Uses
Indications
Flutonin 10mg drug is used in the following cases:
Pharmacy
Fluoxetin is a 2 -round antidepressant, which inhibits the selective Serotonin reabsorption of the neuron cell (SSRI).
In clinical dose, fluoxetin prevents identification with serotonin but does not block norepinephrin in platelet cells.
The time is slow, until the full treatment effect usually must be from 3-5 weeks. Therefore, the case of severe depression cannot be relieved right after taking the drug.
Fluoxetin is the antagonist of Muscarinic receptors, histaminergic and A-| -Denergic, has a similar relationship with anti-anti-cholinergic, sedative and cardiovascular effects like antidepressants (3 rounds) classic. However, the cohesion of fluoxetin with these receptors and other receptors on the membrane of brain cells is inferior to 3 -round antidepressants.
Thanks to its specific effects on serotonin secretions, the side effects are common on 3 -round antidepressants (Muscarinic resistance) often occur when treated with fluoxetin.
pharmacokinetic
absorption
Fluoxetin absorbs well through the gastrointestinal tract. Oral bioavailability is about 95%. Food can reduce the absorption of the drug but does not affect the bioavailability of the drug.
Distribution
The distribution volume is about 35L/kg. With a single dose of 40mg/day by oral, the peak concentration of serum is from 15 - 55mg/ml after 6 - 8 hours. The ratio of binding to protein, albumin and A-1-Glycoprotein is 94.5%.
Metabolism
Most of the fluoxetin converts into the form of norfluoxetin activity and some other types of activity.
Elimination
Fluoxetin was originally converted into inactive substances in the liver, and then released it through the kidney (> 80%). The sale time of fluoxetin after using the only dose is about 1-3 days, but after the dose is repeated, the excretion speed decreases, half a lifetime increases to about 7-15 days. In cirrhosis patients, the half -life of fluoxetin increases from 2-3 days to 6 - 7 days 80 with normal.
Before taking Flutonin 10 Hasan treats depression (3 blisters x 10 tablets)
How to use
take pills with water, once in the morning.
If clinical signs do not progress after a few weeks of treatment, the dose may increase. If used over 20mg/day, it should be divided into 2 times and should not be used more than 80mg/day. Like other depression, the effect of the drug is only after a few weeks (4-6 weeks) of treatment with the given dose.
Should take lower doses or interrupted for patients with liver, kidney, elderly, patients with many diseases simultaneously or treat with many different pharmaceuticals.
With acute depression, drug treatment time can last from a few months to longer. Currently, we can not know the exact dosage suitable for antidepressants used in maintenance treatment.
Dosage
Adults
Depression disorders
Use 20mg/time/day, drink once in the morning.
The maintenance dose is vary depending on the clinical response of each patient. Usually after a few weeks to achieve adequate treatment effect, so do not increase the dose regularly.
Disorders of obsession - Forced
Initial dose 20mg/day.
If necessary, the dose can be increased to 60mg/day.
Invisible food
Use 60mg/day.
panic
Starting dose 10mg/day.
After 1 week can increase the dose to 20mg/day, can increase the dose to 60mg/day.
Maximum daily dose: 80mg/day.
Children
The starting dose is usually 10mg/day.
After 1 week of use can increase the dose to 20mg/day if not effective treatment.
Short -term treatment time (8 - 9 weeks).
Older patients:
Daily dose does not exceed 60mg/day.
Patients with mild weight, liver or kidney function
Must reduce the dose, can be used 10mg/day.
What to do when overdose? When overdose, the symptoms are mainly nausea and vomiting. Also see symptoms of excitement, mild mania and central nerve stimulation signs.
Treatment: mainly symptomatic treatment and support. Use activated carbon and sorbitol, maintain respiration, heart activity and body temperature. If necessary, take anti -convulsions such as diazepam .
Measures for hemolysis, mandatory diuretic or blood change are not effective due to large distribution volume and highly linked drugs with protein.
What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

Warnings
Contraindicated
Do not use Flutonin for humans:
Severe renal failure (due to rapid emissions of creatinine
Be cautious when using
fluoxetin as well as other antidepressants (SSRIs) increase the risk of behaviors and thoughts that want to commit suicide in children and teenagers suffering from depression disorders and other neurological disorders.
Caution for people with a history of epilepsy due to fluoxetin can lower the threshold causing seizures. If there are seizures, immediately stop taking the drug immediately.
Be cautious when used for patients with electric convulsions during the previous 8 weeks.
Be cautious when used for patients treated simultaneously with lithium, tryptophan, patients diabetes , heart disease and respiration.
For diabetics, during treatment with fluoxetin, the phenomenon of hypoglycemia and blood sugar may increase again when stopping the drug.
The drug can cause dizziness, headache, should not stand up suddenly while in a lying or sitting position.
There are no relationships of the effect of drugs on age, including the elderly.
The ability to drive and operate machinery
The drug can cause drowsiness, reduce the ability to consider, judge, think or mobilize, so be careful when driving, operating the machine or jobs that need to be alert.
Pregnancy and lactation
Pregnancy
Fluoxetine's safety for pregnant women has not been determined; Avoid taking drugs during pregnancy, unless no other drug is safer.
Breastfeeding period
Fluoxetine is distributed into breast milk, so it can affect breastfeeding. Fluoxetine should not be used for mothers who are breastfeeding or should not breastfeed while taking the drug.
Medicinal interaction
should not be used simultaneously fluoxetin with monoamine oxidase inhibitors such as furazolidon, processbazin and Selegilin, because they can be confused, agitated, symptoms in the gastrointestinal tract, high fever, severe seizures or hypertension.
Fluoxetin strongly inhibits the liver enzymes cytochrom P450 2D6. Concentrated with metabolic drugs thanks to this enzyme and a narrow treatment index (for example, Flecainid, Encainid, Vinblastin, carbamazepin and 3 -round antidepressants) must start or adjust these drugs at low doses. This also applies if fluoxetin was used within the previous 5 weeks.
The concentration of 3 -round antidepressants, Maprotilin or Trazodon in plasma can be doubled when used simultaneously with fluoxetin. Some physicians recommend reducing about 50% of these drugs when used simultaneously with fluoxetin.
Simultaneous use of Fluoxetin with Diazepam can last half -life of Diazepam in some patients, but physiological and mental response may not be affected.
Electrical shock treatment: Prolonged convulsions when treated simultaneously with fluoxetin.
Simultaneous use with neurological effects may increase the risk of side effects.
Periodic bonds with plasma proteins such as anticoagulant, digitalis or digitoxin, simultaneously used with fluoxetin can be pushed out of the protein bonding position, increasing the concentration of free plasma drugs and increasing side effects.
Phenytoin concentration may be increased when used simultaneously with fluoxetin, leading to poisoning, so closely monitoring the phenytoin level in plasma.
Use fluoxetin simultaneously with lithiums that can or increase or decrease the level of lithium in the blood, and there have been cases of lithium poisoning occurred. Therefore, lithium concentration should be monitored.
Storage
Store drugs in closed packaging, dry place, avoid light, at room temperature, the temperature does not exceed 30 ° C.
