Uses

indications

Lansoprazol 30 drugs are indicated in the following cases:

  • Treatment Stomach ulcer progresses; Benzimidazole, has no anti-cholinergic or antihistamine effect at H2 receptor, but prevents the excretion of stomach acid due to specialized inhibition on the system (H+, K+)-Atpase at the surface of the secretion of the stomach wall.

Because this enzyme system is considered as an acid pump inside the wall of the wall, Lansoprazole has the effect of inhibiting gastroentric acid pump due to the final step of the acid production. This effect is related to the dose and leads to the inhibition of stomach acid excretion in normal and when stimulated from any stimulating agent.

    pharmacokinetics

    absorption:

Lansoprazole is quickly absorbed after oral, the peak concentration in the blood is reached after 15-2 hours of taking the medication. The bioavailability of the drug is about 80% or higher at the first dose.

    Distribution:

lansoprazole combined with blood protein about 97%.

    Metabolism:

The main metabolic drug in the liver is thanks to the cytochrom P450 enzyme system with isenzyme CYP2C19 forming 5-hydroxyl-lansoprazole form and with ISOENZYM CYP3A4 forming the Lansoprazole Sulfone form.

    Era:

Metabolic substances are excreted mainly through biliary feces; Only about 15-30% of the dose is eliminated in the urine. Selling duration of plasma is about 1-2 hours but the effect of the drug is longer. The clearance in elderly patients and patients with liver disease.

    Caution when used

Using proton pump inhibitors, especially when high doses and long -term (> 1 year), may increase the risk of hip, wrist and backbone fractures, mainly occurs in the elderly or when there are other risk factors.

Observatory studies show that proton pump inhibitors increase the risk of common fractures from 10 - 40%, some cases may be due to other factors. Patients at risk of osteoporosis must be cared for in accordance with the existing clinical instructions and need to supplement vitamin D and calcium.

There has been a report on severe blood magnesi on patients treated with proton pump inhibitors for at least 3 months and most cases of treatment for about 1 year. Symptoms of severe blood magmesi may occur such as fatigue, muscle spasms, delirium, convulsions, dizziness and ventricular arrhythmia but the above symptoms may occur dull and not pay attention.

In most patients with blood maggesi, the disease is improved after supplementing magnesi and stopping the use of proton pump inhibitors.

Be careful to measure Magnesi concentration before starting treatment and periodically while treating patients who need long -term treatment or must use proton pump inhibitors simultaneously with digoxin or those that cause blood magnesia (for example diuretics).

In patients with liver failure: Needs to reduce the dose in patients with severe liver disease.

In children: The effect and the level of drug intolerance have not been surveyed. Therefore, do not use drugs for children.

In the elderly: Lansoprazole should be used carefully due to gastric acid secretion and other physiological functions have decreased.

Lansoprazol 30 contains sucrose. This drug should not be used for patients with rare genetic problems galactose intolerance, glucose-galactose malabsorption or enzyme deficiency sucrose-isomtase.

The ability to drive and operate machinery

The unwanted effects of the drug such as dizziness, dizziness, vision loss and chicken sleep may occur, reducing reaction ability.

    During pregnancy

There is no complete study of the use of drugs during pregnancy, only using drugs for pregnant women when really necessary.

    Interaction of drugs:

Lansoprazole may change the metabolism of drugs metabolized through the cytochrom P450 system such as Warfarin, Antyrin, indomethacin , ibuprofen, Phenytoin , ProPranolol, Prednison, Diazepam or Clarithromycin.

When using simultaneously Lansoprazole with theophylllin, theonllin's clearance increases slightly (10%).

lansoprazole can extend Diazepam, Phenytoin and Warfarin.

Lansoprazole reduces the absorption of drugs that depend on the acid pH such as Ketoconazole, and may be iTraconazole. With Voriconazole, the plasma concentration of both drugs increases and recommends reducing the dose of Lansoprazole.

antacids and sucralfat: avoid use within 1 hour after taking Lansoprazole because these drugs reduce the bioavailability of Lansoprazole.

Due to the lack of studies on the correlation of the drug, not mixing this drug with other drugs.
