Indications

0.3% Ciprofloxacin is indicated in the following cases:

  • Treatment of eye infections due to sensitive strains: keratitis, conjunctivitis.
  • Pharmacology

ciprofloxacin is a semi -synthetic antibiotic, wide antibacterial spectrum, belonging to the Fluoroquinolon group. The drug has the effect of killing bacteria due to inhibiting DNA Gyrase and Topoisomerase IV (are essential enzymes for the reproduction of DNA of bacteria), so the copy of Chromosom makes bacteria not reproduce quickly.

Ciprofloxacin has a stronger printing effect than nalidixic acid (non -fluorized quinolone). Ciprofloxacin also works with bacteria that have resisted antibiotics from other groups (aminoglycosides, cephalosporin, tetracyclin, penicilin ...).

Unlike the Beta Lactam antibiotics that have the strongest effect on sensitive bacteria in the fast growing stage (Log), Ciprofloxacin bactericidal at both fast and slow growing stage.

For most sensitive bacteria, the minimum concentration of bactericidal (MBC) of ciprofloxacin is 1-4 times higher than the mic; MBC is sometimes 8 times higher than the mic. At concentrations achieved in treatment, quinolones do not affect Topoisomerase Typ II in mammals, this enzyme is similar to bacteria, but different from the bacterial Gyrase DNA and does not cause supercoiling (Supercoiling).

Although clinical significance has not been determined, Ciprofloxacin has the effect of inhibiting post -antibiotics and has the effect of immune function. The effect of the drug is reduced in the acidic environment, in the urine but does not decrease in serum.

    pharmacokinetics

ciprofloxacin absorbs quickly and easily in the digestive tract. When food and anti -acid drugs, the absorption of drugs is slowed but not significantly affected. After drinking, the maximum concentration of Ciprofloxacin in the blood appears after 1-2 hours with bioavailability of 70-80%.

Ciprofloxacin is widely distributed throughout the body and penetrates well into tissues, with high concentrations in places where infected areas (body fluids, tissues). The concentration in tissue is usually higher than the serum concentration, especially in parenchyma, muscle, bile and prostate. The concentration in lymph and extracellular fluid is also close to the serum concentration. The concentration of drugs in saliva, nose, sputum, abdominal fluid, skin, cartilage and bone is lower, but still at an appropriate level.

If the meninges are normal, the concentration of the medication in the cerebrospinal fluid is only 10% of plasma concentrations. But when the meninges are inflamed, the drug is more absorbed. Ciprofloxacin passes through the placenta and excreted through breast milk. In honey also has high levels of drugs.

Metabolic substances: At least 4 metabolites have been defined. Oxociprofloxaxin appears mainly in urine and sulfociprofloxacin is the main metabolite in the feces. Ciprofloxacin is excreted mainly through the urine by filtering in the glomerular and excretion in the renal tubules.

Other elimination lines account for about 1/3 are metabolic in the liver, the exit through bile and excreted through the mucosa into the intestine (this is the mechanism of clearing the compensation in patients with severe renal impairment).
