Di-Ansel 8 is indicated for treatment for patients over 12 years of age to relieve acute pain at an average level when other pain relievers such as paracetamol or ibuprofen (single) are ineffective, with or not hot:
Headache, muscle aches, bone pain, joint pain, back pain, spinal pain, toothpaste, dysmenorrhea, neurological pain, flu pain, sore throat or sinus nose, injury, sprain, postoperative pain (except for surgery at the head, abdominal) ...
Pharmacology
Paracetamol has a peripheral and fever -reducing effect, ATC code: N02be01.
Codeine is a central analgesic with weak effect, code ATC: N02AA59.
Codeine works through μ opioid receptors. However, Codeine has low affinity for these receptors, and the pain relief effect of the drug is due to the transformed codein into morphine. Codeine, especially when coordinated with other painkillers such as paracetamol, has been proven to be effective in analgesic due to acute neurological sense.
Dynamic pharmacokinetics
paracetamol is quickly absorbed through the gastrointestinal tract, which works within 1 hour after drinking, distributed into tissue and body fluid. Metabolic drugs in the liver into the form of glucuronic and sulfonic combinations. The sale time is about 2 hours. Paracetamol is excreted mainly through the renal in the form of metabolism and constant form (
The disposal time of Codeine is 2 - 4 hours. Codeine is metabolized in the liver and eliminated in the kidneys in the form of freely or in combination with glucuronic acid, excreted in stools very little. Codeine through the placenta and a small amount through the blood - brain barrier.
This combination does not see mutual influence in the process of absorption, distribution, elimination between paracetamol and codeine in the body after drinking.

Related to paracetamol
With a dose of 10 g or more can cause liver damage in adults. In the case of patients with risk factors such as long -term treatment with carbamazepin, phenobarbiton, phenytoin or regular drinking; Or the amount of glutathion in the body is exhausted, the amount of 5 g of paracetamol or more can lead to liver damage.
Symptoms of overdose at the first 24 hours: pale, nausea, vomiting, anorexia and abdominal pain. Liver damage appears clearly 12 - 48 hours after drinking. When severe poisoning, liver failure can progress into brain disease, hemorrhage, hypoglycemia and death. Acute renal failure, acute renal necrosis, arrhythmia, pancreatitis or abnormalities in metabolism of glucose may occur.
Regarding codeine
Inhibiting acute respiratory center (blue purple, reducing respiratory frequency), drowsiness, rash.
Handling
Gastric lavage, preferably within 4 hours after drinking.
n-acetylcystein is used by intravenous or taken, preferably within 10 hours after poisoning.
Use methionin, activated carbon and/or salt bleach.
Response respiration by providing controlled oxygen and respiratory support. Naloxon indications immediately by intravenously in severe cases.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

Warnings
Contraindicated
Di-Answer 8 Contraindicated for use in the following cases:
Children from 0 to 18 years old have just performed tonsils or grated procedures V.A.
Be cautious when using
should not be used during treatment with this drug.
Pay attention to patients in the following cases:
It is not recommended for children under 12 years of age to treat analgesic, due to the risk of opioid poisoning because of unpredictable changes in the transformation of codeine into morphine.
Codeine should only be used at the lowest doses that are effective and in the shortest time.
Codeine is not recommended for children with respiratory function impairment (neuromuscular disorders, severe heart or respiratory diseases, upper and upper respiratory tract infections, multiple trauma or have just undergone major surgery). These factors can worsen the symptoms of Morphin poisoning.
Contraindicated to patients with gene metabolic gene through CYP2D6 is strong or super fast due to increased risk of harmful reactions caused by Opiod poisoning, even permanent dose. These patients have the ability to convert codeine into morphine faster, leading to higher serum morphine levels than expected.
Codeine used after children's surgery from 0 to 18 years old has just performed tonsillectomy procedures and/or grated V.A can cause rare harmful reactions, but life -threatening even death. Therefore, contraindicated drugs for these patients.
Doctors need to warn patients on signs of serious skin reactions such as Stevens-Johnson syndrome (SJS), toxic skin necrosis syndrome (Ten) or Lyell syndrome, acute pustules syndrome (AGEP).
The ability to drive and operate machinery
should be careful to use the drug when driving or operating machinery because the drug reduces alertness.
Pregnancy
Di-Answer 8 can treat short-term at the recommended dose, if the benefits of the drug surpasses the risks due to the fetus.
The period of breastfeeding
In the usual dose of treatment, Codeine and active metabolites may be present in the breast milk at very low doses and does not seem to cause adverse effects on breastfed babies. However, if the patient is a super -fast CYP2D6 metabolic gene, the transformed code into morphine may be in breast milk with higher concentrations and in very rare cases that can lead to symptoms of opioid poisoning in newborns, which can be fatal. Therefore, it is not recommended for breastfeeding women.
Drug interaction
related to codeine
Due to the inhibition of cytochrom P450 enzymes, it reduces cyclosporin metabolism. Used with painkillers, sleeping pills, anti -psychotic drugs, anxiety antidepsus or depression (including alcohol) can add depression, so the dose of these substances must be reduced. Using anti -cholin secretion with codeine can cause intestinal paralysis.
Related to paracetamol
Used with anticoagulants (warfarin) increases the effects of anticoagulants.
Storage
Store in a dry place, temperatures below 30 ° C, avoid light.
