otitis media, recurrent sinusitis, tonsillitis, recurrent pharyngitis, an outbreak of acute bronchitis with superinfection and infected pneumonia in the community.

Treatment of gonorrhea without complications and treatment of Lyme in the beginning during the erythema symptom caused by Borrelia Burgdorferi.

Pharmacology

cefuroxim is an oral, semi -synthetic cephalosporin antibiotic, 2nd generation. Cefuroxim has the effect of killing bacteria in the stage of growth and division by inhibiting synthesis

Like other 2nd generation cephalosporin antibiotics, cefuroxim has in vitro active activity in gram -negative bacteria better than the first -generation cephalosporin antibiotics, but the spectrum on the IAI gram -negative bacteria is narrower than the 3rd generation cephalosporin antibiotics. Cefamandol, therefore, has a better effect on bacterial strains that produce beta lactamase such as Haemophyllus Inffuenzae, Neisseria, Escherichia Coli, Entobacter, Klebsiella.

antibacterial spectrum:

Cefuroxim has an anti -Gram -positive and anaerobic resistance activity, including most staphylococcus strains of penicilinase, and gram -negative bacterial resistance. Cefuroxim has a high activity, so the minimum inhibitory concentration (mic) is low for strepfococcus strains (group A, B, 0 and G), gonococcus and meningoccus strains.

Initially, Cefuroxim also had low mic for gonococcus strains, Moraxella Catatrhalis, Haemophilus Influenzae and Klebsiella spp. Beta-lactamase secretion. But now, in Vietnam, many bacteria have been resistant to Cefuroxim, so the mic of the drug for these strains has changed. En

Clostridium difficile strains, Pseudomonas spp., Campylobacter spp., Acinetobacter calecoaceticus, Legionella spp. All are not sensitive to Cefuroxim. Staphylococcus aureus strains, Staphylococcus epidermidis resistant methicilin are resistant to cefuroxim. Listeria monocytogenes and most of the Enterococcus strains are also resistant to cefuroxim.

pharmacokinetics

absorption:

After drinking, Cefuroxim acetyl is absorbed through the digestive tract and is quickly hydrolyzed in the intestinal mucosa and in the blood to release Cefuroxim into the complete system. The drug is best absorbed when taken at meals.

Distribution:

About 33% to 50% of the amount of cefuroxim in the blood seams with plasma proteins. Cefuroxim is widely distributed to organizations and fluids in the body. The apparent distribution volume in healthy adults ranges from 9.3 - 15.8 liters/1.73 m2. A small amount of Cefuroxim can pass through the brain blood barrier in case the meninges are not inflamed. However, Cefuroxim only achieves the concentration of treatment in cerebrospinal fluid when intravenously in case of brain inflammation. The drug through the placenta and excreted through breast milk.

Metabolism:

Cefuroxim is not metabolized and is discharged in the form that does not change both according to the glomerular filtration mechanism and excreted in the renal tubules.

Era:

Half of the lifetime elimination of the drug in plasma is about 1-2 hours. The drug is high in urine. Cefuroxim only excreted in honey with a very small amount.

    Be cautious when using

Caution for patients with a history of allergies with the beta-lactam group.

Be careful when giving patients simultaneously with strong diuretics, because it can have adverse effects on kidney function.

Should reduce the dose of cefuroxim injected in temporary or chronic kidney failure.

Long -term Cefuroxim can make unnecessary strains overexplicable. Need to follow the patient carefully. If the superinfection is serious during treatment, the drug must be stopped.

Should be very careful when prescribing broad -spectrum antibiotics for people with gastrointestinal diseases, especially colitis.

Therefore, only use drugs for women who are breastfeeding after carefully consider the risk and benefits and must monitor children in the process of treating antibiotics for mothers.

    Medicinal interaction

Probenecid is high doses, reducing the retention of Cefuroxim in the kidney, making the cefuroxim concentration higher and longer.

Cefuroxim simultaneously used with aminoglycosides or strong diuretics (such as Furosemid) may increase the toxicity of the kidneys.

Cefuroxim acetyl canal can affect the bacteria in the intestine, reducing estrogen reabsorption, thus reducing the effects of oral contraceptives containing estrogen and progesterone.

Ranitidin with sodium bicarbonate reduces the bioavailability of cefuroxim acetyl.

Should use Cefuroxim acetyl at least 2 hours after antacids or H2 blockers, as these drugs can increase the gastric pH.
