Uses

Indications

Cerciorat is an emergency contraceptive drug within 72 hours after intercourse without contraception or method of contraception that fails.

Pharmacokic

levonorgestrel is a synthetic progestogen derived from Nortestoren. The drug has the same effect as the same as it describes with progestogen in general, but is a stronger ovary inhibitor than Norethisteron. With internal uterine wear, the drug changes the proliferation phase due to estrogen to the period of secretion. The drug increases body temperature, creates hypopasses changes in the vaginal epithelium, relaxes the uterus smooth muscle, stimulates the development of mammary cysts and inhibits the pituitary gland. Like other progestogen, levonorgestrel has many metabolic effects, the drug can reduce the amount of high density lipoprotein (HDL) in the blood.

levonorgestrel is often used for contraception. The contraceptive effect of levonorgestrel can be explained as follows: The drug changes the cervical mucus, creating a barrier that prevents sperm movement into the uterus. The nesting process of eggs is prevented due to changes in the structure of the endometrium. There is evidence that suggesting that the function of the royal body is reduced also contributed a part to the contraceptive effect. Using contraceptive, levonorgestrel is used in the form of oral contraception only progestogen or in combination with estrogen in oral contraceptive oral contraceptive. Levonorgestrel is also used as a long -term contraceptive pills by placing under the skin into the uterus.

Pharmacokinetics

absorption

After drinking, Levonorgestrel is absorbed quickly and completely completely.

Distribution

After taking 1 Cerciorat emergency contraceptive pill, the maximum serum concentration of levonorgestrel is 18.5ng/ml achieved after 2 hours. After reaching the maximum in the serum, the concentration of levonorgestrel has decreased with an average selling time of about 26 hours.

Metabolism

levonorgestrel is not excreted in a constant form, excreted in the form of metabolites.

Elimination

Levonorgestrel metabolites are excreted in approximately equal ratios in urine and feces. Levonorgestrel is metabolized in the known steroid metabolic pathway, for example, levonorgestrel is hydroxylated in the liver and its metabolites are discharged in the form of glucuronid.

There is no active metabolic substance found.

levonorgestrel is linked to plasma albumin and with globulin bonding sex hormones (SHBG). Only about 1.5% of plasma concentrations are in the form of free steroids, while 65% special connected to SHBG.

The absolute bioavailability of levonorgestrel is identified as almost 100% of the dose.

About 0.1% of the mother's dose can be transferred to breastfeeding.

    Contraindicated

Cerciorat drugs contraindicated in the following cases:

  • Hypersensitivity to active ingredients or any excipients of the drug.
  • Precautions when used

Emergency contraceptive pills only used when intercourse without protective measures and does not replace regular hormone contraceptives.

Emergency contraceptive pills cannot be effective in all cases. If not sure about the time of intercourse there is no protection method or if the woman had a relationship without protective measures more than 72 hours earlier in the same menstrual cycle, maybe they were pregnant, so Cerciorat treatment after the second intercourse, may not be effective in contraception. If the menstrual cycle is more than 5 days later or abnormal bleeding occurs at the expected date of menstruation or suspicion of pregnancy for any other reason, should eliminate the possibility of pregnancy.

If pregnant occurs after using Cerciorat, it is advisable to consider the possibility of ectopic pregnancy. The risk of ectopic pregnancy is absolutely very low, because Cerciorat prevents ovulation and fertilization. Despite the uterine hemorrhage, it is still necessary to take into account the possibility of ectopic pregnancy.

Therefore, Cerciorat is not recommended for patients at risk of ectopic pregnancy (with a history of fallopian tubes or ectopic pregnancy).

Cerciorat is not recommended in patients with severe liver dysfunction.

Severe malabsorption syndrome, for example Crohn's disease, can reduce the effectiveness of Cerciorat.

After using Cerciorat, the menstrual cycle is still normal and occurs on expected date. Sometimes it may occur sooner or later. Women need to be consulted by a doctor when they start using or finding a more appropriate daily contraceptive method. If there is no menstrual period in the daily contraceptive pills, it is more appropriate. If the menstrual period is not in the rest of the hormone, the hormone is used daily after using levonorgestrel, so eliminating the possibility of pregnancy.

Taking medication continuously within a menstrual cycle is not recommended because of the ability to cause cycle disturbance.

Cerciorat is not as effective as other regular contraception and is indicated for emergency contraception. Women who use repeated emergency contraception should be advised to use long -term contraception. Emergency contraceptive pills do not replace the necessary preventive measures to avoid sexually transmitted diseases.

The drug contains lactose monohydrate. Patients with rare genetic problems are galactose tolerance, lactose deficiency or malposure-galactose should not use this drug.

Using drugs for women during pregnancy and lactation

Pregnant women: Cerciorat should not be used for pregnant women but no abortion effect. If you continue pregnancy, the existing epidemiological data shows no adverse effects on the fetus but there is no clinical data on the possible consequences if the dose is greater than 1.5mg levonorgestrel.

Lactating women: Levonorgestrel is excreted into breast milk. The fetal levonorgestrel exposure may be reduced if the mother uses levonorgestrel immediately after breastfeeding and avoiding breastfeeding after taking the drug.

The effect of the drug on driving and operating machinery

There is no evidence of the effect of the drug on driving and operating machinery.

    Medicinal interaction

Metabolism of levonorgestrel increases when used simultaneously levonorgestrel with liver enzyme induction drugs, most of which are enzyme induction drugs CYP3A4. At the same time, using Efavirenz with levonorgestrel reduces the level of levonorgestrel in plasma by about 50%.

Other drugs suspected to reduce the same levonorgestrel levels in plasma including barbiturat derivatives (including primidon), phenyltoin, carbamazepin, drugs of Hypericum Perforatum (St. John's Wort), Rifampicin, Ritonivir, Rifabutin and Griseofulin.

Women who have used enzyme induction drugs for 4 weeks earlier, use emergency contraception is a method of contraception without hormones (for example, placing an IUD in the uterus). For women who cannot or are not ready to place an IUD in the uterus, can take double the dose of levonorgestrel (meaning 3000microgam within 72 hours after unsafe intercourse). However, the coordination of double -dose levonorgestrel and enzyme induction has not been fully studied.

Medicines containing levonorgestrel may increase the risk of ciclosporin poisoning due to ciclosporin metabolism inhibitors.

Mutual correspondence: Because there is no study of the correlation of the drug, not mixing this drug with other drugs.
