Uses
indications
Hansazol 40 mg is indicated in the following cases:
In the acidic environment of wall cells, Pantoprazol is converted into activity and inhibition of H+/K+-Aatpase enzyme, preventing the final step of acid secretion into the stomach heart. This inhibitor depends on the dose and acts on both the basic acid secretion and acid secretion due to stimulation, the inhibitor time of acid excretion lasts for more than 24 hours.
Pantoprazol also has the ability to eliminate Helicobacter pylori in people with duodenal ulcer and/or reflux esophagitis and infected with such bacteria.
Pharmacokinetics
absorption
Pantoprazol is quickly absorbed, orally organized about 77%. After only one single dose of 40 mg pantoprazol has achieved the maximum concentration of the active ingredient. The average peak concentration in plasma is about 2-3 PG/ml, reaching about 2.5 hours after drinking, and does not change even after using many doses. When using repeated oral dose, the pharmacokinetics of Pantoprazol similar to when using a single dose.
Distribution
98% Pantoprazole is connected to plasma proteins, mainly albumin. The distribution volume is about 0.17 I/kg, distributed mainly in extracellular fluids.
Metabolism and elimination
Pantoprazol is metabolized in the liver thanks to the cytochrom P450 enzyme system, isenzyme CYP2C19. The majority of metabolites (about 80%) are excreted through urinary tract, the remaining amount eliminated mainly through feces. In both plasma and urine, metabolites are Desmethylpantoprazol, which is a form of sulphate and is not active. Pantoprazol has a short -term waste time, about 1 hour. However, the inhibition of acid secretion, once achieved, still lasts after the drug is excluded from the circulatory round.
Before taking Hansazol 40mg Hasan Treatment Treatment of gastric and duodenal ulcer, gastroesophageal reflux (3 blisters x 10 tablets)
How to use
Do not chew or crush pills but must take pills with water.
Dosage
Adults
Gastroesophageal reflux disease (GERD)
Dosage 20 - 40 mg pantoprazol per day for 4 - 8 weeks.
Stomach and duodenum ulcers
Dosage 40 mg pantoprazol per day. Treatment within 2 - 4 weeks for duodenal ulcer, 4-8 weeks for benign stomach ulcers.
Gastro -duodenal ulcer, Helicobacter pylori is treated in combination with 2 antibiotics for 1 week. An effective regimen including Hansazol 40 mg, 2 times a day + Clarithromycin 500 mg, 2 times a day + Amoxicillin 1 g, 2 times a day or Metronidazol 400 mg, 2 times a day.
Zollinger-Eleson syndrome and disorders increase gastric acid secretion
The recommended tolerance dose is 80 mg of pantoprazol per day for long -term treatment. The dose may then be adjusted depending on the level of gastric acid secretion. May be up to 240 mg daily. In doses higher than 80 mg of pantoprazol per day, it should be divided into 2 separate doses. There is no limit on the treatment time for Zollinger-Elesison syndrome and other disorders accompanied by increased gastric acid secretion, treatment should be maintained according to pathological requirements.Severe liver failure
Dosage should be reduced to 40 mg of pantoprazol per day, every 2 days.
kidney failure
Do not adjust the recommended dose.
Elderly
Do not exceed the dose of 40 mg of pantoprazol per day.
Children under 12 years old
Pantoprazol is not recommended for children under 12 years of age due to no treatment experience.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose?
Handling
Stomach washing, active carbon, symptomatic treatment and support. Because Pantoprazol is strongly attached to plasma proteins, the method of separation does not eliminate the drug.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and take the next dose at the time as planned. Note that it should not be used double the prescribed dose.
Side Effects
When using Hansazol 40 mg, you may experience unwanted effects (ADR).
Common, ADR> 1/100
Uncommon, 1/1000 Rare, 1/10000 Very rare, ADR Genital: Larger breast condition in men due to hormonal imbalance. Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Hansazol 40 mg is contraindicated in the following cases:
Caution when used
In patients with Zollinger-Elison syndrome and examination disorders accompanied by gastric acid secretion that needs long-term treatment, pantoprazol-like all other acid secretary inhibitors-can reduce the absorption of vitamin B12 (cyanocobalamin) due to hydrochloric acid acid deficiency. This should be considered if there is clinical symptoms.
Eliminate the possibility of stomach cancer before using Pantoprazol as well as other ppi for people with stomach ulcers, as the drug can cover signs or slow down the diagnosis of stomach cancer.
Need to check again if the symptom lasts more than 4 weeks, despite being treated appropriately
In patients with severe liver failure, need to reduce the dose or take a daily oral taking and monitor the liver enzymes while taking the drug. If the value of liver enzymes should be taken immediately.
Stomach acid decreases due to any cause - including proton pump inhibitors - increasing the amount of bacteria present in the digestive tract. Treatment with acid reducing drugs may increase the risk of gastrointestinal infections such as Salmonella and Campylobacter.
The ability to drive and operate machinery
Hasanzol 40 mg does not affect the ability to drive or operate machinery. However, showing some side effects such as dizziness and blurred vision can affect the patient's reflex ability.
Pregnancy
Clinical experience of pantoprazol when used during pregnancy is limited. Therefore, only using pantoprazol when the benefit of the mother is considered greater than the risk for the fetus and the baby.
The period of breastfeeding
Clinical experience of pantoprazol when used during pregnancy is limited. There is no information about the pantoprazol output through breast milk. Therefore, only using pantoprazol when the benefit of the mother is considered greater than the risk for the fetus and the baby.
Drug interaction
pantoprazol reduces gastric acid, thus reducing the absorption of other drugs used at the same time, which depends on the stomach pH (such as ketoconazol, iTraconazole or Atazanavir, Ritonavir).
Sucralfat slowed down and reduced the bioavailability of proton pump inhibitors. Therefore, taking proton pump inhibitors at least 30 minutes before use sucralfat.
Pantoprazol metabolizes in the liver by the cytochrom P450 enzyme system. However, up to now, there has been no significant clinical interaction report between Pantoprazol and metabolic drugs and Cytochrom P450 (such as carbamazepin, caffeine, diazepam, diclofenac, digoxin, ethanol, glibenclamid, metoprolol, naproxen, nifedipine, phenytoin, piroxyllin, and contraceptive medicine).
There is no interaction between pantoprazol and antacids when used simultaneously.
In pantoprazol -interactive studies in Pantoprazol, combined with appropriate antibiotics and no important clinical interactions are observed.
