Uses

indications

Cefixim 100 drugs are indicated in the following cases:

Uncontrolled urinary tract infections due to sensitive strains of E.coli or Proteus Mirabilis and some cases of non -complicated urinary tract infections due to other Gram -negative bacilli such as Citrobacter spp., Entobacter spp., Klebsiella spp.

Some cases of nephritis - pyelonephritis and urinary tract infections have complications caused by sensitive enerobacteriaceae, but the treatment results are worse than in case of non -complicated urinary tract infections.

Middle otitis caused by Haemophilus Influenzae (including beta - lactamase strains), Moraxella Catatrhalis (including beta - lactamase strains), Streptococcus pyogenes.

Sore throat and tonsils due to streptococcus pyogenes.

Acute and chronic bronchitis caused by Streptococcus Pneumonia, or Haemophilus Influenzae or Moraxella Catarrhalis.

Mild and medium -sized pneumonia.

Gonorrhea has no complications caused by Neisseria Gonorrhoeae (including beta lactamase strains); typhoid disease caused by salmonella typhi (including multi -resistant strains); Sensitive shigella dysentery (including ampicillin strains).

Pharmacology

cefixim 100 is a 3rd generation cephalosporin antibiotic. The drug has a bactericidal effect. Cefixim's bactericidal mechanism is attached to target proteins (penicillin -mounted proteins) that inhibit mucopeptide synthesis in bacterial cell walls.Bacterial resistance mechanism of bacteria is to reduce Cefixim's affinity for target protein or reduce the permeability of bacterial cell membranes for drugs.

Cefixim has high sustainability with the hydrolysis of beta-lactamase encoded by the gene located on plasmid and chromosome. Sustainability with beta-lactamase of Cefixim is higher than Cefaclor, Cefoxitin, Cefuroxim, Cephalexin, Cephadin.

cefixim works both in vitro and clinically with most of the strains of the following bacteria:

Gram - positive microorganisms: Streptococcus pneumonia, Streptococcus pyogenes.

Gram-negative microorganisms: Haemophilus influenzae (secretive or no beta-lactamase secretion), Moraxella Catatrhalis (most beta-lactamase secretion), Escherichia coli, Proteus Mirabilis, Neisseria Gonorrhoeae (secretive or penicilinase secretion).

Cefixim also has the effect of printing vitro with the majority of the strains of the following bacteria, but the clinical effect has not been verified:

Gram -positive microorganisms: Streptococcus Agalactiae.

Gram - negative microorganisms: Haemophilus parainfluenzae (secretive or no beta -lactamase secretion), Proteus vulgaris, Klebsiella Pneumoniae. Klebsiella Oxytoca, Pasteurella Multocida, Providencia spp .. Salmonella spp., Shigella spp.

Dynamic pharmacokinetics

absorption

After taking 1 single dose of Cefixim, only 30-50% of the dose is absorbed through the gastrointestinal tract, regardless of or after or after meals, although the absorption rate can be reduced when taken with meals.

The drug in the form of oral fluid is absorbed better than the tablet form. The absorption of drugs is relatively slow.

The peak concentration in plasma is 2 micrograms/ml (for 200 mg dose); 3.7 micrograms/ml (for a dose of 400 mg) and reached after taken 2 - 6 hours.

Half lifetime in plasma is usually about 3 to 4 hours and may last for kidney failure. About 65% Cefixim in the blood is attached to plasma proteins.

distribution

Information about the distribution of Cefixim in the body's tissue and fluid is limited. Currently there is no adequate data on the concentration of cerebrospinal fluid. The drug can pass the placenta. The drug can reach relatively high concentrations in bile and urine.

metabolism and elimination

About 20% of oral doses are eliminated in the form of unchanged urine within 24 hours. Up to 60% of oral doses are not passed through the kidneys. There is no evidence of chemicals but may be part of the drug eliminated from the bile into the feces.

Before taking Cefixim 100 USP oral powder for infection treatment (10 packs x 1.5g)

How to use

Mix the powder into a sufficient amount of water. Do not use milk, tea, coffee or carbonated, alcoholic or calcium drinks to make medicine.

Dosage

adults

Common doses used in adults 200 mg/time, 2 times a day, 12 hours apart.

For uncomplicated gonorrhea treatment caused by Neisseria Gonorrhoeae (including beta-lactamase strains): Taking a dose of 400 mg (single-time, more combined with an effective antibiotic for Chlamydia is likely to be infected). Higher doses (1 time 800 mg) have also been used to treat gonorrhea.

Children

Children> 12 years old or weight> 50 kg dose like adults.

Children> 6 months - 12 years old 8 mg/kg/day can be used once a day or divided 2 times, each time 12 hours apart.

Inadequately verified the safety and effectiveness of the drug for children under 6 months old.

Dosage for patients with renal failure:

No dose adjustment for patients with creatinine clearance> 60 ml/ min.

For patients with creatinine clearance

Adults have creatinin clearance 21 - 60 ml/ minute cefixim doses 300 mg/ day.

Because Cefixim does not lose through hemolysis, patients with artificial dialysis and peritoneal filtering do not need to add Cefixim dose.

For the elderly: In general, there is no need to adjust the dose, unless there is impaired renal function (creatinine clearance

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

What to do when overdose? Because there is no specific treatment, it is mainly symptomatic treatment. When symptoms of overdose, stop immediately and handle as follows: Gastric lavage, anti -convulsions can be used if clinical indications.

Because the drug is not equal to hemolysis, no dialysis or peritoneal filter.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

contraindicated

cefixim 100mg drug contraindicated in the following cases:

Patients with a history of hypersensitivity to cefixim or other antibiotics of cephalosporin groups, who have a history of anaphylaxis due to penicillin.

Be cautious when using

Before starting the treatment of Cefixim 100, the patient's allergic history should be carefully investigated in penicillin and other cephalosporins due to the hypersensitivity between Beta-lactam antibiotics, including: penicillin, cephalosporin and cephamycin.

Be cautious when using Cefixim 100 in people with a history of gastrointestinal diseases and colitis, especially when used for prolonged use, because there may be excessive risk of developing anti -drug bacteria, especially Clostridium difficile in the intestine as heavy diarrhea, need to stop the drug and treat with other antibiotics (Metronidazol, Vancomycin ...).

In addition, diarrhea in the first 1-2 days is mainly due to the drug, if mild does not need to stop the drug. Cefixim 100 also changes the bacteria in the intestine.

The ability to drive and operate machinery

Because the unwanted effect of the drug can cause headaches (3-16%), dizziness, insomnia, fatigue, should be cautious when used for drivers and machinery operation.

Pregnancy

for this time, there is no complete data on the use of Cefixim in pregnant women, during labor and birth, so only use Cefixim for these cases when really necessary.

The period of breastfeeding

Currently, it has not yet been confirmed that CEFIXIM is distributed into milk in breastfeeding women. Therefore, Cefixim should be used for women to breastfeed carefully, can suspend breastfeeding during medication.

Drug interaction

Probenecid increases the peak and AUC concentration of Cefixim, reducing the clearance of the kidneys and the distribution of the drug.

anticoagulants like warfarin when used with cefixim increase prothrombin time, with or without bleeding.

Carbamazepin taken with cefixim increases carbamazepine levels in plasma.

nifedipin when taken with Cefixim increases the bioavailability of Cefixim manifest by increasing the peak concentration and AUC

Storage

Store drugs in a dry place, temperatures below 30 ° C, avoiding light.
