Pharmacology
cefixim is Cephalosporin antibiotic semi -combined 3rd generation orally, with wide bactericidal activity, acting with many strains of bacteria that cause Gram (+), Gram ( -), sensitive to most bacteria capable of enzyme β - lactamase, so Cefixim sensitive to other anti -bacteria Group drug β - lactam.
Cefixim binds to target proteins (penicillin -mounted proteins) that inhibit mucopeptide synthesis in bacterial cell walls. The bacterial Cefixim resistance mechanism is to reduce Cefixim's affinity for target protein or reduce the permeability of bacterial cell membranes for drugs.
Cefixim has high sustainability with the hydrolysis of beta - lactamase encoded by the gene located on plasmid and chromosome. Sustainability with beta - lactamase of Cefixim is higher than Cefaclor, Cefoxitin, Cefuroxim, Cephalexin.
cefixim works both Invitro and clinically with most of the following bacteria strains:
Gram - positive bacteria: Streptococcus pneumonia, Streptococcus pyogenes.
Gram-negative bacteria: Haemophilus influenzae, (secretive or no beta-lactamase secretion), Moraxella Catatrhalis (most beta-lactamase secretion), Proteus Mirabilis, Neisseria Gonorrhoeae (secretion or no penicilinase secretion).
cefixim also has the effect of Invitro, most of the following bacteria strains, but the clinical effect has not been verified:
Gram - positive bacteria: Streptococcus agalactiae.
Gram - negative bacteria: Haemophilus parainfluenzae, (secretive or no beta -lactamase secretion), Proteus vulgaris, Klebsiella Pneumoniae, Klebsiella Oxytoca, Pasteurella Multocida, Providencia spp, Salmonella SPP, Shigella SPP, Citrobacterio Citrobacter Diversus, Serratia Marcescens.
Cefixim has no activity for Enterococcus, Staphylococcus, Pseudomonas Aeruginosa and most bacterioides Clostridia.
Dynamic pharmacokinetics
After taking a single dose of Cefixim, only 30-50% of the dose is absorbed through the gastrointestinal tract, regardless of or taken before or after meals, although the absorption rate can be reduced when taken with meals.
The absorption of drugs is relatively slow. The peak concentration of plasma is 2 micrograms/ml (for 200mg dose), 3.7 micrograms/ml (for 400mg dose) and reaching after drinking 2-6 hours. Half -life in plasma is usually about 3 to 4 hours and may last for kidney failure. About 65% Cefixim in the blood is attached to plasma proteins.
The drug can pass the placenta. The drug may reach relatively high concentrations in bile and urine. About 20% of oral doses are eliminated in the form of unchanged urine within 24 hours. Up to 60% of oral dose without kidney.

How to use
This drug is only used by a doctor.
Normal dose: Drink away from meals.
Dosage
Children over 6 months to 12 years:
Take 8mg/kg/day, 1-2 times.
Dosage for patients with renal failure depending on the clearance of creatinine:
Over 60ml/minute: No dose adjustment.
From 21 - 60ml/minute: 300mg/day
less than 20ml/minute: 200mg/day.
The usual treatment time is from 5 - 14 days.

Side Effects
Common

Less:
severe diarrhea, fake colitis, anaphylaxis, angioedema, diverse roses, thrombocytopenia, leukocytes, hepatitis and jaundice, acute kidney failure.
Rare:
Prothrombin time, convulsions.

Warnings
Contraindicated
Patients with a history of hypersensitivity to cefixim or other antibiotics of Cephalosporin group, who has a history of anaphylaxis due to penicillin.
Be cautious when used
Caution for patients with a history of allergies to penicillin and other cephalosporins.
People with a history of gastrointestinal diseases and colitis.
Patients with kidney failure, children under 6 months old.
The ability to drive and operate machinery
The drug does not cause drowsiness, so it can be used for drivers and operating machinery.
Pregnancy
There is no complete data on the use of Cefixim in pregnant women, so only use Cefixim for pregnant women when really necessary.

Probenecid increases the peak and AUC concentration of Cefixim, reducing the clearance of the kidneys and the distribution of the drug.
Antagulants like warfarin when used with cefixim, increasing prothrombin time, with or without bleeding.
Carbamazepin taken with cefixim increases carbamazepine levels in plasma.
Nifedipin when taken with Cefixim increases the bioavailability of Cefixim manifest by increasing the peak and AUC concentration.
