Food slows down and may reduce the absorption of the drug. Oral bioavailability of cimetidine is about 60-70%.

cimetidine is widely distributed in the body. The half -life is about 2 hours and lasts longer in people with renal failure. Cimetidine is partially metabolized in the liver into hydroxymethylcimetidine and sulfoxid, about 50% of oral doses are excreted through urine in the formless form after 24 hours.

    Medicinal interaction

cimetidine and other anti -H2 drugs reduce the absorption of drugs that their absorption depends on the stomach pH such as ketoconazol, otraconazole. These drugs must be taken at least 2 hours before taking cimetidin.

cimetidine may interact with many drugs, but only some clinical significance, especially with drugs with narrow therapeutic values, are at risk of toxicity, need to adjust the dose. Most of the interactions are due to the attachment of cimetidine with the isenzyme of Cytochrom P450 in the liver, especially in CYP1A2, CYP2C9, CYP2D6 and CYP3A4, leading to inhibition of oxidation in the liver microsom and increase bioavailability or plasma concentration of drugs metabolized by these enzymes.

A few other interactions are due to the competition with the excretion in the renal tubules. The mechanism of interaction such as changing blood flow through the liver only plays a secondary role.

    Meaningful interactions occur with:

  • anti -epileptic drugs such as phenytoin, carbamazepin, valproic acid.
  • Cancer treatments such as alkylation, anti -chemical drugs.

  • Benzodiazepin derivative.

  • zalcitabin, zolmitriptan.
  • to avoid coordination of cimetidine with these drugs or use caution, monitor the effects or concentrations of drugs in plasma and reduce the appropriate dose. Antacids can reduce cimetidine absorption if taken with, should drink an hour apart.
  • When using cimetidin, you need to be cautious if drinking.
