    Pharmacy

Amoxicillin is a semi-synthetic antibiotic belonging to the β-lactam group with wide bactericidal spectrum for many gram-positive and gram-negative bacteria due to the synthetic inhibition of bacterial cells. Amoxicillin is easily destroyed by β-lactamase, so it does not work for bacterial strains producing these enzymes (multiple enzenta strains and haemophilus influenzae).

Clavulanic acid produced by the fermentation of Streptomyces Clavuligerus, with β-lactam structure similar to penicillin nucleus, capable of inhibiting β-lactamase due to the majority of Gram-negative bacteria and staphylococcus. Clavulanic acid helps amoxicillin not be destroyed by β-lactamase, and expanding the antibacterial spectrum of amoxicillin effectively to many common bacteria that have resisted amoxicillin, resistant penicillin and other cephalosporins. Amoxicillin and clavulanic acid are bactericidal.

The antibacterial spectrum of the drug includes:

    Gram -positive bacteria:

  • Aerobic type: Streptococcus Faecalis, Streptococcus Pneumoniae, Streptococcus Pyogenes, Streptococcus Viridans, Staphylococcus aureus, Corynebacterium, Bacillus anthracis, Listeria monocytogenes.
  • Anaerobic type: Clostridium, peptococcus, peptostreptococcus.
  • Gram -negative bacteria:

  • Aerobic type: Haemophilus influenzae, Moraxella Catatrhalis, Escherichia Coli, Proteus Mirabilis, Proteus vulgaris, Klebsiella, Salmonella, Shigella, Bordetella, Neisseria Gonorrhoeaee, Neisseria Vibrio Cholerae, Pasteurella Multocida.
  • Anaerobic type: Baceroides including B. Fragilis.

    Dynamic pharmacology

Amoxicillin and Clavulanic acid are easily absorbed by oral. The concentration of these 2 substances in the serum is maximum after 1-2 hours of taking the medicine. The absorption of the drug is not affected by food and it is best to drink right before meals.

Amoxicillin's oral bioactivity is 90% and of Clavulanic acid is 75%. Amoxicillin waste time in serum is 1-2 hours and of clavulanic acid is about 1 hour. About 55 - 70% amoxicillin and 30-40% Clavulanic acid are discharged through urine in the form of activity.

Symptoms of overdose: abdominal pain, vomiting, diarrhea. A small number of patients with rash, excitement or drowsiness.

    Management:

  • Stop taking medicine immediately. Symptomatic treatment and support if needed.

    Side Effects

When using Claminat 625mg, you may experience unwanted effects (ADR).

  • Common: diarrhea, itching, nausea, vomiting.
  • Uncommon: Non -blood leukemia, rash, hepatitis and jaundice, transaminase increases. Rare: Anaphylaxis, leukopenia, hemolytic anemia, fake colitis, Stevens-Johnson syndrome, interstitial nephritis.

    How to handle ADR

If allergic reactions are available, amoxicillin must be stopped and immediately emergency with adrenalin, oxygen breathing, intravenous corticosteroids and ventilation, including endotracheal intubation and never being treated with penicillin or cephalosporin again.

    fake colitis:

  • Severe (possibilities due to Clostridium difficile): Water and electrolyte, using antibiotic antibiotics against Clostridium (metronidazol, vancomycin).
  • Warnings

Before using Claminat 625mg, you need to read the instructions carefully and refer to the information below.

    Contraindicated

Claminat 625mg contraindicated drug in the following cases:

  • Hypersensitivity to the ingredients of the drug or antibiotics group penicillin, cephalosporin.
  • History of jaundice. liver failure.

    Precautions when using

Signs and symptoms of cholestasis jaundice are rarely occurring when taking the drug but may be severe. However, these symptoms are often recovered and will end after 6 weeks of stopping treatment.

Severe hypersensitivity reactions can sometimes lead to death (anaphylaxis) that have been reported in patients using penicillin antibiotics.

There has been an erythema (diverse) rash that comes with lymphadenopathy (mononoconemosis of infection) in patients using amoxicillin. Should avoid using drugs if suspected of monomeric hyperlemen.

Taking drugs for prolonged drugs sometimes develops resistant bacteria.

The ability to drive and operate machinery

The drug does not affect the ability to drive and operate machinery.

    Pregnancy and nursing mothers

Use drugs for pregnant women: Experimental research on the reproductive process of animals (mice) proved when using non -deformed oral preparations.

However, because there is little experience in using preparations for pregnant women, it is necessary to avoid using drugs in pregnant women, especially in the first 3 months of pregnancy, except for the necessary case prescribed by a physician.

Women who are breastfeeding: During breastfeeding can use preparations. A small amount of drugs can be diffused into breast milk, causing hypersensitivity to children. Should consider when using this object. Notify the doctor if the patient is breastfeeding.

    Drug interaction

The drug can cause prolonged bleeding and blood clotting time.

The drug can reduce the effectiveness of oral contraceptives, so warns patients to know this.

Probenecid reduces amoxicillin elimination but does not affect the elimination of clavulanic acid.

    nitedipine increases amoxicillin absorption.

Patients with hyperuricemia when using allopurinol along with amoxicillin will increase the ability of amoxicillin. Bacterial pliers such as: Fusidic acid, chloramphenicol, tetracylin can reduce the bactericidal effect of amoxicillin.

Amoxicillin reduces methotrexate secretion, increases toxicity on the gastrointestinal tract and hematopoietic system.

    Storage

Leave a cool place, avoid light, temperature below 30⁰C.

To be out of reach of children.
