    Pharmacokology

Esomeprazole is the S-isomer of Omeprazole and reduces the excretion of stomach acid with a specialized mechanism of action. The drug is a specific inhibitor of acid pump in the wall of the wall. Both types of isomers R-and S-S- of Omeprazole have similar pharmacological effects. Esomeprazole is a weak alkaline, concentrated and transformed into an active form in a high acidic environment in the sub-traces of the cell's secretion, where H+K+-Aatpase inhibitors (acid pumps) and inhibit both basic fluid and stimulating fluid.

    Dynamic pharmacokinetics

Esomeprazole absorb fast, plasma peak concentration achieved after 1-2 hours. Absolute bioavailability is 50% after taking a single dose of 20 mg and increasing by 68% after taking the dose once a day. Esomeprazole is completely metabolized through the Cytochrome P450 (CYP) system. Esomeprazole completely eliminated from plasma between doses without accumulating when used once a day.

The average peak concentration of the drug in the corresponding oral form in plasma is about 4.6 mcmol/l. The main metabolites of Esomeprazole do not affect the secretion of stomach acid. About 80% of Esomeprazole oral dose is excreted in the urine in the form of metabolites, the rest through the feces. Less than 1% of the drug is found in urine in constant form.

    Treatment of Zollinger Ellison syndrome

The recommended starting dose is 40 mg x 2 times/day. The dose then should be adjusted by individuals and continue treatment according to clinical signs.
