indications
Freshvox drugs are indicated in the following case:
Acute and chronic gastritis, gastric ulcer and duodenum with Helicobacter pylori infection.
Pharmacokology
Esomeprazole
Esomeprazole is the S isomers of Omeprazole and reduces the excretion of stomach acid with a specialized mechanism of action. The drug is a specific inhibitor of acid pump in the wall of the wall. Both types of isomers R-and S- of Omeprazole have similar pharmacological effects.
Esomeprazole is a weak alkaline, concentrated and transformed into an active form in a high acidic environment in the sub -trailer of the cell's secretion, where H+/K+ - Atpase inhibitors (acid pump) and inhibit both basic fluid and irritability.
Clarithromycin
Clarithromycin is an antibiotic macrolid antibiotic Helicobacter pylori bactericidal effect by attaching Ribosom 50S units of sensitive bacteria and inhibiting protein synthesis, clarithromycin has in vitro activity (MIC - 90 is 0.03 μg/mL) against H. pylori, allowing for combination treatment in H. pylori.
tinidazole
Tinidazole is a drug of the group 5 - nitroimidazole has a stronger and longer effect than Metronidazol. It has the effect of fighting against monopoly microorganisms and anaerobic bacteria.
Tinidazole is considered as antibiotics with strong bactericidal activity. Tinidazole acts by the mechanism of decomposing DNA chain or synthesis inhibitors.
Dynamic pharmacokinetics
Esomeprazole
Esomeprazole absorbs fast, plasma peak concentration is achieved after 1-2 hours. Absolute bioavailability is 50% after taking a single dose of 20 mg and increasing by 68% after taking the dose once a day.
Esomeprazole is completely metabolized through the Cytochrome P450 (CYP) system. Esomeprazole completely eliminates plasma between doses without tend to accumulate when used once a day. The average peak concentration of the drug in the corresponding oral form in plasma is about 4.6 μmol/l. The main metabolites of Esomeprazole do not affect the secretion of stomach acid.
About 80% of Esomeprazole oral doses are excreted through urine in the form of metabolites, the rest through feces. Less than 1% of the drug is found in urine in constant form.
tinidazole
Tinidazole is absorbed quickly and completely after drinking. The peak concentration in plasma is achieved after 2 hours. Selling waste time in plasma is 12 - 14 hours. The drug is quickly distributed into the body tissue. About 12% of the drug associated with plasma proteins. Most drugs are excreted in urine.
Clarithromycin
After drinking, Clarithromycin is quickly absorbed through the digestive tract. Clarithromycin is widely distributed into tissues including duodenal lining. The peak concentration in the serum after taking 250 mg Clarithromycin is 0.6 mg/ml respectively for clarithromycin and 0.7 mg/ml for its basic active metabolites (14 - hydroxy clarithromycin).
Clarithromycin is metabolized mainly in the liver. Selling time for plasma is about 3-4 hours. About 20% are excreted in a constant form and 15% in the form of 14 - hydroxy clarithromycin through the kidney.
Before taking Freshvox ACME medicine for acute and chronic gastritis (7 blisters x 6 tablets)
How to use
Do not chew or crush the Esomeprazole tablet covered in the intestine but must take the pills with water. The first dose taken 30 - 60 minutes before breakfast, the second dose taken before dinner 30 - 60 minutes. Dosage
Drink 1 blister/day, divide 2 times in the morning and evening. Each time 1 Esomeprazole, 1 Clarithromycin and 1 tinidazole tablet. The recommended treatment is 7 days.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when using overdose?
In case of overdose, symptomatic treatment and supportive treatment. Need to be absorbed from the gastrointestinal tract and need to monitor patients carefully.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Do not drink twice as prescribed.
Side Effects
When using Freshvox, you may experience unwanted effects (ADR).
Sometimes unwanted effects such as headache, vomiting, nausea, diarrhea, abdominal pain.
Very rare other unwanted effects such as skin rash, inflammation, urticaria ... these unwanted effects will be lost when stopping using the drug.
Instructions on how to handle ADR
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Freshvox drugs contraindicated in the following cases:
Do not use for cases of a history of sensitivity to any ingredients of the drug, pregnant and lactating women.
Be cautious when using
It is necessary to rule out the possibility of malignant tumor in patients with stomach ulcers before treatment with Esomeprazole because Esomeprazole can obscure clinical symptoms and delay diagnosis of malignancy.
Do not use Tinidazole for people with a history of blood disorder because it can cause leukopenia.
Do not use ethylic alcohol while being treated with Tinidazole because it can cause "alcohol antabuse syndrome".
If there is a serious renal failure, with or without liver failure, clarithromycin should be reduced and prolong the medication distance.
The ability to drive and operate machinery
does not affect.
Pregnancy
Contraindications to use drugs during pregnancy.
Breastfeeding period
contraindicated medication during breastfeeding.
Medicinal interaction
Using clarithromycin for patients who are using theophyllline may increase plasma concentration.
When used with clarithromycin will increase plasma carbamazepine levels.
When used simultaneously with clarithromycin may increase the effectiveness of warfarin, leading to prolongrombin in these patients.
When used simultaneously with clarithromycin can increase the effectiveness of digoxin.
When used simultaneously with clarithromycin will increase plasma terfenadin levels. Therefore, do not simultaneously use these two drugs for patients with cardiovascular abnormalities (arrhythmia, tachycardia, prolonged QT interval, myocardial ischemia, blood congestion) or electrolyte disorders.
Ergot: Theoretically not use mushrooms simultaneously with clarithromycin.
The reduction of stomach acid when treated with Esomeprazole may increase or reduce the absorption of other drugs if the absorption mechanism of these drugs is affected by stomach acid. Like other gastric acid secretions or antacids, the absorption of ketoconazole and iTraconazole may decrease during treatment with Esomeprazole.
Esomeprazole inhibits CYP2C19, so when Esomeprazole is used with metabolic drugs through CYP2C19 (such as diazepam, citalopram, imipramine, clomipramine, phenytoin ...), the concentration of these drugs in plasma can increase and need to decrease the dose.
Avoid using alcohol when taking medicine.
Storage
Store in a cool dry place at a temperature of less than 30 ° C, avoiding light.
