Uses

Indications

Fexofenadin drugs are indicated in adults and children aged 12 and older to reduce symptoms related to seasonal allergic rhinitis.

Pharmacokological

Fexofenadin is a second -generation antihistamine drug, with specific antagonistic and selective effects on peripheral H1 receptors.

The drug is a metabolic substance of terfenadin, also competing with histamines at H's receptors, in the digestive tract, blood vessels and respiratory tract, but no toxicity for the heart due to not inhibiting potassium channels associated with heart protection.

Fexofenadin has no significant effects on acetylcholin, dopamine antagonism and no effect inhibiting alpha receptors, or beta adrenergic. At the treatment dose, the drug does not cause sleep or affects the central nervous system. The drug has a fast and prolonged effect due to slow -attached drugs into receptors H, forming a sustainable complex and separating slowly.

pharmacokinetics

Good absorbing drugs when taken oral and started to work after drinking 60 minutes. The peak concentration in the blood is achieved after 2 to 3 hours. Fat -rich foods reduce the peak concentration in plasma by about 17% and extend the time to reach the peak concentration of the drug (to about 4 hours). The antihistamine effect lasts more than 12 hours. The associated ratio of plasma proteins of the drug is 60 to 70%, mainly with albumin and alpha - Glycoprotein acid. Fexofenadin does not pass the brain barrier.

Fexofenadin is very less metabolized (about 5%, mainly in the intestinal mucosa. Only about 0.5 to 1.5% are metabolized in the liver thanks to the cytochrom P450 enzyme system into non -active substances). About 3.5% of the dose of fexofenadin metabolized through phase II (not related to the cytochrom paso enzyme system) into methyl ester derivatives.

This metabolite is only seen in the section, so there may be the participation of intestinal bacteria in this metabolism. Half of the excretion of Fexofenadin is about 14.4 hours, longer (31 - 72%) in kidney failure.

Mainly eliminated drugs (approximately 80%) and urine (11 -12%) in constant form.

Pharmacokinetics in people with renal failure:

  • CLCR 41 - 80ml/minute: The peak concentration is higher than 87%, half -life eliminated longer than 59%. Strong.
  • Before taking Fexofenadin 60 Pharimexco treats allergic rhinitis (1 blister x 10 tablets)

    How to use

Oral, tablet tablets with water. The time to take the drug does not depend on the meal. Do not take medicine with fruit juice.

    Dosage

    Adults

recommended dose for adults is 120 mg x 1 time/day (taking 2 tablets Fexofenadin 60 or taking 1 tablet of Fexofenadin 120) before meals.

Fexofenadin is the active metabolic substance of terfenadin.

    Children

Children aged 12 years and older: The recommended dose for children aged 12 and older is 120 mg x 1 time/day. Take 2 tablets Fexofenadin 60 or take 1 tablet Fexofenadin 120) before meals.

Children under 12 years: Not enough data on the effectiveness of drugs when used for children under 12 years old.

Elderly patients, kidney failure, liver failure

No dose adjustment in these patients.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.

    What to do when overdose?

Information about fexofenadin level toxicity is limited. However, drowsiness, dizziness, dry mouth have been reported.

    How to handle

Use conventional measures to remove the removed drugs that have not been absorbed in the digestive tract. Support support and symptomatic treatment. Hemolysis is negligible in blood concentration (1.7%). There is no specific antidote.

In an emergency, call the 115 emergency center immediately or go to the nearest local health station.

What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

The unwanted effects of the drug are not affected by the dosage, age, gender, and race of the patient.

    Common, ADR> 1/100

  • Mental: Drowsy, tired, headache, insomnia, dizziness. 1/100
  • Neurological: Fear, sleep disorders, nightmares.
  • Digestive: dry mouth, abdominal pain.
  • Skin: Ban, urticaria, itching.

ADR of the drug is usually mild, only 2.2% of patients have to stop the drug due to ADR of the drug.

  • Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

Fexofenadin drug is contraindicated in the following cases:

  • Patients with a history of allergies/hypersensitivity to cefaclor and antibiotic group cephalosporin or other ingredients of the drug.

    Precautions when used

Be careful and adjust the appropriate dose when using drugs for people with impaired renal function because the concentration of drugs in plasma increases due to half -life exhausted. Be careful when taking medicine for the elderly (over 65 years old) often has a decline in kidney physiology.

Safety and effectiveness of drugs in children under 6 months of age has not been identified.

It is necessary to stop Fexofenadin at least 24-48 hours before conducting antigen tests in the skin. Use fexofenadin to make psoriasis worse.

Patients with a history of cardiovascular disease or cardiovascular disease should be warned that antihistamine drugs are associated with side effects such as tachycardia and chest drum.

Lactose -containing drugs and therefore should not be used for patients with rare genetic problems such as galactose intolerance, severe or poorly absorbed lactase - Galactose.

Use drugs for women during pregnancy and lactation

    pregnancy

Because there is no complete research on pregnant women, only fexofenadin is used for pregnant women when the maternal benefits are superior to the risk of the fetus.

    Breastfeeding period

It is not clear whether the drug is excreted in milk or not, though not seeing unwanted effects in infants when nursing mothers use fexofenadin, so be careful when using fexofenadin for breastfeeding women.

The effect of the drug on the ability to drive and operate machinery

drugs that cause less drowsiness, but still need to be cautious when driving and operating machinery requires alertness.

    Drug interaction

erythromycin and ketoconazole increases fexofenadin levels in plasma but does not change the QT interval. Fexofenadin levels may be increased by erythromycin, ketoconazole, verapamil, p-glycoprotein inhibitors.

  • Not simultaneously used with antacids containing aluminum, magnesi because it will reduce the absorption of fexofenadin. Betahistin.

Cavalry: Fruit juice (oranges, grapefruit, apples) can reduce the bioavailability of fexofenadin by 36%. Avoid using fexofenadin with ethylic alcohol (alcohol) because it increases the risk of sedation (sleep).

  • Storage

Leave a cool place, avoid light, temperature below 30⁰C.
