Division of drugs: Corticosteroid anti -inflammatory drugs
tacrolimus is a macrolid immunosuppressant that is extracted from streptomyces tsukubaensis that has the same immunosuppressive effect like cyclosporin but not related to the structure. The mechanism of immunosuppressive effects of Tacrolimus is not well known.
tacrolimus has the effect of inhibiting T cells T, through inhibition of Interleukin-2 production. Tacrolimus inhibits 100 times the immunosuppressor of the same dosage.
It has shown that Tacrolimus inhibits activating T lymphocytes by cohesion to an intracellular protein, FKBP-12. Tacrolimus-FKBP-12 complex, calcium calmodulin and calcinerurin ions are formed and inhibited phosphatase activity of calcinerurin and therefore has the ability to prevent phosphorylation and transfer of T-cells factor (NF-AT) to be activated, an ingredient of cell humanity through processing.
Tacrolimus also inhibits the transcription of genes with symbols IL-3, IL-4, IL-5, GM-CSF and TNF-A, all of these genes are involved in the activity process at the initial stage T.
It is also proved that Tacrolimus has the ability to inhibit the release of intermediate substances that have been formed from skin moisturizers, alkaline leukocytes and reduce the manifestations of high -pitched receptors on Langerhans cells.
tacrolimus is used to prevent transplantation of organization in liver, kidney, and heart transplant. The drug is also used to treat Crohn disease with holes and on -site treatment of allergies.
In patients with eczema, the recovery of skin damage while treating with Tacrolimus ointment is associated with a decrease in the cohesion of FC receptors on langerhans cells and reducing their stimulating activity to Tacrolimus ointments does not affect collagen synthesis in humans.
Dynamic pharmacokinetics
Clinical data shows that the concentration of Tacrolimus during the body circulation after use is low and when measured, it only feels transient. Data from healthy people show that there is no or very little exposure to the body with Tacrolimus when taking Tacrolimus ointment on the spot alone or repeating. Most organic eczema patients are treated with solitary or repeated Tacrolimus ointment (0.03 - 0.3%) with blood concentration in blood Due to the low exposure of the body with Tacrolimus ointment, the high cohesion of Tacrolimus (> 98.8%) with plasma proteins is considered as clinically unrelated
Tacrolimus metabolism in human skin has not been found. Tacrolimus in the body is completely metabolized in the liver via CYP3A4.
With the topical ointment, the average half -life of Tacrolimus is estimated at 75 hours for adults and 65 hours for children.
