Uses

indications

Cefaclor 125 mg drug is indicated in the following cases:

  • Treatment of upper and upper respiratory infections below mild and medium levels caused by sensitive bacteria, especially after using common antibiotics (due to "recommended national anti -respiratory infections programs: acute otitis media, acute sinusitis, sore throat, recurrent recurrence of tonsillectomy, acute bronchitis, chronic bronchitis. For acute pharyngitis due to streptococcus beta blood -soluble group A, the first popular drug is penicillin v to prevent low heart disease.
  • Treatment of uncomplicated urinary tract infections due to sensitive bacterial strains (including nephritis - pyelonephritis and cystitis). Do not use for prostatitis because the drug is difficult to penetrate into this organization.
  • Treatment of skin infections and soft tissue due to staphylococcus aureus sensitive to methicillin and streptococcus pyogenes sensitive.

    Pharmacology

cefaclor is an oral, semi -synthetic cephalosporin antibiotic, 2nd generation, has the effect of killing bacteria that are developing and dividing by inhibiting synthesis of bacterial cells.

Cefaclor has a vitro printing effect on Gram-positive bacteria similar to cefalexin, but has a stronger effect on Gram-negative bacteria, especially with Haemophilus Influenzae and Moraxella Catarrhalis, even with H. Influenzae and M. CatVrhalis producing Beta-Lactamase.

On In vitro, Cefaclor works for the majority of the following bacteria: Staphylococcus, including strains of penicilinase, positive, negative coagulase, but there is diagonal manifestation between cefaclor and methicillin; Streptococcus pneumoniae; Streptococcus Pyogenes (Streptococcus Beta Group A); Propionibacterium acnes; Corynebacterium diphtheriae. Gram -negative aerobic bacteria: Moraxella catrhalis; Haemophilus Influenzae (including the strains producing beta-lactamase, ampicillin resistance); Escherichia coli; Proteus Mirabilis; Klebsiella spp; Citrobacter Diversus; Neisseria gonorrhoeae. Anaerobic bacteria: Bacteroides spp (except for bacteroides fragilis is resistant); peptococcus; peptostreptococcus.

    Dynamic pharmacokinetics

absorption: Cefaclor is well absorbed after drinking at hunger, at a dose of 250 mg and 500 mg of average peak concentration in plasma, about 7 and 13 micrograms/ml, achieved after 30 - 60 minutes. Food slowly absorbs, but the total amount of drugs is absorbed is unchanged, the peak concentration is only 50 - 75% of the peak concentration in the patient when hungry and usually appears later from 45 - 60 minutes. Cefaclor concentration in serum exceeds the minimum inhibitory concentration, for most sensitive bacteria, at least 4 hours after taking the dose of treatment.

Distribution: About 25% Cefaclor binds to plasma proteins.

Cefaclor is widely distributed throughout the body, passes through the placenta and excreted in low concentrations.

Metabolism: There is no significant biological change in the liver.

Elimination: Cefaclor's waste time in plasma is from 30-60 minutes, this time usually lasts a bit longer in people with reduced renal function. If the kidney function is completely lost, the sale time lasts from 2.3 to 2.8 hours.

Cefaclor quickly excreted through the kidneys, 85% of the doses were eliminated through urine in the form of unchanged within 8 hours, most of the excretion in the first 2 hours. Cefaclor reached high concentration in urine within 8 hours after drinking, over these 8 hours the peak concentration in the urine reached 600 and 900 micrograms/ml after the doses of use, respectively 250 and 500 mg. Probenecid slowed Cefaclor excretion. A little cefaclor is eliminated through blood decomposition.

    How to use

use it during or outside the meal.

Put the powder in 10 - 15 ml of water (about 2-3 tablespoons of coffee), stir and drink immediately.

    Dosage

    Adults and the elderly

The dose of 250 mg, every 8 hours. Severe cases may doubled the dose. Maximum 4 g/day.

  • Sore throat, bronchitis, tonsillitis, skin and soft tissue infections, uncomplicated urinary tract infections: 250 mg, every 8 hours. In case of more severe infections or less sensitive bacterial bacteria can be used 500 mg, every 8 hours. Treatment period is 10 days.
  • Cases of recurrent pharyngitis due to streptococcus beta blood -soluble group A, should also be treated for family members carrying asymptomatic pathogens.
  • Lower respiratory infections: 250 mg, every 8 hours. In case of more severe infections (pneumonia) or bacterial strains of isolated bacteria can use 500 mg, every 8 hours. Treatment period is 7 to 10 days.
  • Cefaclor can be used for patients with renal impairment. In case of severe renal failure, the dose adjustments for adults are as follows:

  • Creatinine clearance 10 - 50 ml/min, use 50% - 100% common doses.
  • Creatinine clearance below 10 ml/min, using 25% common doses.
  • For patients, they have to hemorrhage regularly, so the starting dose of 250 mg - 1 g should be used before the hemorrhage and maintain the treatment dose of 250 - 500 mg every 6 - 8 hours, between the separation.
  • Children

Use 20 mg/kg body weight/24 hours, divided into 3 drinks. May double the doses in severe cases. Maximum dose: 1 g/day.

Children 1 month - 1 year of age taking 62.5 mg, every 8 hours; Children from 1 - 5 years old take 125 mg, every 8 hours; Children over 5 years old take 250 mg, every 8 hours.

Safety and efficiency for children under 1 month of age has not been determined.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? Protect the respiratory tract for patients, support ventilation and infusion. Reduce the absorption of drugs by giving activated carbon many times. You can wash your stomach and add activated carbon or use activated carbon. The diuretic benefits, the abdominal duct ratio or dialysis have not been determined to be beneficial in the treatment of overdose.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

When using Cefaclor 125 mg, you may experience unwanted effects (ADR).

    Common, ADR> 1/100

  • Blood: Eosin hypernagus.
  • digestive: diarrhea.
  • Skin: measles skin.
  • Uncommon, 1/1000

  • Body: Test Coombs directly positive.
  • blood: increased lymphocytes, leukopenia, neutropenia. digestion: Nausea, vomiting. skin: itching, urticaria.

urinary - genital: Itchy genitals, vaginitis, Candida fungus.

Instructions on how to handle ADR

Stop using Cefaclor if allergies occur. Hypersensitivity symptoms may persist for a few months. In case of allergies or serious hypersensitivity reactions, it is necessary to conduct supportive treatment (maintain ventilation, oxygen breathing, use adrenalin, corticosteroid intravenous injection).

    Stop treatment if severe diarrhea.

Cases of fake colitis due to Clostridium difficile growing too mild, usually just stop the drug. Cases of moderate and severe cases, should be noted for transmission of fluids and electrolytes, protein supplements and antibiotic treatments that work with C. Difficile (should use metronidazol, do not use vancomycin). Serum -like reactions usually occur a few days after starting treatment and decreases after a few days of stopping the drug. Sometimes severe reactions need to be treated with antihistamines and corticosteroids.

If convulsions are caused by medication, it is necessary to stop the drug. Can be treated with anti -convulsions if necessary.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    contraindicated

Cefaclor 125mg contraindicated drug in the following cases: patients with a history of allergies to cefaclor and cephalosporin group.

    Be cautious when using

Patients with penicillin allergies because of cross -sensitive.

Cefaclor for a long time can cause fake colitis caused by Clostridium difficile. Caution for patients with a history of gastrointestinal tract, especially colitis. It is necessary to suspect fake colitis when prolonged diarrhea appears, bloody stools in patients are taking or within 2 months after stopping antibiotic therapy.

People with severe impaired renal function, because the sale time of Cefaclor's exhaust in patients with anuria is 2.3 - 2.8 hours (compared to 0.6 - 0.9 hours in normal people) so often do not need to adjust the dose for average kidney failure but to reduce the dose in people with severe kidney failure. Kidney function should be monitored during cefaclor treatment in combination with antibiotics with potential toxic kidneys or with furosemid, ethacrynic acid.

The ability to drive and operate machines

drugs do not affect the driver and operate machinery.

    Pregnancy

Cephalosporin antibiotics are often considered safe when used for pregnant women. However, because no project has been fully studied in pregnant women, Cefaclor is only designated in pregnant women when really necessary.

    The period of breastfeeding

Cefaclor concentration in breast milk is very low. The impact of the drug on breastfeeding is unknown but should pay attention when the child has diarrhea, speech and rash.

    Drug interaction

simultaneous use of cefaclor and warfarin rarely increases prothrombin time, causing bleeding or not clinically bleeding. Patients with vitamin K deficiency (diet, malabsorption syndrome) and patients with renal failure are high -risk subjects interactive. For these patients, regular prothrombin should be monitored and dose adjustment if necessary.

Probenecid increases the concentration of cefaclor in serum.

Cefaclor is simultaneously used with aminoglycosides or Furosemid diuretic, which increases the toxicity of the kidneys.

    Storage

In a dry place, temperatures below 30 ° C, avoid light.
