Indications
Clealine 50 mg is indicated in the following cases:
Pharmacy
Sertraline is a strong inhibitor and specific recovery of serotonin (5-HT) at the tip of the nerve.
In clinical doses, sertraline inhibits the recovery of serotonin into platelets in humans.
The drug has no stimulating effect, sedation or anti -cholinergic or toxic effect on the heart in animals.
Thanks to the selective inhibitory effect of 5-HT recovery, Sertraline does not enhance the activity of the catecholaminergic system.
Sertraline has no affinity for the Muscarinic receptors (Cholinergic), Serotonergic, Dopaminergic, Adrenergic, Histaminergic, GABA or Benzodiazepine.
Sertraline long -term use in animals involved in the adjustment of norepinephrine receptors in the brain as well as common in antidepressants and other haunting drugs with other clinical effects.
Unlike 3 -round antidepressants, there is no weight gain phenomenon, even a few patients also lose weight when treated with sertraline.
Sertraline proves that it is not likely to cause drug abuse.
pharmacokinetic
absorption
Sertraline has dynamic pharmacokinetic properties depending on the dose ranging from 50 - 200 mg in humans, after taking a daily dose daily in the range of 50 - 200 mg in 14 days of plasma peak concentrations (cmax) of sertraline appear in the range of 4.5 to 8.4 hours after drinking. Animal studies show that sertraline has a large distribution.
Sertraline pharmacokinetic properties in pediatric patients with haunting forced disorders have been shown to be similar to adults (although, pediatric patients using Sertraline are a bit more effective). However, it is recommended to lower the dose in pediatric patients with low body weight (especially for children from 6 to 12 years old) to avoid too high concentration in plasma.
Metabolism
Sertraline is metabolized mostly in the head phase in the liver. The main metabolites in plasma are N-Desmethylsertraline, a significant less active substance (about 20 times) compared to Sertraline on In Vitro, but there is no trial of active activity on Vivo model in depression patients.
Elimination
Semi -selling time of N -Desmethylsertraline is in the range of 62 - 104 hours. Sertraline and N-Desmethylsertraline are both metabolized largely in the human body and produces metabolites excreted through feces and through urine in unprecedented form.

How to use
Clealine 50 mg should be used once a day in the morning or evening, use the same food or no food.
Dosage
Initial treatment
Depression and obsession with obsession: The recommended dose is 50 mg/day.
Panic disorders and post -injury stress disorders: should be treated at a dose of 25 mg/day. After a week, the above dose should be increased to 50 mg, each day. This dose has been shown to reduce the frequency of the allowances at the beginning of treatment, which is a characteristic of panic disorders.
standard doseDepression disorders of obsession, panic disorders and stress disorders after injury.
Patients who do not respond to a dose of 50 mg may obtain good results when increasing the dose of treatment. The change of dose should be conducted at a distance of at least 1 week, which can increase to a maximum of 200 mg/day.
Sertraline has 24 -hour waste sale time, all changes in dosage should not be carried out at a distance of less than a week.
The beginning of treatment may be observed within 7 days. However, it is usually necessary to have a longer period of time to be able to meet clear treatment, especially in the obsessive impulsive impulse disorder.
Maintain treatment
Dosage during long -term treatment should be kept at the lowest level that is effective, then adjusted depending on the level of treatment.
Use in children
Safety and effective treatment in children have been established in pediatric patients with forced disorder that haunts the age of 6 to 17 years old.
Patients aged 6-12 years old: The starting dose should start 25 mg/day, increasing to 50 mg/day after 1 week of treatment. The next dose may increase, in case of lack of response to 50 mg/day, up to 200 mg/day if needed.
Patients aged 13 to 17: Should be started at a dose of 50 mg/day.
Should be considered before increasing the dose exceeds 50 mg/day.
Used in the elderly: The dosage can be used similar to young patients.
Patients with liver failure
Sertraline is metabolized large in the liver. Be cautious when using sertraline in patients with liver disease. Should use lower doses or increase the distance between doses in patients with liver failure.
Patients with renal failure
Most of the sertraline is metabolized in the body, only a small amount in the form of untreatedness is eliminated through the urine, so it is not required to adjust the dose according to the level of renal failure.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? The deaths have also been reported regarding the overdose of Clealine 50 mg, mainly when used in combination with other drugs and alcohol. Any overdose must be treated positively.
Overdose symptoms: Sleeping, digestive disorders (nausea and vomiting), tachycardia, tremor, agitation and dizziness, less common than coma.
There is no specific antidote, should set up and maintain a breathing line to ensure adequate oxygen and gas exchange. Activated carbon in combination with bleach can be equivalent or more effective than gastric lavage. It is not recommended to use vomiting measures. Should monitor important parameters of life and heart parallel to general symptoms and resuscitation measures. Due to the wide distribution of Sertraline in the body, measures such as mandatory diuretic, hypoclower, blood transfusion, blood transfusion are not possible.
What to do when forgetting a dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
When using the 50 mg Clealine, you may experience unwanted effects (ADR).
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the Clealine 50 mg, you need to read the user manual carefully and refer to the information below.
Contraindicated
Clealine 50 mg contraindicated in the following cases:
Caution when using
Monoamine oxidase inhibitors (IMAO): Cases with serious reactions sometimes lead to deaths that have been reported in patients using Clealine 50mg in combination with IMAO. Do not use in combination with imao or within 14 days after stopping treatment with these drugs. Similarly, it must be stopped with sertraline at least 14 days before treatment with IMAO drugs.
Other serotonergic drugs: enhance the neurotransmitter effect on the Serotonergic system, should be conducted carefully and should be avoided whenever possible due to the risk of pharmacological interaction.
switching between selective inhibitors of Serotonin recovery (SSRIs), antidepressants or anti -obsessive drugs: should be monitored and have caution assessments when converting, especially from long -lasting drugs such as fluoxetine. The time needed to clean the drug from the body before converting from a selective inhibitor to recover serotonin to another drug has not been set up.
Increase manic/decrease in the Renabitants: Rise/Reduce Hung Giac is also reported in a small percentage of patients with severe emotional disorders, treated with antidepressants and other obsessive anti -obsession drugs on the market.
Corruption: Epilepsy is a potential risk to the use of anti -obsessive drugs. However, because Sertraline has not been assessed in patients with seizures disorders, it should be avoided for patients with unstable epilepsy that have been controlled, so they should be carefully monitored. Sertraline should be stopped in any patient developing seizures.
suicide: Due to the possibility that the patient wants to commit suicide associated with depression patients and may exist until there is a significant remission, it is necessary to closely monitor patients in the beginning of treatment.
The ability to drive and operate machinery
Clealine 50 mg does not affect mental activity. Because psychotropic drugs in general can impair the ability to work in intelligence or muscles necessary for high -risk jobs such as driving or operating machinery. Patients should be cautious.
Pregnancy and lactation
Clealine 50 mg is recorded related to slow bone formation in the embryo. The drug should only be used for pregnant women when the benefits that it brings are greater than possible risks.
There are very little data related to the concentration of Clealine 50 mg in milk. There is no recommendation for medication for breastfeeding mothers unless there is a thorough assessment of the doctor that the benefits of treatment bring greater than possible risks.
Women who are at risk of pregnancy should use adequate contraception when using sertraline.
Drug interaction
Monoamine Oxidase Men inhibitors (IMAO): Do not use in combination with Disulfiram or use within 14 days after stopping treatment with disulfiram.
Central and alcoholic inhibitors: Not simultaneously used with Clealine 50 mg.
Lithium: There should be appropriate monitoring measures when using simultaneously 50 mg with lithium drugs, this substance can be effective through the activity mechanism of Serotonergic system.
phenytoin : It is recommended to monitor the concentration of phenytoin in plasma at the beginning of treatment with Clealine 50 mg and adjust the dose of phenytoin accordingly.
Sumatriptan: It is necessary to take appropriate patient monitoring measures if clinically required to treat the coordination between Clealine 50 mg and Sumatriptan.
drugs that are bound to plasma proteins: Because the 50 mg Clealine is associated with plasma proteins, it is necessary to remember the potential risk of the interaction between sertraline and other drugs associated with plasma proteins.
Warfarin : simultaneously causing statistical significance of prothrombin time, the clinical significance of this effect has not been known.
There are other drug interactions: simultaneous use of 50 mg of 200 mg/day with diazepam or tolbutamide causes a bit of a change but has statistical significance of some pharmacokinetics parameters. Simultaneous use of Clealine 50 mg with cimetidine significantly reduces the clearance of sertraline. There is no interaction between a daily daily sertraline with glibenclamide or digoxin.
Electric shock treatment (ETC): There is no clinical research to establish risks or benefits of combining electrical shock and Clealine 50 mg.
