of epigastric pain and diarrhea depends on the amount of drug used. If there are other symptoms, it may be due to the secondary reaction of a hidden disease, an allergic reaction or the effect of other poisoning.

    To treat overdose should consider the overdose of many drugs, interact between drugs, abnormal pharmacokinetics of the patient.

    Protects the respiratory tract of patients, respiratory support and infusion. Careful monitoring and maintenance levels allows the signs of survival, blood gases, serum electrolytes, etc. can reduce the absorption of drugs through the gastrointestinal tract by using activated carbon. In many cases, this measure is more effective than washing the intestine or causing vomiting. It is advisable to consider using activated carbon instead of gastric lavage or combining both methods. Using many consecutive doses of active carbon can increase the elimination of absorbed drugs. Need to protect the patient's airway when applying stomach lavage or activated carbon.

    Using strong diuretic, abdominal distances, clinical absorption or blood transfusion proved to be ineffective in the overdose of Cefaclor.

    What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

    When using Ceclor drugs, you may experience unwanted effects (ADR).

    Common, ADR> 1/100

  • Digestive system: diarrhea, vomiting, nausea and indigestion.
  • Skin: Serum -like reaction.
  • Instructions on how to handle ADR

    Notify the doctor with unwanted effects when using the drug.

    Warnings

    Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

    Ceclor medicine for people with a history of hypersensitivity to cefaclor and other cephalosporin groups.

    Precautions when using

    using long -term Ceclor can cause excessive development of non -sensitive bacterial strains. Need to monitor patients carefully, if superinfection, must have appropriate treatment.

    Patients using Ceclor may have false positive results for urinary tests when testing with Benedict, Fehling or Clinitest® solutions, but does not affect TES - Tape® method (Glucose Enzymatic Test Strip, USP)

    The ability to drive and operate machinery

    does not affect.

    Pregnancy

    Studies on fertility performed in rats, rats with 12 times the dose and in ferrets with 3 times the maximum dose for people who do not see evidence of infertility or harmful to the fetus due to Cefaclor. However, there are not enough strictly controlled studies in pregnant women. Because studies on animals do not always be able to speculate the response of humans, only this drug is used in pregnant women when they are really necessary.

    Breastfeeding period

    There has been no research on the use of ceclor on this object. A small amount of Cefaclor is found in breast milk after taking a dose of 500mg. The average concentration in milk is 0.18; 0.20; 0.21; 0.16 μg/ml corresponding to the time after 2, 3, 4 and 5 hours. After 1 hour the concentration in the milk is only in the form of traces. The effects of the drug on breastfed babies are not known. Be careful when using Ceclor in nursing mothers.

    Medicinal interaction

    The level of Ceclor absorption decreases if taking antacids containing aluminum hydroxide or Magnesi hydroxide within 1 hour after drinking Ceclor. The anti -H2 drugs do not change the speed and level of drug absorption. Like other β - lactam antibiotics, Probenecid inhibits cefaclor elimination (perhaps with ceclor).

    Storage

    Store at temperatures below 30 ° C.
