Pharmacokology

Amoxicillin is a semi-synthetic antibiotic belonging to the Beta-Lactam group with wide bactericidal spectrum for many gram-positive and gram-negative bacteria due to the synthetic inhibition of bacterial cell walls. Amoxicillin is easily destroyed by beta-lactamase, so it does not work for bacterial strains producing enzymes bouncing (multiple enters strains of Entobacteriacede and Haemophilus influenzae).

Clavulanic acid produced by the fermentation of Streptomyces Clavuligerus, which has a beta-lactam structure similar to penicillin nucleus, has the ability to inhibit beta-lactamase due to the majority of gram-negative and staphylococcus bacteria.

Clavulanic acid helps amoxicillin not be destroyed by beta-lactamase, while expanding the antibacterial spectrum of amoxicillin effectively to many common bacteria that have resisted amoxicillin, resistant penicillins and other cephalosporins. Amoxicillin and clavulanic acid are bactericidal.

The bactericidal spectrum of claminat 250 includes:

Gram -positive bacteria

  • Aerobic type: Streptococcus Faecalis, Streptococcus Pneumoniae, Streptococcus Pyogenes, Streptococcus Viridans, Staphylococcus aureus, Corynebacterium, Bacillus anthracis, Listeria monocytogenes. Clostridium, peptococcus, peptostreptococcus.
  • Gram -negative bacteria

  • Aerobic type: Haemophilus Influenzae, Moraxella Catnrhalis, Escherichia Coli, Proteus Mirabilis, Proteus vulgaris, Klebsiella, Salmonella, Shigella, Bordetella, Neisseria Gonorrhoeae, Neisseria Meningitides, Neisseria Cholerae, Pasteurella Multocida. The concentration of these 2 substances in the serum is maximum after 1-2 hours of taking the drug. The absorption of the drug is not affected by food and it is best to drink right before meals. Amoxicillin's oral bioactivity is 90% and of Clavulanic acid is 75%. Amoxicillin waste time in serum is 1-2 hours and of clavulanic acid is about 1 hour. About 55 - 70% amoxicillin and 30-40% clavulanic acid are discharged through urine in the form of activity.

    How to use

Mix claminat powder 250 in a little water before drinking.

Should take medicine at the beginning of eating to minimize the phenomenon of non -tolerated drugs in the stomach - intestines.

Do not use Claminat 250 more than 14 days without checking the treatment.

    Dosage

Dosage is indicated according to amoxicillin content in combined drugs.

Adults and children over 40kg (for all indications):

    CLAMINAT 250 dose: 1000mg Amoxicillin/125mg Clavulanic acid x 3 times/day.

Lower doses can be used (especially in mild sinusitis skin infections): 1000mg of amoxicillin/125mg clavulanic acid x 2 times/day.

    Children under 40kg:

    Recommended dose: From 40mg amoxicillin/5mg Clavulanic acid/kg weight/day to 80mg amoxicillin/10mg clavulanic acid/kg weight/day (no more than 3000mg amoxicillin/375mg clavulanic acid/day) depending on the level of infection, divided into 3 times/day.

Treatment of otitis media, sinusitis, lower respiratory infections and severe bacterial infections, usual doses: 40mg amoxicillin/kg weight/day, divided into 08 hours apart, for 05 days.

Elderly patients: No need to adjust the dose.

For patients with kidney failure and liver failure:

Patients with kidney problems need to notify the doctor for consideration or continued medication or change to another drug.

Patients with liver problems need to be cautious when used and should check the liver function periodically.

    What to do when overdose?

Claminat 250 drugs are less likely to cause complications, because they are well tolerated even at high doses. The acute reactions occur depending on the exaggeration of each individual. The risk is definitely hyperkalemia when using a very high doses because clavulanic acid is used in the form of potassium salt.

    Symptoms of overdose:

    Abdominal pain, vomiting, diarrhea .

A small number of patients with rash, excitement or drowsiness.

    Management:

Stop taking medicine immediately. Symptomatic treatment and support if needed.

If the overdose occurs early and without contraindications, it may cause vomiting or gastric lavage.

Need to provide enough water and electrolytes to the body to maintain the urinary and reduce the risk of crystal pee.

Kidney damage can recover after the drug is stopped. Hypertension can occur even in people with kidney function.

The method of hemolysis can be used to remove amoxicillin and acidClavulanic from the circulatory system.

What to do when you forget the dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.

    Side Effects

When using Claminat 250mg, you may experience unwanted effects (ADR).

  • Common: diarrhea, itching, nausea, vomiting. ADR:

If allergic reactions are available, amoxicillin must be stopped and immediately emergency emergency with adrenalin, oxygen breathing, intravenous corticosteroids and ventilation, including endotracheal placement and never being treated with penicillin or cephalosporin anymore.

    fake colitis:

  • Mild: Stop drugs.
  • Severe (possibilities due to Clostridium difficile): Water and electrolyte, using antibiotic antibiotics against Clostridium (metronidazol, vancomycin).
  • Notify the doctor with unwanted effects when using the drug.

    Warnings

    Contraindicated

Claminat drug 250mg contraindicated in the following cases:

Hypersensitivity to the ingredients of the drug or antibiotics group penicillin, cephalosporin.

    History of jaundice.

    liver failure.

Patients with urinary phenylketon (because the drug contains amoxicillin effectively for many aspartam bacteria).

    Be cautious when taking drugs

Signs and symptoms of cholestasis jaundice are less occurring when taking the drug but may be severe. However, these symptoms are often recovered and will end after 6 weeks of stopping treatment.

Severe hypersensitivity reactions can sometimes lead to death (anaphylaxis) that have been reported in patients using penicillin antibiotics.

There has been an erythema (diverse) rash that comes with lymphadenopathy fever (mononoconemosis of infection) in patients using amoxicillin. Should avoid using drugs if suspected of monomeric hyperlemen.

Must periodically check the hematology, liver and kidney function during treatment.

Taking drugs for prolonged drugs sometimes develops resistant bacteria.

The ability to drive and operate machinery

The drug does not affect the ability to drive and operate machinery.

    Pregnancy and lactation

    Pregnancy

Experimental research on the reproductive process of animals (mice) has proved when using oral preparations that do not cause deformities. However, because there is little experience in using preparations for pregnant women, it is necessary to avoid using drugs in pregnant women, especially in the first 3 months of pregnancy, except for the necessary cases prescribed by a physician.

    Breastfeeding period

A very small amount of drugs that can be diffused into breast milk, causing hypersensitivity in children. Should consider using medicine for this object. Notify the doctor if the patient is breastfeeding.

    Medicinal interaction

Claminat 250 medicine can cause prolonged bleeding and blood clotting time.

The drug can reduce the effectiveness of oral contraceptives. Therefore should warn the patient to know this.

Probenecid reduces amoxicillin elimination but does not affect the elimination of clavulanic acid.

    nifedipine increases amoxicillin absorption.

Patients with hyperuricemia when using allopurinol along with amoxicillin will increase the ability of amoxicillin.

Bacterial pliers such as fusidlc acid, chloramphenicol, tetracyclin can reduce the bactericidal effect of amoxicillin.

Amoxicillin reduces methotrexate secretion, increases toxicity on the digestive tract and hematopoietic system.
