Uses

indications

Diacerein drugs are indicated in the following cases:

  • Treatment of osteoarthritis, osteoarthritis and related symptoms. Anti -osteoarthritis properties and cartilage stimulation are shown in in vitro and animal tests. Diacerein and Rhein are capable of inhibiting Interleukin-1 Beta biosynthesis in human monomers.

Studies show that Diacerein does not prevent the synthesis of Prostaglandins, Thromboxanes or Leukotrienes but have the ability to stimulate the synthesis of a special prostaglandin Alpha, a prostaglandin that is associated with cell protection in the stomach mucous membrane. At the dose of diacerein treatment inhibits the stimulation of Interleukin-1 Beta and produces nitrous oxide.

Diacerein also significantly reduces pathological changes in osteoarthritis when compared to Placebo and increases the growth of Biomedical Metabolism of TGF-Beta 1 and TGF-Beta 2, accompanied by the recovery of cartilage. Diacerein does not change the activity of the kidneys and cyclo-oxygenase enzymes of bloody urine, so it can be tolerated in patients with renal function dependent on prostaglandin (Prostaglandin-Dependent).

    Pharmacokinetics

    absorption

Diacerein's oral dosage is about 35% - 55%. Food slows down the time to reach the peak concentration from 2.4 hours to 5.2 hours (p

    Distribution

Rhein is linked to a total protein of about 99% of plasma albumin, linked to lipoproteins and gamma-silmunoglobulins. The concentration in the epidemic is about 0.3 mg/liter - 3.0mg/liter.

    Metabolism

After drinking, diacerein is thoroughly metabolized (100%) in the liver into an active substance, Deacetylate Rhein mainly goes into the whole body system. Most of these metabolites include Rhein Glucuronide and Rhein Sulfate with half a lifetime of plasma about 7-8 hours.

    Excretion

Diacerein is excreted in the form of metabolites through urinary tract about 35% - 60% in nearly 20% in the form of free Rhein and 80% is the conjugated rhein.

    How to use

Diacerein orally orally. Drink in the main meals to achieve maximum absorption.

    Dosage

Adults: Start taking 1 capsule during dinner for 2 to 4 weeks; After adjusting the dose to 2 tablets/day, divided into 2 times.

Patients with renal impairment: The dose reduction must be reduced (½ of the above dose if severe renal failure).

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do

do when overdose? It is necessary to perform symptomatic and supportive treatments such as electrolytes, if necessary.

What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.

    Side Effects

When using diacerein, you may experience unwanted effects (ADR).

    Good tolerated drugs.

Unwanted effects are diarrhea (37% of patients). Some cases of pain under the abdomen are reported. The change of dose in the early stages of treatment (2-4 weeks) can increase or reduce side effects.

Other side effects are reported, discolored urine (14.4%) and 1 case of hypokalemia, liver toxicity in acute hepatitis and Lyell syndrome.

Instructions on how to handle ADR

Notify the doctor with unwanted effects when using the drug.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Diacerein drug contraindicated in the following cases:

  • Hypersensitivity to Anthraquinon.
  • Children under 15 years old.
  • Be cautious when using

It is necessary to start the treatment dose, take a capsule during dinner for the first 2 to 4 weeks because the diacerein can cause diarrhea or loose stool.

Need to survey hematology parameters such as liver enzymes every 6 months, whether the treatment time is short or long.

    Not used with laxatives.

Patients with renal impairment: Patients with medium to severe renal impairment show significant differences such as increasing the area under the curve and reducing the total clearance of the expression when compared to normal adults. Severe impaired renal function (creatinine clearance; 10 - 17m/minute) will significantly reduce Rhein's clearance; Therefore, 50% of the Diacerein dose must be reduced for these patients.

Patients with hepatic impairment: There is no significant difference in the pharmacokinetics parameters of Rhein in plasma and in the urine, between patients with liver impairment and healthy people volunteering to participate in testing. However, cirrhosis patients can cause drug accumulation after multiple doses, so it is necessary to test the subclinical test for patients with liver failure before taking the drug.

Pediatric: Diacerein's poison has not been studied in children. Therefore, do not take medicine for children under 15 years old.

The elderly: The suggestion can be up to 100mg/day.
