Uses

indications

Dudencerr 20 drugs are indicated in the following cases:

  • Acidic indigestion reduced symptoms.
  • Treatment of steroidal anti-inflammatory drugs. Cholinergic resistance or H2 receptor resistance of histamine, which inhibits the excretion of gastric acid by inhibiting the H+/K+ ATPASE enzyme system at the excretion surface of the stomach wall.

Because this enzyme is considered as an acid pump in the gastric mucosa, Omeprazol is considered as a acid pump inhibitor of the stomach, blocking the final step of acid production. This effect is related to the dose and leads to inhibition of both basic acid secretion and acid secretion when stimulated by any stimulating agent.

    pharmacokinetics

omeprazol is absorbed quickly but often changes after drinking. The absorption is not much affected by food. Omeprazol is an unstable and pharmacokinetic acid of different forms of preparation to improve oral bioavailability.

The absorption of omeprazol depends on the dose of increasing the dose of over 40mg has been reported to increase plasma concentrations in non -linear form due to the metabolism through the liver which is saturated for the first time. Moreover, the bioavailability of the drug is higher after use for a long time.

Omeprazol's bioavailability may increase in older patients, in some ethnic groups such as Chinese people and in patients with liver failure, but not significant effects in patients with renal failure.

After absorption, omeprazol is almost completely metabolized through the liver, mainly thanks to the Cytochrom P450 enzyme system with ISOENZYM CYP2C19 into hydroxy-andprazol form and a small amount thanks to CYP3A4 into omeprazol sulfon form.

The metabolites are not active in the urine and a smaller amount through the bile. Selling time for plasma is about 0.5-3 hours. Omeprazole is connected to plasma proteins about 95%.

    How to use

Take oral use. Drugs should be taken in whole and should not be ground or chewing, should be taken at least 1 hour before meals.

    Dosage

    Reduce acid indigestion symptoms:

  • 10 or 20 mg/day for 2-4 weeks.
  • Treatment of gastroesophageal reflux syndrome (GERD):

  • The usual dose: 20mg x 1 time/day for 4 weeks, another 4-8 weeks if not completely cured. In the case of persistent esophagitis, the dose of 40 mg/day can be used.
  • The maintenance dose after the esophagitis is 20mg x 1 time/day and for acid reflux is 10 mg/day.

    Treatment of gastrointestinal ulcers:

  • Single dose: 20 mg/day, or 40 mg/day in case of serious illness. Continue treatment for 4 weeks for duodenal ulcer and 8 weeks for stomach ulcers.
  • Maintenance dose: 10-20 mg x 1 time/day.
  • To kill Helicobacter pylori in gastrointestinal ulcers: Omeprazol can be combined with other antibiotics in double or three drugs. Double therapy: Omeprazol 20mg x 2 times/day for 2 weeks.

  • Therapy: Omeprazol 20mg x 2 times/day for 1 week.
  • Treatment of steroid anti -inflammatory stomach ulcers:

  • 20 mg/day, a dose of 20 mg/day is also used for prevention in patients with a history of a peptic damage that needs to continue treatment with non -steroid anti -inflammatory drugs.
  • Zollinger-Elison syndrome:

  • 60 mg x 1 time/day, adjust when necessary.
  • Most patients are effectively controlled at the dose of 20-120 mg/day, but the dose may be used up to 120mg x 3 times/day.
  • Dosage daily over 80mg should be divided into 2 times.
  • Preventing acid inhalation during the usual anesthesia:

  • The dose of 40mg in the evening before surgery and another 40mg 2-6 hours before proceeding.
  • Patients with renal function: No dose adjustment in patients with renal function.

Patients with liver failure: 10-20 mg/day.

Elderly (> 65 years): No dose adjustment in the elderly.

Children: Omeprazol treatment experience in children is limited.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? Indifference, depression, confusion are also described in individual cases.

Symptoms described by overdose Omeprazol are only fleeting and have no serious consequences caused by the report. The speed of excretion does not change (first of the dynamic) when increasing the dose. If necessary, symptomatic treatment.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

Instructions on how to handle ADR:

When encountering side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    contraindicated

omeprazol is contraindicated in patients with hypersensitivity to the drug, Esomeprazol, or other benzimidazol derivatives (such as Lansoprazol, Pantoprazol, Rabeprazol) or any component in the formula.

    Caution when using

If the patient has symptoms such as heavy weight loss, prolonged vomiting, difficulty swallowing, vomiting or black stools), suspicion or gastric ulcer, it is necessary to eliminate the possibility of malignant tumor because the treatment may mitigate symptoms and make late diagnosis.

It is not recommended to use a combination of proton pump inhibitors (PPI) and Atazanavir. If the combination of PPI and Atazanavir is considered necessary, closely monitoring clinical manifestations (such as virus infection) in combination with increased atazanavir dose to 400mg and 100mg ritonavir, do not increase the dose of omeprazol.

As other acidic blockers, omeprazole can reduce the absorption of vitamin B12 (cyanocobalamin) due to the reduction or deficiency of hydrochloric acid. Therefore, this factor should be considered in patients with a decrease in body reserve or a risk of reducing vitamin B12 absorption if treated for a long time.

omeprazol is CYP2C19 inhibitor. When starting or ending treatment with omeprazol, the risk of drug interactions should be taken into account with metabolic drugs through CYP2C19. When monitoring drug interactions between clopidogrel and omeprazol, the clinical correlation of this interaction is unclear. However, to prevent, should not be used simultaneously clopidogrel and omeprazol.

There has been a report on severe blood maggesi on patients treated with PPIs for at least 3 months and most cases of treatment for about 1 year. Symptoms of severe blood magmesi may occur such as fatigue, muscle spasms, delirium, convulsions, dizziness and ventricular arrhythmia but the above symptoms may occur dull and not be noticed. In the majority of patients with hypoglycemia, the disease is improved after supplementing the magnesium and stopping using PPIs.

Be careful to measure magnesium levels before starting treatment and periodically while treating patients who need long -term treatment or must use ppi simultaneously with digoxin or drugs that cause blood lower blood (for example diuretics).

Using PPIs, especially when taking high doses and for a long time (> 1 year), may increase the risk of hip, wrist and backbone fractures, mainly occurs in the elderly or when there are other risk factors. C

The observation research shows that ppi increases the risk of common fractures from 10 to 40%, some can be due to other factors. Patients at risk of osteoporosis must be taken care of under the existing clinical instructions and need adequate vitamin D and calcium.

The ability to drive and operate machinery

omeprazole hardly affects the ability to drive or operate machinery. There may be unwanted effects such as dizziness and visual disorders. At that time, patients should not drive or operate machinery.

    Pregnancy

Although experimental research does not see omeprazol is likely to cause deformities and toxic to the fetus, but should not be used for pregnant women, especially in the first 3 months.

    Demonstrate period

    Not recommended.

    Drug interaction

omeprazol can prolong the excretion of diazepam, phenytoin and warfarin.

Reduce the absorption of drugs dependent on gastric acid such as dasatinib, ketoconazole and otraconazole.

Using omeprazol along with clarithromycin may bring benefits in Helicobacter pylori infection treatment.

Consider reducing the dose for patients treated simultaneously fluvoxamine and ppis.

    Storage

In closed packaging, cool dry place, avoid light, avoid moisture temperature not exceeding 30⁰C.

To be out of reach of children.
