Cefimed 200mg drug is indicated for treatment of acute bacterial infections due to sensitive bacterial strains in the following cases:

  • Upper respiratory tract infection: otitis media , sore throat , sinusitis and tonsillitis.
  • Lower respiratory infections: acute bronchitis , acute attacks of chronic bronchitis, and pneumonia.

Urinary tract infection: gonorrhea cervical and uncomplicated urethra, cystitis, cystitis - urethritis, nephritis - pyelonephritis.

    Pharmacy

cefixim is the 3rd generation Cephalosporin antibiotic for oral. Cefixim is a Cephem ester with ethyl group at 3 'and the carboxyl group adds the iMinomethoxy group at the Acyl-7 position. Like other cephalosporin antibiotics, cefixim is an antibacterial agent through the mechanism of inhibiting the synthesis of bacterial cells by connecting one or more PBP (penicillin binding proteins). The antibacterial spectrum of Cefixim is related to Cefixim's cohesion affinity with the PBP of different bacteria.

When the cohesion with the PBP occurs, Cefixim has the effect of stabilizing the bacteria wall of the bacterial cell due to the synthesis of defect cell walls. Cephalosporin can also increase the destruction of bacterial cell walls due to reduced inhibition of available inhibitors for Murein Hydrolase enzymes. This enzyme when not inhibited has the effect of destroying the integrity of the cell wall.

    pharmacokinetics

cefixim absorb slowly through orally, the peak concentration reaches 2-6 hours after drinking. Oral bioavailability reaches 40-50%, the absorption of drugs is not affected by food. After using single or multiple dose, the serum concentration is 1 - 4mg/ml (at a dose of 200mg) or 1.9–7.7mg/ml (at a dose of 400mg).

The level of cohesion with plasma protein 50 - 65%. The distribution volume 0.6 - 1.1 kg. About 50% of the dose is excreted in the urine, 5% of the dose is excreted through the bile. The sale time is from 3-4 hours, lasting about 6.4 hours in average renal failure people and about 11.5 hours in people with severe kidney failure.

    How to use

The drug is used by oral. The drug should be taken in whole with water and can be taken before or after eating.

    Dosage

The normal treatment is 7 days, which can last up to 14 days in case of severe infections or infections due to sensitive bacteria.

Should take medicine at the same time every day to get the best response.

adults and children> 12 years old or children weighing over 50 kg

The recommended dose for most types of infections is from 200 - 400mg/day, depending on the severity of the infection. This dosage can be used single or divided into 2 equal doses.

    Cervical gonorrhea or uncomplicated urethra

Recommendation for a single dose of 400mg or 800mg/day.

    Elderly

recommend adult dose, except for the case of renal failure need to reduce the dose.

Children from 6 months to 12 years old

Recommended use of oral fluid. The usual dose for children 8mg/kg weight/day of single dose or divided into 2 equal doses.

    Children under 6 months old

The safety and effectiveness of Cefixim has not been determined.

    kidney failure

The following dosage is recommended for cefixim based on creatinine clearance:

  • Creatinin clearance> 20 ml/min: Using the usual dosage of adults, using the dose.
  • Creatinine clearance

Chronic hemolysis/chronic peritoneal refinement: The dose must not exceed 200mg/day, use single dose.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when using overdose?

Symptoms: Predict the symptoms similar to unwanted effects listed above.

Treatment has no specific antidote. Gastrointestinal lavage may be useful. The significant separation does not remove the cefixim significantly.

Should apply symptomatic treatment and support treatment.

What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

  • Side Effects

When using Cefimed 200mg, you may experience unwanted effects (ADR).

Most of the side effects of Cefixim are light and limited, rarely cases of drug use. The following side effects have been reported:

  • Central nervous system: Dizziness , headache .
  • skin: Hypersensitivity reactions such as joint pain, fever due to drugs, itching, rashes and . These reactions often decrease when stopping medication. digestion: usually diarrhea , nausea, vomiting, hair decomposition, flatulence, abdominal pain, indigestion and rarely fake colitis. In case of severe diarrhea, it is necessary to stop taking the drug.

Genital urinary: Itching genitals and vaginitis, including the report of Candida has been reported. Hematology: less likely leukemia, leukopenia and thrombocytopenia, these reactions are often recovered.

Liver: Increase and recover ALT, AST and Alkaline Phosphatase.

Kidney: Increase and recover nitrogen urea (bun) and creatinin.

Notify the doctor of unwanted effects when taking the drug.

Instructions on how to handle ADR:

Can reduce the side effects of the drug by reducing the dose.

When encountering side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Cefimed 200mg medicine is contraindicated in the following cases: Patients with hypersensitivity to cephalosporin antibiotics or any ingredients in the formula of the drug.

    Precautions when using

Caution indications CEFIXIM in patients with a history of allergies to penicillin. There are some evidence that there is a cross -allergy between cephalosporin and penicillin groups.

Similarly need to be cautious when indicating Cefixim in any patient who has previously been allergic to any drug, because these patients are more likely to occur allergies. There have been cases of severe hypersensitivity reactions in sensitive patients, including anaphylaxis. Cefixim should be stopped when the hypersensitivity reaction occurs. In case of anaphylactic shock, it is necessary to apply appropriate clinical therapy including pharmacological intervention.

Be cautious to use Cefixim for patients with severe renal failure.

Like other broad -spectrum antibiotics Cefixim can lead to over -developing non -sensitive bacterial strains. In this case, it is necessary to apply appropriate treatment and clinical therapy

Treatment with Cefixim can cause changes to the intestinal bacteria, which can cause excessive growth of clostridie species, patients with bleeding, massive, severe, likely to have false colitis due to the toxicity of clostridie difficile. It can occur while or after completing the treatment with antibiotics. It is necessary to fully and quickly evaluate this diarrhea, including Sigina colon, and bacterial transplant. Other causes of colitis should be excluded.

Supplementing fluid, electrolytes, and proteins are necessary. If colitis does not improve after stopping the drug or more severe symptoms, recommend the use of vancomycin oral.

There has been a report that changes in the results of liver function test, which should be monitored in liver function in patients with liver failure, especially when taking high doses and prolonged. Pready Prothrombin time when using cephalosporin has reported that it is necessary to closely monitor patients with anti -coagulants.

Do not use lifestyle vaccine for patients being treated with cefixim, only used for at least 24 hours after the last dose of cefixim, due to immune response to the vaccine may be reduced.

In theory, the same antibiotic aminoglycosides with cephalosporin increases the toxicity of the kidneys. Do not share these two drugs in patients with renal impairment, in case of need to be used in patients with no renal failure, the toxic level should be monitored on the kidneys.

Due to impaired intestinal bacteria system, bacteria system has a synthetic function vitamin K , patients with vitamin K deficiency when treated with cefixim. In patients with poor diet, alcoholic patients, or patients with poorly absorbent syndrome, they will have higher risk of vitamin K shortage.

Cefixim should be carefully indicated in patients with a history of colitis because these patients are at risk of fake colitis.

The ability to drive and operate machinery

cefixim does not affect the ability to drive and operate the machine. However, Cefixim can cause dizziness, recommend that patients should not drive or operate the machine until certainly not affected by the drug.

    Pregnancy

There are no adequate studies and good control in humans. Animal studies do not show harmful effects, but these research results are not always the results used for predictions. Cefixim should not be used in pregnant women unless it is necessary to clinically and after careful consideration of the dangers that may occur for the fetus compared to the treatment benefits for the mother

    Breastfeeding period

There is no data showing Cefixim excreted through breast milk. Therefore, breastfeeding should be stopped during treatment with cefixim.

    Drug interaction

    with other drugs

aminoglycosides: In theory the use of aminoglycosides and cephalosporins may increase kidney toxicity. There has been a report on this toxicity, but so far the clinical importance is still unknown. It is recommended to avoid combining these two drugs in patients with renal impairment, in patients with normal renal function if the common use is necessary, it is necessary to monitor the toxicity on the kidney.

Antibiotics: Prothrombin time may be prolonged. Prothrombin time tracking recommendations when shared and adjusted the dose of anticoagulants if necessary.

Live typhoid vaccine: Cefixim has antibacterial effect Salmonella Typhi, thus reducing immune response for vaccines. The appointment of typhoid vaccine so the dose of cefixim is at least 24 hours in the end

    Testing

Experiments Coomb's: Like other cephalosporin antibiotics that react CoAMB's directly positive.

Glucose test: Cefixim may cause a fake positive reaction to glucose when using copper reduction method (Fehlings solution, Benedicts solution).

It is recommended to use glucose oxidation catalysts when testing for urinary glucose in patients being treated with Cefixim.

Ceton test: Cefixim can cause false positive reactions when testing ceton urinary trial by testing nitroprussid. This sand is found that the report occurs with the trial method of nitroferricyanid.

    Storage

Leave a cool place, avoid light, temperature below 30⁰C.

To be out of reach of children.
