Dorogyne drugs are indicated in the following cases:
spiramycin:
Spiramycin has a bactericidal effect on bacteria that are dividing cells. In serum concentrations, the drug has a bactericidal effect, but when the concentration of the drug tissue has a bactericidal effect.
The mechanism of action of the drug is the effect on the sub -units of 50S of bacterial ribosom and prevent protein synthetic bacteria. Spiramycin has an anti -Gram -positive effect such as Staphylococcus, Pneumococcus, Meningoccus, Gonoccus, Streptococcus, Enterococcus, Bordetella Pertussis, Corynebacteria, Chlamydia, Actinomyces, MyCoplasma, Toxplasma.
metronidazol:
Metronidazole is a 5-nitro-in-idazol derivative, has a broad spectrum on protozoa such as amoeba, giardia and in anaerobic bacteria.
The mechanism of action of Metronidazol is not very clear. In parasites, the 5-nitro group of the drug is reduced into toxic mediators to the cell. These substances bind to the twisted structure of the DNA molecule that breaks these fibers and eventually causes dead cells.
The effective average concentration of metronidazol is ≤ 8 micrograms/ml for most protozoa and sensitive bacteria. The minimum concentration inhibits sensitive strains about 0.5 micrograms/ml.
Metronidazol is a very strong drug in the treatment of primeval animal infections such as Entamoeba Histolytica, Giardia Lamblia and Trichomonas Vaginalis. Bacteral bactericazole on bacteroides, fusobacterium and other mandatory anaerobic bacteria but no effect on noni bacteria.
Trichomonas vaginalis infection is treated with Metronidazol oral oral. Both women and men must be treated because men can carry pathogens without symptoms.
Metronidazole causes the vaginal bacteria to return to normal, when cervicitis/vaginal inflammation and no effect on normal bacteria in the vagina.
Metronidazol treats prolonged diarrhea and giardia. Metronidazole is the first choice for acute dysentery and heavy liver abscess.
Metronidazole works well against bacteroides, so it is often used as a standard medicine for prevention of gastric and intestinal surgery.
Metronidazol is selected in diarrhea due to Clostridium difficile.
pharmacokinetic
spiramycin:
Spiramycin is incomplete in the gastrointestinal tract, the drug is absorbed about 20-50% of the dose. The peak concentration of plasma is within 2-4 hours after drinking. Peak concentration in plasma after oral 1g dose achieved 1microgam/ml. Drinking Spiramycin when there is food in the stomach greatly reduces the bioavailability of the drug. Food reduces about 70% of the maximum concentration of the drug in the serum and makes the peak time from 2 hours.
Spiramycin is widely distributed throughout the body. The drug reaches high concentrations in the lungs, tonsils, bronchial and sinuses, spiramycin less penetrated into the cerebrospinal fluid. Serum concentration has a bactericidal effect in the range of 0.1 - 3.0 micrograms/ml and the concentration of drugs in tissue has bactericidal effect in the range of 8 - 64 micrograms/ml.
Spiramycin has short distribution time (10.2 ± 3.72 minutes). The average selling time is 5 - 8 hours. The drug is excreted mainly in bile. The concentration of the drug in bile is 15-40 times larger than the serum concentration. After 36 hours, only about 2% of the total dose is found in the urine.
metronidazol:
Metronidazole absorbs quickly and completely after drinking, reaching a plasma concentration of about 10 micrograms/ml, about 1 hour after drinking 500mg. Linear relationship between dose and plasma concentration takes place within the dose range from 200 - 2000mg. The repetitive dose every 6 - 8 hours/time will accumulate drugs.
Metronidazol's semi -discharged time in plasma is about 8 hours and an integral distribution of water volume in the body (0.6 - 0.8 liters/kg). About 10 - 20% of the drug is linked to plasma proteins. Metronidazole penetrates well into tissues and fluids, into saliva and breast milk. The concentration of treatment is also achieved in cerebrospinal fluid.
metronidazole metabolizes the liver into hydroxy and acid metabolic substances, excreted in urine partially in the form of glucuronid. The metabolites still have some pharmacological effects.
Average half -life in plasma is about 7 hours. The sale time of hydroxy metabolites is 9.5 - 19.2 hours in patients with normal kidney function.
Over 90%of oral doses are eliminated through the kidneys for 24 hours, mainly hydroxy metabolites (30-40%), acid form (10 - 22%). Under 10% excreted in the form of parent substance, 14% of the dosage excreted in feces.

What to do when overdose?
Symptoms: Metronidazol taken a single dose up to 15g has been reported. Symptoms include nausea, vomiting and loss of air conditioning. The nerve effect of convulsions, peripheral neuritis has been reported after 5 to 7 days of dosage 6 - 10, 4G 2 days/time.
Management: No specific antidote, symptomatic treatment and support.

Lactation period: Metronidazol and Spiramycin through breast milk, thus avoiding the use of drugs during breastfeeding.
Drug interaction
When combined with disulfiram will lead to paranoia and mental disorders.
Be cautious when coordinating with anticoagulic drugs (warfarin), cura derivatives (vecuronium), fluoro-racil.
Metronidazol may increase blood lithium concentration, which can be immobile, so it can wrong Nelson test results.
