Uses
indications
Dronagi medicine used in the following cases:
RISDEDRONAT directly acts on the skeleton, and thus helps the bones are less likely to be broken.
risesronat combines with hydroxyapatite in the bone and acts as an anti -bone destruction agent.
At the cell level, Riseadronate inhibits the cell cancel. Normally, the cell canvel stick to the bone surface, but shows evidence of reducing bone destruction activity (for example, no rippling banks).
Morology test on mice, dogs and mice shows that Riseadronat treatment reduces bone movement (frequency of operation, that is, the proportion of bone restructuring positions is activated) and bone destruction in restructuring positions.
In clinical and clinical research and clinical research, the biochemical imprint of the bone transfer within 1 month and reaches the maximum of 3-6 months.
Dynamic pharmacokinetics
Absorption: After taken, the drug is absorbed relatively quickly (T max about 1 hour) and independent of the dose. The average bioavailability when taking tablets is 0.63% and reduces when sodium Riseadronate is taken with food. Born in men and women similarly.
Distribution: The volume of distribution in a state of stability is 6.3 liters/kg. About 24% of the drug is attached to plasma proteins.
Metabolism: There is no evidence of systemic metabolism for sodium Riseadronate.
Elimination: About half of the absorbed dose is excreted in the urine within 24 hours. The absorption drug is excreted in the form of unchanged in feces.
Before taking Dronagi 35 Agimexpharm treat and prevent osteoporosis (1 blister x 4 tablets)
How to use
Dronagi drug used oral. Take the medicine when hungry, at least 30 minutes before breakfast or at least 2 hours away from another meal or at least 30 minutes before going to bed. Swallow the tablet (do not break, suck or chew) with 1 glass of normal water (filtered water) (120 ml), keep the posture vertically for at least 30 minutes after drinking to help the drug easily reach the stomach (patients should not lie within 30 minutes after taking the drug).
Avoid drinking vitamin D, calcium with rISEDRONAT.
The important thing is that the diet should have products containing calcium and vitamin D (for example dairy preparations). Calcium and vitamin D should be supplemented if the diet is not complete.
Dosage
Dosage recommended for adults is 1 tablet of 35 mg (taken on a certain day of the week). If you forget to take 1 dose, you should take the next day after remembering, then take 1 tablet every week on the selected day. Do not take 2 tablets 35 mg in the same day.
Special patient group:
Elderly: No dose adjustment. Biological use, distribution and elimination in the elderly (> 60 years old) similar to young people.
People with kidney failure:
Compared to those with normal renal function, sodium ridedronate clearance in the kidney decreases by about 70% in patients with creatinine clearance about 30ml/ min.
Adjusting the dose is not necessary in patients with mild to medium to moderate renal failure (CLCR ≥ 30 ml/ minute). It is not recommended to use Riseadronat in patients with severe renal failure (CLCR
Children: It is not recommended to use RISDEDRONAT for children under 18 years old because there is not enough data on safety and efficiency.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Overdose:
After taking riseadronate, severe overdose can lead to reduction in calcium and serum phosphorus in some patients. In addition, some patients also show signs and symptoms of blood calcium.
Management:
Should drink milk or antacids containing magnesi, calcium or aluminum to bind to sodium riseadronate and reduce the absorption of drugs. In case of severe overdose, take the patient to the hospital immediately to handle such as gastrointestinal lavage to eliminate drugs that have not been absorbed or can be injected with intravenous calcium to restore ionized calcium, reducing signs and symptoms of blood calcium.
In an emergency, call the 115 emergency center immediately or go to the nearest local health station.
What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
Most of the unwanted effects are from mild to medium and usually do not have to stop treatment.
Common, ADR> 1/100
Notify the physician the unwanted effects when using the drug.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
contraindicated
DRONAGI drugs contraindicated in the following cases:
Be cautious when using
side effects in the gastrointestinal tract: Food and drink (except for normal water) and pharmaceuticals containing multi -chemotherapy cations (such as calcium, magnesium, iron or aluminum) can hinder the absorption of risesDronate, so it is not used at the same time when taking this medicine.
Therefore, it must be taken at at least 30 minutes before eating, taking or before taking the first medicine for the first day.
Some bisphosphonats, including riseadronat, can cause disorders in the gastrointestinal tract such as difficulty swallowing, esophagitis, gastritis, esophagus or gastrointestinal ulcer. So be cautious when taking this medication:
In patients with a history of esophageal disorders such as esophageal stenosis or esophageal belt slow or difficulty giving food through the esophagus.
In patients who cannot hold the vertical position for at least 30 minutes after taking the drug.
In patients with problems of the above gastrointestinal tract (including Barrett esophagus).
Doctors should remind patients to pay attention to adhere to the instructions for drug use to minimize the risk of the above side effects and alert the patient to stop taking the drug, report immediately to the doctor if there is a new or worse symptom.
Mineral metabolism: thoroughly treating blood calcium and bone metabolic and mineral metabolic disorders (for example, hyperthetic dysfunction, vitamin D deficiency) before starting treatment with RISEDRONAT. The addition of calcium and vitamin D is important for all patients, especially patients with paget disease with significant increase in bone rotation.
Glucocorticoid osteoporosis: Before starting to use riseadronat to treat and prevent osteoporosis caused by glucocorticoids, it is recommended to determine the status of sex steroid hormones of both men and women and consider the appropriate replacement.
Jaw bone necrosis: Jaw bone necrosis has also been reported in patients with osteoporosis bishosphonate.
Dental test with appropriate dental provision should be considered before bisphosphonate treatment in patients with risk factors (such as cancer, chemotherapy, radiation, corticosteroid, poor dental hygiene).
During treatment with bisphosphonate, patients should avoid invasive dental procedures (for example, tooth extraction, dental transplant). For patients with jaw bone necrosis while treating bisphosphonate, tooth surgery can worsen this condition. Patients who need dental procedures, stop treatment of bisphosphonate can reduce the risk of jaw bone necrosis. The suspension of bisphosphonate should be treated by a doctor and/ or a dental surgeon based on the assessment of the benefits/ risks of each patient.
Not typical femoral fractures: The typical femoral fracture has been reported with bisphosphonate therapy, mainly in patients who are treated for long -term osteoporosis. During the time patients treated with bisphosphonat, it should be advised to report any thigh pain, hip pain or groin pain and any patient with such symptoms must be assessed by the femoral fracture.
Patients with a typical crack should also be evaluated for symptoms and signs of fractures on the opposite limb. Stopping bisphosphonate treatment in patients suspected of not typical femoral fractures should be considered based on the risk/ benefit assessment of each patient.
Patients with renal failure: Riseadronate is not recommended for patients with severe renal impairment (creatinine clearance below 30 ml/minute).
The effect of the drug on the ability to drive and operate machinery
has not seen any adverse effects on the person who is driving and operating machinery.
Use drugs for women during pregnancy and lactation
Do not use this drug for pregnant or lactating women. Only use when benefits bring higher risk to fetus or breastfeeding.
Medicinal interaction
Take taken simultaneously with drugs containing polymer chemotherapy (such as calcium, magnesium, iron and aluminum) can hinder the absorption of Riseadronat. These drugs should be taken at another time during the day without coinciding with drinking rises.
Can use Riseadronat simultaneously with estrogen supplements if considered carefully.
Riseadronate is not metabolized in the body, does not cause P450 enzyme and is less attached to protein.
Acetylsalicylic or NSAID acid: Among those who regularly use acetylsalicylic or NSAID acid (3 days or more per week) The ratio of gastrointestinal effects in patients treated with Risedronate is similar to the control patients (using Placebo).
