Uses

indications

G5 Duratrix drugs are indicated in the following cases:

  • Treatment of reducing the risk of cardiovascular disease or brain -artery diseases (myocardial infarction, stroke and stroke) in patients with atherosclerosis manifested by recent ischemic stroke, recent myocardial infarction, or peripheral artery disease. Patients with healthy treatment and those patients undergoing their subcutaneous coronary intervention or without the conductor or grafting of the coronary artery. GPIIB/LLA, thus inhibits platelet's collection. The metabolism of clopidogrel is needed for the inhibition of plateletal condensation, but the active metabolites of the drug have not been isolated.

Clopidogrel also inhibits platelet training due to movement in other places but blocking platelet activation process from ADP release. Clopidogrel does not inhibit the operation of phosphodiesterase.

Clopidogrel is impacted by non -recovery of ADP platelet recovery. Consequently, platelets attached clopidogrel will affect the later stage of platelet life.

The inhibition of platelet collection depends on the dose, can be seen after 2 hours using the single dose. The repeated dose of Clopidogrel 75mg per day significantly inhibits platelet collection on the first day and this inhibitor reaches a stable level in the 3rd and 7th days. Platelet's collection and bleeding time gradually returning basically, usually for about 5 days after treatment.

    Pharmacokinetics

    absorption

Clopidogrel is quickly absorbed but not completely after drinking, the absorption of at least 50%.

    Distribution

Clopidogrel and derivative of carboxylic acid are strongly connected to plasma proteins.

    Metabolism

Clopidogrel is a precursor and metabolism mainly in the liver, the main metabolite is not active as the derivative of carboxylic acid. The active metabolite is a derivative of Thiol but not detected in plasma.

    Elimination

Clopidogrel and its metabolites are eliminated in the urine and in stool, after oral medication, about 50% of the dose is eliminated in the urine, 46% are eliminated in feces.

  • Before taking G5 Duratrix Ha Tay reduces the risk of myocardial infarction, stroke, stroke (5 bags x 10 blisters x 10 tablets)

    How to use

Clopidogrel is used by oral regardless of meals.

    Dosage

    Adults

To reduce the risk of myocardial infarction, stroke or stroke, the dosage in adults is 75mg/day.

In patients with acute coronary syndrome without a difference (unstable angina or myocardial blood without Q wave), the first dose is 300mg, the next dose is 75mg/day in combination with Aspirin (first of 75 - 325mg, then 75 - 162mg/day).

    Children

There is no optimal dose for children, the information about the dose in children is very limited, there should be continued studies.

    Research shows that children

For children> 2 years old, no optimal dose is recommended, but not higher doses of adults, 1 mg/kg initial dose may be used, then adjust to response.

    Other objects

Adjusting the dose in patients with renal failure, the elderly is not necessary.

Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose? If there is hemorrhage, appropriate therapy should be applied. There is no active antidote of dopidogrel. If it is necessary to quickly adjust the phenomenon of prolonged bleeding time, platelet transmission may cause the effect of clopidogrel.

What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

    Side Effects

When using G5 Duratrix, you may experience unwanted effects (ADR).

  • Respiratory system: interstitial lung disease.
  • Digestive system: abdominal pain, indigestion, anorexia, gastritis, constipation and gastric bleeding. Skin: Skin disorders, diverse roses, Stevens - Jonhson syndrome, flat licks. blood and lymphatic system: serum disease, intracranial hemorrhage, hemorrhage, bruising, hematoma, bleeding.

    musculoskeletal system: muscle pain.

Instructions on how to handle ADR

Periodically monitor the parameters of signs of anemia, hemoglobin, hematocrit during clopidogrel treatment.

Replacement of emergency plasma in case of thrombocytopenia.

When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.

    Warnings

Before using the drug you need to read the instructions carefully and refer to the information below.

    Contraindicated

drug G5 Duratrix is contraindicated in the following cases:

  • Hypersensitivity to clopidogrel or any ingredient of the drug.
  • There is pathological bleeding such as digestive ulcers, bleeding in the brain.

    Be cautious when using

    clopidogrel causes prolonged bleeding time.

Be cautious when used for patients at risk of bleeding due to trauma, surgery, ulcers, or other pathological conditions, especially the stomach or internal eye. If there is suspicion of bleeding or hematological disorders, counting the number of blood cells and/ or testing appropriate tests.

Stop using Clopidogrel 5-7 days before surgery or coronary artery bridge (CABG) if the anti-platelet aggregation effect is unwanted. Aspirin drugs, other NSAID drugs can cause similar injuries, therefore should be used carefully in patients using clopidogrel.

Use cautiously because it is possible to bleed the organs in patients with severe liver disease.

Be cautious when taking medication in patients with severe renal impairment.

The ability to drive and operate machinery

clopidogrel does not significantly affect the ability to drive and operate machinery.

    During pregnancy

There are no adequate and controlled studies on pregnant women, clopidogrel is only for pregnant women when necessary.

    Breastfeeding period

It is not known whether the drug is excreted in milk or not. Should stop breastfeeding or stopping the drug because of the serious risk of reactions to children.

    Drug interaction

Clopidogrel is converted into metabolites operating by CYP2C19, using these enzyme inhibitors that will be expected to lead to the concentration of dopidogrel activation and reduce the effectiveness of treatment.

Aspirin: There is a risk of increased bleeding if sharing clopidogrel with aspirin, but low -dose aspirin with clopidogrel may be beneficial.

Heparin: No need to change heparin's dose or change the anticoagulant effect of heparin and do not affect the plateloon condensation due to clopidogrel when used simultaneously.

Other nonsteroidal anti -inflammatory drugs: Clopidogrel can cause gastrointestinal bleeding if used with Naproxen or other nonsteroidal anti -inflammatory drugs.

warfarin: increases the risk of bleeding.

Avoid simultaneous use of omeprazol or esomeprazol with clopidogrel.

    Clopidogrel's

metabolites of carboxylic acid metabolites can inhibit the activity of Cytochrome P4502C9. This is likely to increase the plasma concentrations of drugs such as Phenytoin and Tolbutamide and nonsteroidal anti -inflammatory drugs metabolized by Cytochrome P4502C9. Phenytoin and Tolbutamide can combine safety with Clopidogrel.

    Storage

    In a dry place, temperatures below 30 ° C.
