Uses
indications
Esocon 20 drugs are indicated in the following cases:
Position and impact mechanism
Esomeprazole is a weak base, which is transformed into an active form in a high acidic environment in the cellular control tube of the cell. Here H+- K+- ATPase inhibitors (acid pump) and inhibit both basic and stimulating fluids.
impact on gastric secretion
After taking the dose of Esomeprazole 20 mg, 1 time/day for 5 days, the maximum maximum excretion after stimulation by Pentagastrin decreases 90% when measured at 6 - 7 hours after taking the drug on the 5th day.
After 5 days of taking the oral dose of Esomeprazole 20 mg and 40 mg, the pH in the stomach> 4 has been maintained within an average time of 13 and 17 hours in 24 hours in patients with symptoms of gastroesophageal reflux. The proportion of patients maintaining pH in the stomach> 4 minimum for 8, 12 and 16 hours equivalent is 76%, 54%, 24% for Esomeprazole 20 mg and 97%, 92% and 56% for Esomeprazole 40 mg.
It has been proved to have a relationship between acid secret inhibition and concentration and contact time through parameters representing plasma concentrations as AUC.
The therapeutic effect of acid secretion
When using Esomeprazole 40 mg, about 78% of esophagitis patients are healed after 4 weeks and about 93% are healed after 8 weeks. For patients infected with H. Pylori treated with Esomeprazole 20 mg, 2 times/day in combination with appropriate antibiotics in 1 week, Helicobacter Pylori has successfully recovered at about 90% of the disease. After treatment for eradication within 1 week, no additional anti -acid anti -acid medications to heal ulcers and reduce symptoms in patients with uncomplicated duodenal ulcer.
Other effects related to acid secretion
During treatment with gastric acid -secreting drugs, serum levels increased in response to gastric acid reduction.
Increasing the number of ElC cells is probably due to increased serum gastrin levels that have been recorded in some patients during long -term treatment with Esomeprazole. After a long period of treatment with anti -acid discharge drugs, the gastric gland cysts are recorded relatively more often. Changes (the physiological results of strong inhibition on gastric acid secretion) are benign and recovered.
Pharmacokinetics
absorption
Esomeprazole is easily destroyed in an acidic environment and is taken in the form of intestinal solubility. In animals (in vivo), the transformation to the isomer r is negligible. Esomeprazole is quickly absorbed with peak concentration in plasma reaching about 1-2 hours after drinking. Absolute bioavailability is 64% after taking a single dose of 40 mg and increased by 89% after repeated dose/day.
For Esomeprazole 20 mg, these values correspond to 50% and 68%. Food slows down and reduces the absorption of Esomeprazole, although this does not significantly affect the effects of Esomeprazole on gastric acid secretion.
Distribution
Expected distribution integral in a healthy state is about 0.22 l/kg of body weight. Esomeprazole connects 97% to plasma proteins.
Metabolism
Esomeprazole is completely metabolized through the Cytochrome P450 (CYP) system. The main part of Esomeprazole metabolism depends on the morphological CYP2C19, forming the metabolites of hydroxy and desmethyl of Esomeprazole. The rest of the metabolic process depends on another specific homogeneous substance, CYP3A4, forming Esomeprazole sodium sulphone, the main metabolite in plasma.
The following parameters mainly reflect pharmacokinetics in individuals with CYP2C19 function yeast, a group of strong metabolic people. The total plasma clearance is about 17 l/hour after the single dose and about 9 l/hour after the dose is repeated. Selling time for plasma is about 1.3 hours after the dose is repeated once a day.
The area under the curve shows plasma concentrations over time (AUC) increases after repeatedly used Esomeprazole. This increase depends on the dose and resulting in the non -linear relationship between AUC and the dose after the dose is repeated. The dependence on this time and dose is due to the reduction of metabolism in the early stages through the liver and reducing the body clearance is probably due to Esomeprazole and/or or the metabolites inhibiting CYP2C19 enzyme. Esomeprazole completely eliminated from plasma between doses without a tendency to accumulate when used 1 time/day.
Elimination
The main metabolites of Esomeprazole do not affect the secretion of gastric acid, about 80% of esomeprazole oral doses are excreted through urine in the form of metabolites, the rest through feces. Less than 1% of constant drugs found in urine.
Before taking Esocon 20 BiPharm Treatment Treatment of stomach ulcers due to the use of NSAID drugs (5 blisters x 6 tablets)
How to useDrink whole with water.
Dosage
Gastroesophageal reflux disease (GERD)
To reduce symptoms: The usual dose is 40 mg of Esomeprazole/day for 4 weeks.
If the symptoms are still there, can add 4 more weeks of treatment.
esophagitis
To heal lesions: The dose is 20 - 40 mg, 1 time/day, for 4-8 weeks.
If not completely healed after 8 weeks, the doctor may add 4 - 8 weeks of treatment with the maintenance dose to heal the lesions of 20 mg, 1 time/day.
duodenum ulcer
Esomeprazole is a part of 3 drug therapy to treat ulcer completely caused by H.pylori: dose of 20 mg, 2 times/day for 10 days with amoxicillin and clarithromycin.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do
do when overdose? The data is limited but the single doses of Esomeprazole 80 mg are still safe to use. There is no specific detox. Esomeprazole is strongly connected to plasma proteins so it is not easily separated. In the case of overdose, symptomatic treatment should be treated and used for general support. What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Esocon 20 often has unwanted effects (ADR).
Common, ADR> 1/100
Rare, ADR
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Contraindicated
Esocon 20 drug contraindicated for patients allergies to any component of the drug.
Be cautious when using
When there is a presence of symptoms such as significant weight loss, recurrent vomiting, difficulty swallowing, vomiting or defecating black blood and having/or suspecting stomach ulcers, so eliminating malignant ability because Esomeprazole can cover symptoms and delay diagnostic.
The ability to drive and operate machinery
No report.
Pregnancy
There is no complete and controlled study for the use of Esomeprazole for pregnant women, so it should only be used for pregnant women when needed.
The period of breastfeeding
The excretion of Esomeprazole into breast milk has not been studied. Because Esomeprazole has the ability to excrete on breast milk, deciding to stop breastfeeding or stopping the drug. Should calculate the importance of the drug for the mother.
Drug interaction
Esomeprazole inhibits gastric acid secretion, so the drug can prevent the absorption of other drugs where the stomach pH is an important factor for the bioavailability of that drug (such as ketoconazole, iron salt, digoxin). Esomeprazole does not have a significant drug interaction with phenytoin, warfarin, quinidine, clarithromycin or amoxicillin, birth control pills, diazepam, phenytoin, these drugs do not change the pharmacokinetic properties of Esomeprazole when used.
Storage
Store in a cool dry place, avoid light.
