    Pharmacokology

Esomeprazol is attached to H+/K+-Aatpase (also known as proton pump) in the walls of the stomach wall, inactivating this enzyme system, preventing the final step of the excretion of hydrochloric acid secretion into the stomach heart.

    Dynamic pharmacokinetics

Esomeprazol absorbed quickly after oral, reaching the highest concentration in plasma after 1-2 hours. Esomeprazol's bioavailability increases according to the dose and when used to repeat, reaching about 68% when taking the dose of 20 mg and 89% when taking the dose of 40 mg.

Food slows the ability to absorb Esomeprazol, but does not change the effect of the effect of the drug on the acidity in the stomach. About 97% Esomeprazol attaches to plasma proteins. The drug is metabolized mainly in the liver thanks to the Cytochrom P450 enzyme system, the isenzyme CYP2C19. Selling time is about 1.3 hours.

    contraindicated

EMANERA drug contraindicated in the following cases:

  • Hypersensitivity to Esomeprazole, Benzimidazole or any ingredient of the drug.

    Be cautious when using

when there is any alarm symptom (such as significant weight loss, recurrent vomiting, difficulty swallowing, vomiting of blood or black stools) and when suspected stomach ulcers or stomach ulcers, should eliminate malignant diseases because treatment with Emanera can mitigate symptoms and delay the diagnosis of the diagnosis.

Patients with long -term treatment (especially those who have been treated for more than 1 year) should be monitored regularly.

Patients treated when necessary (on-demand treatment) Contact your doctor if there are changes in the properties of symptoms. When prescribing Esomeprazol in the treatment mode when necessary, it is advisable to consider interaction associated with other drugs due to the change in the plasma Esomeprazol level.

When prescribing Esomeprazol to eliminate Helicobacter pylori, drug interactions should be considered in the 3 -drug treatment regimen.

This drug contains sucrose. Patients with rare genetic problems such as fructose intolerance, malventa-galactose absorption or deficiency of sucrase-isomtase should not be taken.

Proton pump inhibitors may increase the risk of gastrointestinal tract infections caused by Salmonella and Campylobacter.

It is not recommended to simultaneously use Esomeprazole with Atazanavir.

Esomeprazole, as well as other antacids, can reduce the absorption of vitamin B12 (cyanocobalamin) due to a reduction or lack of gastric acid.

There have been reports on severe blood magnesium reduction in patients treated with proton pump inhibitors (PPI) such as Esomeprazole for at least 3 months, and in most cases, they often start silently and are not concerned.

Proton pump inhibitors, especially when taking high doses and for a long time (> 1 year), may increase the risk of thigh, wrist and spinal bone fractures, especially in elderly patients or when there is a presence of other risk factors.

The ability to drive and operate machines

Emanera does not affect driving and operating machinery.

    Pregnancy

should be cautious when prescribing pregnant women.

    Breastfeeding period

should not use Emanera during breastfeeding.

    Interactive drug

like other drugs that reduce other stomach acid, the absorption of drugs such as ketoconazol, iTraconazole and erlotinib may decrease and the absorption of digoxin may increase during treatment with ecomeprazol.

There is a report that Omeprazol interacts with some protease inhibitors.

Therefore, when Esomeprazol is used with metabolic drugs with CYP2C19 such as diazepam, citalopram, imipramin, clomipramin, phenytoin ..., the concentration of these drugs in plasma can increase and need to reduce the dose.

When concurrent use Esomeprazol with a CYP3A4 inhibitor, Clarithromycin (500 mg, 2 times/day) doubled the area under the curve (AUC) of Esomeprazol.
