Uses

indications

Esorest 300 mg drug is indicated in the following cases:

  • Neurological pain. Learning

gabapentin has a structure related to GABA neurotransmitters (Gama Aminobutyric Acid).

The drug is not converted into GABA or an antagonist with GABA, nor is it inhibitors inhibitors. Gabapentin is said to be cohesive with the subunit Alpha 2 Delta unit of calcium channel. Gabapentin's pain relief mechanism is not well known, but on animal pain relief experiments shows gabapentin prevents allodynia (responding to pain with normal stimuli) and an increase in pain (overwork response to pain).

Special gabapentin prevents pain response in several nervous experiments in white mice and mice (e.g. experimental spinal spinal belt experiments, experiments that cause spinal cord damage, acute zoster herpes infection). Gabapentin also reduces pain after peripheral inflammation (the late period of Formalin). Gabapentin does not change the painful pain reaction immediately (testing in mice, Formalin test, abdominal contractions with acetic acid, heart radiation test).

There have been several animal design testing systems to record gabapentin anti -seizures. Like other anti -convulsions, gabapentin reduces excessive discharge and reduces the spread of stimulus from central nervous nerves.

    Pharmacokinetics

    absorption

Gabapentin serum levels correspond to the dose between 300 mg and 400 mg, treated every 8 hours. The absorption of gabapentin is saturated and non -linearly related to the dose.

Gabapentin's bioavailability is reduced at higher doses, about 60% at a dose of 300 mg and about 42% after taking the dose of 800 mg.

Food does not affect Gabapentin's pharmacology.

Gabapentin's pharmacokinetics are not affected by repeated dose and stable state of plasma concentration that can predict the single dose.

    Distribution

Maximum concentration in plasma (e.g. 4 - 5.5 ng/ml according to the daily dose of 900 - 1200 mg) of Gabapentin is achieved within 2 to 3 hours with the dose of treatment.

Gabapentin's percentage is under 3 % less than 3 % snowy citizens. The distribution of the drug is 57.7 liters. Patients with epilepsy Gabapentin concentration in the central nervous system approximately 20% of stable concentration in plasma is low.

    Biological metabolism

Gabapentin is not metabolized in humans and does not cause Cytochrom P450.

    Elimination

Gabapentin's waste sale time depends on the dose and the time is 5-7 hours. In older patients, renal function changes related to age causing reduced plasma gabapentin clearance and increasing gabapentin waste time. Patients with renal function, excretion rate in the kidneys completely corresponds to the creatinine clearance. Gabapentin is eliminated from plasma by hemolysis.

    How to use

    Take oral use.

    Dosage

    epilepsy

Adults and children over 12 years:

  • First day: 300 mg x 1 time.
  • Monday: 300 mg/time x 2 times.
  • day 3: 300 mg/time x 3 times.
  • The usual dose: 900 - 1200 mg/day is divided into 2-3 times. Be careful to avoid using the distance between 2 doses of drugs for more than 12 hours. In the case of older patients may increase the dose until treatment is effective.
  • The starting dose is proposed 100 mg in the evening and increases to 100 mg daily to 600 mg divided into 2-3 times. An additional dose may depend on the patient's ability to respond.
  • Older patients: careful monitoring of possible adverse cases to patients. Older patients can adjust the dose depending on the function limit of the kidney (creatinine clearance).

Children under 12 years: Safety and efficiency have not been established.

Patients with renal failure should be prescribed carefully, depending on the clearing of creatinine because the gabapentin secretion is reduced in patients with renal impairment.

Gabapentin can be used simultaneously with phenobarbital, phenytoin, valproic acid and carbamazepin that does not change the concentration of plasma or serum of gabapentin or other anti -epileptic groups.

Stop gabapentin as well as the addition of other therapies, should be done slowly with a minimum dosage for each week.

    Neuria

Adults: Take no more than 1800 mg, divide 3 times, or use as follows:

  • First day: 300 mg x 1 time.
  • Monday: 300 mg/time x 2 times.
  • day 3: 300 mg/time x 3 times.

Elderly: The dose may be low due to poor kidney function.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What do

do when using overdose? All patients recover after supportive treatment. Gabapentin can be eliminated through hemorrhage, although there have been some cases of overdose treatment without using hemorrhage. This procedure is indicated depending on the clinical condition of the patient, or in patients with signs of severe renal failure.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

  • Side Effects

When using Esorest 300 mg, you may experience unwanted effects (ADR).

Gabapentin tolerates well. Side effects are usually mild or medium and tend to gradually decrease when continued treatment. The most common side effects for nerves and are often the cause of stopping.

    Common, ADR> 1/100

Instructions on how to handle ADR

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Esorest 300 mg is contraindicated in the following cases:

  • Hypersensitivity to Gabapentin or any ingredients of the drug.
  • Children under 12 years old.
  • Pregnant and lactating women.

    Caution when using

should not suddenly stop anti -epileptic drugs due to the ability to increase the frequency of epilepsy again. During the study before bringing the drug to the market, there were 8 cases of sudden death of unknown reasons in a population of 2203 patients treated with Gabapentin (of which 2103 patients were treated for many years).

Patients should be recommended that gabapentin can cause dizziness, drowsiness and other signs as well as central nerve inhibition symptoms. Therefore, when using the drug, you should not drive or operate machinery until it is sure that there is no mental and movement side effect.

In patients who need treatment in combination with morphine, gabapentin levels should be noted. Patients should be monitored by central nerve inhibitors such as drowsiness, and the dose of gabapentin or morphine should be adjusted appropriately. When combined with gabapentin with other epilepsy medications, it has recorded a fake positive result with an Ames N dip stick - Multistix SG SG detection of proteinuria, so it is recommended to use a more specific sulfosalicylic precipitate test to evaluate the presence of proteinuria.

The ability to drive and operate machinery

Patients should be recommended that gabapentin can cause dizziness, drowsiness and other signs as well as central neurological inhibitors. Therefore, when using the drug, you should not drive or operate machinery until it is sure that there is no mental and movement side effect.

    Pregnancy

Safety and effectiveness of Esorest 300 mg has not been established.

    Breastfeeding period

The safety and effectiveness of Esorest 300 mg has not been set.

    Drug interaction

There is no interaction with other anti -epileptic drugs such as phenobarbital, phenyoin and valproic acid. Gabapentin's pharmacokinetics in the same stable state in healthy people and epilepsy patients are using anti -epileptic groups.

Simultaneous treatment of gabapentin with oral contraceptives contains norethindron and/or ethinyl estradiol, does not affect the pharmacokinetic properties of any component.

Antacids containing magnesi, and aluminum reduces the bioavailability of gabapentin about 24%. Gabapentin should be recommended after about 2 hours.

Gabapentin's renal tubules are not affected by probenecid.

When using gabapentin with cimetidin shows a mitigation of gabapentin excretion in the kidney, this is not important in treating disease.

    Storage
